Have you ever wondered if staff faint in the operating room. Well it does happen. It is frequently a fear of medical students who are starting on their surgical rotations. With experience anyone can get over this reflex. In this video Dr. Hilsden discusses this common phenomenon. He shares his own experience and what still makes him "squeamish"

Seasoned OR nurse here, and fainted while scrubbed during a routine case. I now wear my compression socks consistently. And FYI, coffee is not breakfast 😆

One thing I didn't put in the video because I wasn't sure of the evidence is the role of fatty foods. Fighter pilots when they are doing high G force training will often eat fatty meals to increase the blood viscosity so they don't black out at high G's. So a proper meal, with a little bit of fat may help out.

Once again, a great medical explanation of vasovagal syncope. Also, I sincerely appreciate you being vulnerable. I’m almost done with my first year of medical school and even though the culture is starting to shift, showing any signs of vulnerability almost makes you a target. With that I must say hats off for going against the norm in medicine.

It’s really important not to lose your humanity in all of this. You go into medicine because of the humanity, but the reality is the practice can make doctors cold and detached. Thank you for the comment!

Looking at the pros/cons of different fellowships options (gonna keep an open mind but currently focusing on CT, trauma and surgical oncology). I'd be interested in why you chose Trauma over other potential fellowships. Thanks Dr.Hilsden

Not a doctor, but I used to faint during routine blood draws even when reclined with my eyes closed. I watched and rewatched tons of phlebotomy training youtube videos and worked on tensing my leg muscles (like fighter pilots when they’re experiencing lots of G forces). Now it’s not a problem for me. Exposure definitely helps.

Eating breakfast before going into the OR should be emphasized. Almost every classmate who fainted in the OR had skipped breakfast. It must be a combination of hypogylcemia and adrenaline

I suffer from these vasovagal episodes. Have had 14 in total so far. None of mine were from like sight of blood or anything like that. Mine were a little more random. Mime were from.... blowing my nose, taking my dog for a walk, 3 different bike rides, 2 different times of just walking into a room, 3 of them in the first two days after a spine surgery, 2 from being sick, and 2 from getring spinal injections. Some kf them i did full blackputs on, some i just drop in and out of consciousness, and the rest i just get really weak, along with thenrest of the symptoms, pale skin, drentched in sweat, heart either racing or just beating hard, struggling to breath, shaking, hot cold flashes, and my pulse spikes, and bp drops. Lowest recorded bp was 64/48. I also havensome other health issues. Have had some tests done, and getting some more tests done. I go for a tilt table test in a few weeks here. I did get a service dog. Mainly for these episodes, but she also helps me with other medical conditions too.

Thank you so much for this. I’m an M3 and am currently getting worked up for what my cardiologist is thinking could be vasovagal syncope due to standing for long periods of time (all the lifestyle modifications I have tried have not worked so far). This has been a huge fear of mine going into my surgery rotation in December.

Recently I experienced my first ever exposure in Amputation. I had syncope episode after seeing and smelling of electric knife cutting. Is there anyway I can improve myself ?

Idk if you have talked about mental illness in healthcare but I think that would be an interesting topic. I have wanted to be a surgeon forever but have recently started treatment for depression and am curious as to how it will affect my ability to get into medical school, residency, etc.

Also I know many surgeons who are obviously suffering (depression, burnout, anxiety) but refused to seek treatment due to how it may affect their reputation.

What about fainting from being pregnant ans standing for 12 hours and forgetting to eat?

the past 2 times i've had any type of blood drawn, i hadn't eaten in over 24 hours (yes i did this twice expecting different results, because i am a little bit stupid.).... those were the closest i've ever come to completely passing out. i have a horrific fear of needles, and i was so stressed out and nervous about getting it done that i felt dizzy before i even walked into the hospital. i've had pre-syncope episodes before (thanks for the terminology btw, now i have a word for it that isn't just "felt dizzy and couldn't see/hear clearly") during panic attacks, but nothing as severe as what i experienced after getting my blood drawn on an empty stomach and then standing up and trying to walk out of there as quickly as possible. pro tip: if you don't have to fast before bloodwork, don't do it! and if you do, don't try to run immediately after! take the damn OJ and goldfish crackers
