I found the amnesia question interesting as my family had a woman show up at our door late one night asking if she could pet our dogs. She didnt have amnesia, she had some type of dementia. We let her come in and meet our dogs and cats but ended up taking her to the ER as it was near midnight and we had no idea who she was or where she lived and sadly neither did she. They managed to get enough information from her with various methods at the ER (it really is amazing the way the brain and memory can work) and they were able to contact her family and we met them after that. We played impromptu host to that sweet lady several times after that as she likes dogs and something seemed to know we were safe.

I was explaining my vision problem to a resident and when I said I had cataracts as a child, she cut me off and asked "Why did you have cataracts?" The tone she used made it sound like I chose to have cataracts. I felt like saying "Because I wanted to be different. Everyone else has normal eyesight and I wanted to stand out." Some doctors have terrible bedside manners. When my father was in the ER, the on call doc came in and nonchalantly dropped his clipboard and binder on my father's legs like he was a piece of furniture. My dad cried out in pain and his body stiffened up. My father's legs were sensitive due to diabetes. The nurses in the room couldn't believe what this guy did and after he was informed of my father's condition, he leaned over and put his hand on my dad's shoulder and in a not so convincing tone, said "I'm sorry". Later when my dad was up in a room, that same doctor was discussing a DNR with my mother, just a few feet away from my father who is freaking out because he can hear what's being said and is scared. I really wanted to punch that doctor.

You should see cook county ER which was renamed to stroger. They see on average 4 to 5 patients an hour. During the Orlando mass shooting incident over at Orlando regional, it was like 7 to 8 an hour. The doctors that night never had a mass casualty situation before and it was a trial by fire for all of them. They literally did it but also completely rewrote their policies right after to at least help in future incidents. It was literally all hands on deck for 8 straight hours maybe more. Every operating room was full and the ER was completely packed, 14 operating rooms completely full. They had makeshift operating rooms in the ER of all places. it was crazy.

I figured out how to skip the line in the ER. I showed up with blood pouring out of every hole in my face into a large metal dog bowl that was sloshing and filled with blood and squeaked, "Help me," and then kind of fell over, with the bowl spilling all over the floor. According to my partner, who had driven me there, I got right in! I do not remember that part. 😂 Meanwhile, when I once came in with a shattered spine, I had to chill for about 6 hours. So, yeah, blood helps.

Yeah, that thing about having clothes cut off is real. Even when there’s no time crisis, they just whip out those scissors and snip away. I’ve had expensive slacks cut off me - no injury to my lower body and I wasn’t going to die if I didn’t get treated immediately - and I got really mad at the ER staff for doing that. I’m poor and disabled and can’t afford to be buying nice new clothes, much less paying for ER visits and ambulance rides. And I had no pants to wear for the Uber trip back to my hotel, where I was staying for my niece’s wedding. I guess that’s what I get for allowing my family to pay for me to come to a family wedding in another part of the country….I caught norovirus, went into dehydration-related kidney shutdown, passed out while sitting in the waiting room of a minor emergency clinic, and awoke to find myself in the ED at a real hospital….with my nicest pants in two pieces.

We shocked one patient 169 times in a 12 hour period. The patient was coding almost continuously and the family kept screaming to code the patient. It took that long to get legal to allow us to stop the code and finally pass on. This was about 25 years ago.

I work on ambulance. I've seen people shuffle into the hospital, skip triage, and get rushed to CT for suspected stroke in under 10 minutes. On the other side of the coin, I've transported patients that smugly said they only called an ambulance to get seen faster, "transfered care to waiting room" is one of the most common things I document.

I went to the ER at 10pm one night for some abdominal pain. It took 10 hours to get seen where they just said "it's gas, drink a lot of water". I'd say I'd like to skip the line but there was nobody else in the waiting room.

Piggybacking off the mental health question, are there counselors available for physicians who see patients under their care die? How do you handle those feelings of, "Could I have done more," or, "Could I have done things differently to keep the person alive?" I’m super sensitive so a profession like this where death can sometimes be inevitable I think would eventually be too much for me to handle.

With meditation, especially ones for specific goals like handling potentially traumatic experiences, it's very important to fully immerse yourself in the exercise, almost literally getting lost in it. If you don't, you can lose sight of what you're meditating on. I have an undiagnosed pain disorder, likely stemming from the brain. My pain can randomly spike from a 2 to a 7 in an instant sometimes. I cannot function without meditation, period. I know several others who feel the same way for various reasons. Do not be afraid or be embarrassed to meditate. You do not need to toughen up and embrace everything you see and feel. Even the most simple of exercises can have a profound impact on your ability to handle trauma.

Deaf Interpreter here: Sometimes the hospitals would hire Deaf Interpreters because the patient is unable to express in sign language or in tons of pain. The ipad interpreter might not be perfect because of spotty wifi, limited view space, and the deaf patients cannot see well especially post surgery. There are mixed feelings of using ipad interpreters or Video Relay Interpreters. Better have live interpreter in person to see the full action. I have worked with a mix of ipad interpreters and live interpreters. I would prefer live interpreters easier to capture but ipad interpreters do it in a pinch. Some hospitals are lucky to have STAFF ASL interpreters in house.

With regard to the mental health of staff, I'd suppose that'd be relative to what situation they work in, recently it's ebeen found here in the UK that there's a major problem with untreated mental health conditions with NHS workers, be it the general workload and under-staffing, to the fear of whether the damned hospital will fall apart around them, as has happened where large chunks of RAAC (Reinforced Airated Autoclaved Concrete) have broken away from a ceiling and srtuck a doctor, breaking their leg, Conservative Britain everybody, where saving lives could cost you your own...
