In this episode, Dr. Shawn Tassone discusses the recent recommendations from the United States Preventive Services Task Force (USPSTF) regarding mammography guidelines. The USPSTF recommends biennial screening mammography for women aged 40 to 74, with a B recommendation. Dr. Tassone explains the incidence and mortality rates of breast cancer, particularly among non-Hispanic white and non-Hispanic black women.
He also addresses the potential harms of mammography, such as false positive results and overdiagnosis. Dr. Tassone discusses thermography as an alternative to mammography and emphasizes the importance of individualized screening based on risk factors.
Episode Highlights
- The USPSTF recommends biennial screening mammography for women aged 40 to 74, with a B recommendation.
- Breast cancer is the second most common cancer and cause of death among women in the US.
- Mammography has potential harms, including false positive results and overdiagnosis.
- Thermography is not currently considered an alternative to mammography.
- Screening should be individualized based on risk factors.

I had a breast reduction and DCIS found in rt breast, and lobular hyperplasia was found throughout my breasts. I opted for a bil mastectomy. The pathology from my left breast showed a 4mm invasive lobular carcinoma, clear nodes, stage 1, grade 1. ER+, PR+, HER-. I had a clear mammogram prior to my reduction.I was told invasive lobular grows in a line, linear. It spreads out like a spider web is missed in mammograms until it is spread in the nodes, etc. Sometimes it is not found until you are Stage 4 breast cancer. Thank God I had the breast reduction and then the bil mast. BC does not run in my family, I am a healthy petite 63 yo woman, who looks 45, had 3 children young and nursed them all, hardly drank alcohol. I had false assurance by those blasted tortuous Mammograms I got every yr or 2 since I was 40!! The Pink ribbon Industry needs to use their money on prevention or a cure. Not giving the CEO a yrly salary of $750k a yr, plus others $250k a yr. I abhor the color pink now and the month of Oct. I also am not taking inhumane Hormone Blockers! Menopause is an ongoing thing as it is. Back to using small amt of bioidentical creams. Med Onc is not happy with me. Women deserve better. I was told it is 1 in 7 women after 50 now have breast cancer. WTH!!

Once, I had a “concerning” mammogram and I was called in for a closer look and they did a sonogram. So, why don’t they just do a sonogram instead of a mammogram every time? Now I get a thermogram instead and never have a false positive or concern.

Ladies, please only have ultrasound if you haven't had children. I had cystic breasts and a mammogram caused them to burst. Within a week I noticed my right breast changed. I ended up with breast cancer and had to have a mastectomy. Why torture us, when we can have a non invasive ultrasound?

I have asked for ultrasound only. Dr won’t prescribe it because she says the med system is set up to use it only as diagnostic, not for screening. I’ve had two mammograms about 15 years apart. Never again. Way too painful. It can’t be good to crush the body that way.

@@salviaprezzemolo2165 Please look at my comment under healthyteddy as I don’t want to be redundant. I’m with you on this going forward for me. So happy you are doing this. I was not given a choice, so I’m searching for another medical specialist and medical facility who will listen to me and respect my wishes for ultrasounds only. There’s a company which I hope expands called Herscans, who does ultrasound breast exams, that visits different cities. I’ll be contacting them. Thank you for sharing about your experience. I wanted to add that dense breasts does not correlate with not having children, as I have dense breasts too, and I have two adult children and I had 3 miscarriages. I was told by a doctor that it is genetic. God bless you always!

Tomorrow I will be 70. I had a mammogram last month. Last summer I went over to talk with our neighbor because I had not seen her in a while. She told me she had breast cancer. She was not going to have any more mammograms but the doctor said to have one more. She was 76 at the time. She underwent treatment and is in remission. I was thinking about not having any more but after what happened my neighbor, I think I will continue for a few more years.

About ultrasounds: I had mammograms that detected something that needed further assessment. In these instances, ultrasounds were done to precisely detail a calcified cyst in the first one and nothing present in the second exam. These were done years apart on alternate breasts. I questioned the radiologists each time as to why the ultrasounds, which were more precisely accurate, aren’t done instead of the mammograms. It only made sense to me considering the results. I was told that they follow protocol. I’ve since heard that there are medical specialists who do perform the ultrasounds in lieu of the mammograms. I will be researching this before my next breast exam.

Because each modality picks up different things A mammo takes a picture of the entire breast , can show microcalcifications ( ultrasounds can't) , asymmetry.... for exp. An ultrasound can be used to determine if a finding may be a fluid filled cyst or a solid mass.( an example ) I have both done yearly.

I get Private ultrasound's! Herscan is 225.00 your ultrasound is looked over by a radiologist too! Also I already went through 7 years of over radiating my breast with false positives over & over! I hired a Private surgeon And had a golf ball sized calcification- removed from my left breast! The Radiologist through the hospital said to me the day I went in for surgery to mark my breast with a wire before the surgery.. But the radiologist says to me while marking my breast. I'm 99% sure you don't have cancer! They could have told me that 7 years prior. And not put me through a shit ton of fear and panic! I've had it with the fnn" medical establishment!

I’ve discovered HerScan on line. No more getting irradiated by mammograms that were unable to properly detect and diagnose two calcifications and one fibrous cyst that the follow up ultrasounds were able to. I questioned the radiologist as to why I just couldn’t have the ultrasounds as they accurately identified the issues, and I was told, “because having the mammograms first is the protocol”. Not for me anymore.

Are dense breasts considered a high risk for yearly mammo's?
@lizstraub6621

I remember the day I sat waiting in the very posh radiologist's office, and had been handed a print out (first ever, and I had been going there for years) about "Dense breasts and the use of ultrasound" and I knew then and there that I would be told that I had dense breasts and wouldn't get out of there without an ultrasound as well as my mammogram. Then came the false positive, then the biopsy, then the cessation of HRT...still have a nasty scar from the biopsy and I remember when I was finally able to drive again and I put my seatbelt across my chest, it hit EXACTLY where the "Suspicious" nothing-burger had been! It crossed my mind that I had been putting a seatbelt across my right breast regularly since 1979 and maybe it was just scarring, or toughening of the tissue in that spot. I'll never know. I just live with a mutilated breast and zero HRT. Go figure, but now I have other issues because I have been denied hormones. It's insane.

@@lizstraub6621 I am sorry to hear about your experience. I have not had a mammogram in years to similar reasons. Underwire from bras causing spots on the sides of my breasts is what I believe happened. Stopped wearing bras, stopped getting mammograms and did the research which indicates mammograms do not change outcomes and can cause many unneeded procedures. And many countries have stopped doing them altogether (Switzerland for one).
