Acute phase reactants (APRs) are substances that increase in the plasma in case of inflammation, but their plasma level decrease when there is no inflammation (or when the inflammation subsides).

Please dont ignore me i need help because my mother is 60 years old negative CRP negative ANA but ESR 111 and has been like that since 19 months ago. She is diabetic and have hashimoto but 19 months ago she was so active and just one night began to cry like a baby girl with chronic pain in her neck and shoulders, days after she cant even raise her arms and it doesnt happen yet, presented chronic pain in both sides of their lower and upper extremities and had a muscle biopsy with negative results but although she is taking Methrothexathe 2.5 mg 4 pills am and 4 pills pm once weekly during the last 5 weeks her ESR increased from 81 to 111. We dont know how to help her. Now she is using an electric position bed for sleep and has lack of mobility. She's not the same anymore. I trust God in the Name of Jesus Christ for Her Healing. He is my strenght. I just only ask anyone here because last year she had MRI from cervical, lumbar,brain and knees but her pain doesnt go away although every mri result was normal except from a severe lumbar radiculophaty. She also got tested for arsenic and was high but we are in Puerto Rico and even her oncologist who said she is normal in other tests and no evidence from malignancy, even he doesnt know how to treat her high arsenic. Thats the only issue that we dont treat yet but she had a reumathologist, oncologist, neurologist and physiatry and no one help her or found her diagnosis. Please does someone here could give me and advice or recommendation? She was pretty active and now she cant touch her own face because her arms seems to be stick to her torax. She had good apetite and a lot of dreams and purpose to reach. I apologize because my english domain is pretty poor but i tried my best. Help us please! God bless you all.

CRP is by definition, REACTIVE, so it both builds and dissipates rapidly. The sed rate is by nature, SEDENTARY, so it is slow. That's the mnemonic. The supporting facts are that the half-life of CRP is 19 hours, while in contrast, the half life of fibrinogen is FOUR DAYS and the half life of IgG is TWENTY-ONE DAYS!!. Those rouleaux are not going to be able to unstick themselves from each other for a long, long time. I think of CRP as carry-on luggage, and rouleaux as a big stack of luggage that has to be checked.

Crp will normalize first because esr is an indirect measure for fibrinogen which has a longer half-life than crp

Why does macrocytosis cause increased ESR? I think the reason you gave is wrong as larger the cells, the faster it sediments. Hence high ESR. Nonetheless, great video!

CRP has a doubling time and decay time of about 6 hours and the maximum concentrations are reached in less than 2 days So after the inflammation is resolved or treatment cause reduction in inflammation CRP drops rapidly. ESR by the way is affected by many factors like the concentration of Albumin in plasma and the size and shape of the RBCs which takes longer to be substituted. I think that’s why CRP drops faster

The answer is CRP as it's half life is less than Fibrinogen and IgG. One thing need to bring in focus, there are a lot of Inflammatory Rheumatological conditions not start with symmetrical arthritis. Even through their whole course of illness, some remain as asymmetric arthritis as their onset. So Inflammatory arthritis can be either symmetrical like RA or asymmetrical AS or other seronegative arthritides . some seronegative arthritides can be both symmetrical or asymmetrical like psoriatic arthritis. At about 5:34 min. "Macrocytosis (larger is slower)" not right analogy. The larger, the quicker to sediment. Remember that ESR increases as components are descending the column. Faster sedimentation rate = increase sedimentation rate. The same holds with microcytosis in which the ESR is decreased. This is particularly true with Hb C (sickle cell trait) patients. Again smaller is faster is not a good analogy. I really enjoy your videos!

We will rely on CRP during short term follow up because ESR continues to be in the blood months after resolution of the cause

CRP will normalize first before ESR. because ESR is a general and longer process. While CRP is a more specific and shorter process.

I guess there's a mistake in the mnemonic for macrocytosis and microcytosis macrocytosis can increase ESR (Erythrocyte Sedimentation Rate). Macrocytosis is a condition where red blood cells are larger than normal. This can be caused by various factors, such as: 1. Vitamin deficiency (e.g., vitamin B12 or folate deficiency) 2. Liver disease 3. Alcoholism 4. Bone marrow disorders 5. Certain medications When red blood cells are larger than normal, they tend to settle more quickly to the bottom of the test tube, resulting in a higher ESR value. This is because larger red blood cells have a greater volume and density, causing them to sediment faster. In addition, macrocytosis can also lead to an increase in ESR due to: 1. Increased rouleaux formation (red blood cells stacking together) 2. Increased aggregation of red blood cells 3. Decreased red blood cell deformability As a result, macrocytosis can artificially elevate ESR values, potentially leading to misinterpretation or overestimation of inflammation or disease activity. It's essential to consider other laboratory tests and medical evaluation to accurately diagnose and monitor conditions.

Is % of each white blood cell type (neutrophils, lymphocytes...) more important indicator than total number of each group? For example my neut and lymph % are always slightly off, but they are within the reference ranges (total numbers).

ESR is not a very accurate test. It also depends on the degree of active inflammation.

Hello, My husband heart rate is 126 , he was given a heart monitor to wear for 2 weeks. He has hbp, high cholesterol and his ESR. 57 and CRP 31.6. Any advice? I am feeling a little anxious now.

My CRP is 13. My ESR is 6. I was diagnosed with delayed pressure urticaria. (Finally). After a myriad of labs, the only two that are out of normal range are my CRP, and C1q. Oh, also was told I have NAFLD. My doc has no idea why I’ve developed pressure urticaria. What’s up with my CrP, and C1q?

At 5:31 you can see the list of things that increase and decrease esr. Why is it that macrocytosis increases ESR if it slows the cells down? Shoudn't it be decreasing ESR? And likewise goes for microcytosis since it speeds the cells up

This video is awesome! Thanks for sharing. My ESR is 121, and my CRP is 14.8. PCP cannot figure out why. Negative ANA. My WBC is 11.9, neutrophils 9.5, and platelets 399. I have been sick past 2 weeks, night sweats, fatigue, have lost about 20 pounds in a week, weakness mainly in legs. Had nausea, vomiting for 6 months on/off (when this was worked up my CRP at the time was 0.4). I had nausea, vomiting, for 5 days. Noted a lump in chest while taking a shower. I had fevers for over a week up to 102, but the past 3 days I have had no fever, and when I had labs drawn I was afebrile. COVID negative. I had CT yesterday and one radiologist noted a mediastinal isolated lymph-node and minimal pleural effusion, however the radiologist who actually did my report did not note this. Hopefully I will get some more answers soon.

Update: more of my labs came back! Haptoglobin and PCR high, 457 and .33 LDH normal. Blood cultures normal, complement c3,c4 normal. Magnesium phosphorus normal. DNA antibody normal, chromogranin normal...my 24hr urine for metanephrine was elevated at 668 (but not four fold) only symptom I have there is tachycardia and sweating, no flushing, no high BP, no headaches, no tremors...I don’t think elevated enough to mean anything, maybe just from stress. I am to see hem/onc on the 29th but hate to wait so long, however he is the best in town so I don’t mind. I have been told I may need a bone marrow biopsy....(I am terrified because I work in medical field and know that ish hurts!), but I will come back to update. Your videos are so awesome and I have been watching them because I love learning new things especially in hem/onc. Thanks for what you do! ♥️

My WBC is 19.6 High, Neutrophils is 16.0 High, Monocytes is 1.4 High, Immature Granulocytes is 0.12 High, CRP -C-reactive Protein is 153.6 High, ESR Erythrocyte Sedimentation Rate 51 High, Glucose 104 High this was put on my chart on January 27,2024. I'm 66 average weight non-smoker, but I was told my lungs show 2nd hand smoke. Both my parents were heavy smokers. I have severe stomach pain. Extremely tired, sleeping all the time. Hot and Cold Flashes. They order a CT scan with IV and oral contrast to follow-up with a colonoscopy in 4 weeks .The test were ordered before my blood work came back. I have ASAMA... any thoughts?

"larger is slower" and "smaller is faster" go against the reasoning of the ESR, as in that case macrocytic anaemias would have low ESR and vice versa.. Maybe you meant "larger is HEAVIER" and "smaller is LIGHTER".

There are more than 30 causes of fever. And there’s also fever of unknown origin. I don’t know 🤷‍♂️ Please talk to your doctor!

Actually, the heavy macrocytes sink rapidly like the proverbial "lead balloons", which increases the ESR. In contrast, the light microcytes drift down slowly like snowflakes, and that increases ESR.

I agree. Full stop his slide on esr is not all correct. Dense rbcs (microcytic) fall faster than fluffy rbcs (macrocytic) and that overpowers the negative charge concept (something my instructor never touched upon btw)

Mine is 4 mm and dermatitis said I have psorasis.All my flares are gone and only had them once..no knees or elbows. My dermatitis may be quack

ESR 32 and CRP 10. 4 doctors said nothing to worry about but no one actually tells me where this is coming from .. is ESR and CRP always an indicator for cancer? Flow Cytometry was good, ANA was negative.

They are taken in context with clinical picture and other investigations, then judged by doctor, so thats according the doctor who tells you.

@@tammyque1506 we've been trying to figure that out. Iv been tested for lupus negative. And my ANA always comes back negative. And iv had 3 done. One being recently. My heart rate is always low as well. I even make the finger thing go off at the doc. But they just say well, looks like that's normal for you. And they don't seem concerned. 🤷

@@tammyque1506 the rheumatologist I saw said well all results are normal, you don't have autoimmune. Your inflammation is a little elevated...so let's put you on gabapentin. She lost me at this point. Gabapentin? Seriously! Disgusting addictive weight gaining seizure medicine? And a little elevated? Seriously? How about let's find out what the root cause of my inflammation is and treat that. Not just cover it up. If I don't have autoimmune fine. Great. But something else is causing me to feel this way and be sick. And I'm riddled with inflammation. 😮‍💨 So exhausting... I just want help... But nobody wants to help ppl anymore.. just here.. here's a pill.. then that pill causes other symptoms and then here's a pill for that. 😖 So frustrating. The only thing I take is armour thyroid 30mm. That's it. Ibuprofen for the really bad days. But hardly ever.

@@Redvixion_123 I'm not a doctor but my labs are similar to your and I see a rheumatologist. He said, 'I have some kind of inflammatory problem. Right now he wants to put me on biological meds but I have no definite diagnosis. I told him no meds until I have a definite diagnosis. He said, it looks like Rheumatoid Arthritis with negative serum but can't be sure. My labs goes up and down never stays the same. The doctor said,"it goes up during a flare up." I'm scheduled for a kidney biopsy because my GFR is 45. I don't have hypertension and I'm borderline DMII. Guess we shall see what's up. Also I am a combat veteran who was exposed to hazardous chemicals. Doctors today are not quick to give out diagnosis as compared to 20 years ago. Seems like We're headed more towards socialized medicine managed by our government.

I feel like we are just numbers to them and they are trying to make their numbers for big pharma. 🙄 It's funny when I smoked pot my inflammation was next to none. But I didn't like being altered all the time. Just hoping I can find the answers. I have cptsd and ADHD which I know mental health can play a big role in your health as well. So who knows. I try to eat right and take care of myself The best I can. So when I do go to the docs I can be like yes I do. No I don't. Etc. Because, there's no magic pill. But when you feel like death warmed over all the time something is definitely wrong.
