Healing From TMJ and Chronic Pain: My Journey as a Myofunctional Therapy Patient

My deep passion for speech language pathology, and specifically Myofunctional Therapy, was born from an undiagnosed orofacial myofunctional disorder (OMD) that impacted my life for over 20 years. Like some patients with OMD, I suffered from debilitating TMD pain, chronic headaches and upper back and neck pain. The pain was so severe and frequent that it impacted my ability to exercise, socialize and fulfill simple activities of daily living. I saw over 25 specialists in the course of 14 years and while some interventions temporarily alleviated the pain, I never found a solution that significantly remediated the pain long-term.
This is my story of living with chronic pain, becoming a Myofunctional Therapy patient, undergoing treatment, and experiencing an 80% reduction in chronic pain and TMD symptoms. My problem started when I started to lift weights and lots of stress I tried chiropractics and acupuncture but is not healed completely it sucks

This is amazing! Can myofunctional therapy work for bruxism-caused TMD? I've had braces twice and still suffer from neck pain, forward head posture and headaches. I'm afraid if I do braces a third time, my teeth will die from the movement trauma!

Hi Cara, it is not uncommon for patients to see changes in TMD symptoms and clenching and grinding habits when they undergo OMT intervention. As noted by the Academy of Orofacial Myofunctional Therapy: "Clenching is closing the teeth tightly, often for prolonged periods of time. It's a normal function when bearing down (as in weightlifting or constipation). The habit of clenching damages the teeth and the temporomandibular articulation; causes chewing muscle dysfunctions and pain. Bruxing is grinding of the teeth, usually at night and it’s sign of sleep disordered breathing. OMT provides awareness of the habit to the person who clenches; helps identify the connections between stress and clenching, as a stress management parafunction; provides strategies to reduce or eliminate clenching; identify some symptoms of disordered nasal breathing, which is often the main contributor to bruxing, or other symptoms of OSA, and make the appropriate referral." I am based in UK so obviously don't have direct access to your services but like you I have gone from specialist to specialist and done the research to no avail. I am waiting to see a tmj specialist and facio maxillary doctor here and I don't know if you know about the healthcare system here but bc of covid there are massive waiting lists. I was wondering if you had any connections or recommendations for people in the UK? I would greatly appreciate it

My scope of practice as an SLP focuses on the structure and function of the orofacial complex. That being said, diet can have certainly have an impact on inflammation - I'd recommend following up with your doctor.

Can a maxillary cant, cause by a slightly asymmetric bite, cause TMD?

This is a nuanced and complex question, as many factors impact maxillary growth and development. These factors can occur as early as infancy (e.g. early feeding habits, prolonged pacifier use, sucking habits) or later in life (e.g. development of chronic allergies, development of chronic respiratory conditions that impact breathing habits). TMD is complex and manifests in a variety of ways. Resting tongue posture, resting jaw posture, eating habits, drinking habits, and breathing habits can all contribute to TMD symptoms but aren't necessarily the "cause." The "TMJ Healing Plan" by Cynthia Peterson has been a helpful resource for many of my patients.

I’ve been suffering from TMJ for the last year. I got 3 wisdom teeth removed and frenectomy last May and weeks after I noticed extreme pain in my face and body. Constant neck and shoulder pain, facial pain, facial twitching, list goes on. Oral surgeon never mentioned how crucial myofunctional therapy is before a frenectomy…I have been wearing a splint 24/7 since December, see a myofunctional therapist weekly, and just started PT a few weeks ago. How long did it take for all the symptoms to be cured?

I'm curious, was your tongue thrust ever related to a tongue tie?

Tongue thrusting is typical for typically-developing children 0-6 months of age. By 9-10 months, children transition to typical swallowing posture, although oral habits such as prolonged thumb sucking, prolonged bottle and pacifier use can reinforce and prolong tongue thrusting. My tongue thrust was not related to a tongue tie but a prolonged oral habit and atypical oral rest posture that went undiagnosed for many years. Within 5 weeks of therapy, I was swallowing saliva, liquids and solids of various consistencies without tongue thrusting. It's also interesting to note that tongue thrusting's genesis is not specific to early childhood. A tongue thrust can develop in adulthood as well. I've worked with patients who've developed a tongue thrust as early as their teenage years and as late as their 60s

@@JAYNICOLEE77 Its getting better now feeling much more confident. If someone is expreriencing what I have then I sugeest you to try resisted openings but put the fist more to the weaker side. That really helped fix my tmj now my mouth is opening 3 fingers wide I hope that helps someone

Hey my name is David I am 17 years old. When I swalow my ears cracks, my left jaw opens wider when I open my mounth. It just started to hurt a littel bit a few days ago. I am relly scared that it will get worse. Will it ?

Hi David, the TMJ joint sits very close to the Eustachian tubes. For patients who present with co-existing ear and airway issues, I often refer patients back to their doctor and for further evaluation with an otolaryngologist (i.e. ENT) to further assess the ears and airway. ENTs are often the first to formally diagnose a patient with TMD and can write referrals for therapy to address pain symptoms. They can also further evaluate your structures and functions to ensure that the pain is not the result of a more complex issue.

@@daniellekleeslp4625 Well thanks for responding. Yesterdey I want to a TMD doctor end he told me that I will need to wear some night guard end after that they will put braces. Because I don't bite how I sopousd to end my left jaw it's in a wrong position. He said that my ear cracking shoud go awey end probebli my Tinnitus to. PS: You look like a very kind person!

Hi T, the TMJ joint sits very close to the ears. Referred pain is not uncommon. For patients presenting with ear issues, breathing problems or structural anomalies, I frequently refer patients for further evaluation with an ENT. ENTs have the ability to more thoroughly assess the ear, nose and airway and ensure that there aren't any underlying issues

Ear pain can be secondary to/correlated with TMD. It wasn’t a part of my personal experience. I’d recommend following up with you PCP for further assessment.

Hello I suffered tmj since I was 18 when I had my first panic attack because thr headaches where so bad to the point it was feeling like shocks and pressure😔im 23 now n my life got harder and harder everyday it hard and I'm trying to survive everyday through the pain amd all kinds of worse problems my life hsrd im even suffering from stomach problems now to the point i use to weigh 150 n now 113 because it hard to eat Im ready to cure my tmj I'm so tired aand in all kinds of messed pain I pray I get help I pray I can keep working I barley started work today n after work I'm in a lot of pain and earblockage and can't eat ugh its time to cure this I hope I. Can😔

I learned that i prolly have TMJ. But not how to relieve it naturally like i typed in the search. I know theres some herb or remedy besides petrochemical poisons that could help. I just cant find anyone who knows what that might be.

There are a variety of different ways to address TMD symptoms. Orofacial myofuncitonal disorders are often co-existent with TMD symptoms. For many patients, addressing oral posture and swallowing dysfunctions can decrease TMD symptoms. There is peer-reviewed research that discusses this further.
