A teenage girl (Hayley Chase) comes to the hospital displaying classic smallpox symptoms, which the team dismisses as impossible until her step-father (Andrew Fiscella) gets sick as well. Once the CDC intervenes and isolates the patients, the team is unable to explore any further until House takes a life-threatening risk.

The role of the CDC guy who tells House that he can't move him is such a mixed bag... On one hand, the actor is fucking phenomenal and totally manages to convey the seriousness of the situation, while also showing that he has human emotions but can't risk having a disease like that spread. He comes off like the villain of the episode, but he was literally just doing his job exactly like he would have been trained to. I'm really glad the writers included a redeeming scene for him where he instantly allowed House and the surviving patients to be released from quarantine and treated for R-Pox the second there was proof in the S-Scars. Rough role to play on a show like this, but performed impeccably.

@@roguegirl29 Well said. Yeah calling him a "villain," doesn't really seem fair considering he was literally just doing his job by the book to prevent a potential larger outbreak. His role was essentially written in order to give House someone to clash with, therefore building drama and tension. I can't even begin to imagine if an actual doctor acted the way House does in this episode to CDC employees... probably would have left the hospital in handcuffs in real life, but I digress. Either way, using the word "villain" to describe his character seems absurd to me now after reading your comment. Don't really know what was going through my head there when I originally wrote that comment. Have a good night dude!

@@GBRyker61 Yup. And in a way that showed near indoctrination level assertion. Like how it was stated, he wasn't even near any of the patients and went on a summary of what they knew. It actually bothered me because when House went to examine him, he was essentially taking steps to destroy evidence just so he could be right. That's how it looked the first time I saw this episode and every time I've sen it since. It's like he was following procedure more than having had the experience.

Made me angry. It would be possible but extremely expensive. There is no airlock. Normally you would leave the room into the airlock and be showered by something that kills everything. They would have to bring in a CDC response team with mobile decontamination and scare the shit out of all the patients. But mostly money. Seems they are super cheap with the suits too. They make suits that can get oxygen refilled with no risk, again used by the CDC and again, expensive lol. Irl, I imagine an oxygen tank inside a bucket of bleach (or something stronger) would work as a makeshift solution. Go to the corner of the room away from the patient, submerge your gloves/arms, spray down that area of the room with bleach, spray down the connection, remove tank, pull bleached tank out and connect (and hope to god it only takes a second or two), try not to puke in the helmet from the smell.

In reality, a doctor at CDC Atlanta would be assigned as liason to local health and would report upchain on the cases until the alert level reached escalation. No boots on the ground until the threat starts spreading as budget limitations limit responses. If local health has it under control the CDC will backseat and backstop and receive evidence from local for analysis. Even at the height of recent Ebola out breaks, the CDC hasn't sent teams to Africa. They rarely even send one person. During this current pandemic, they don't send teams out, they have doctor/researcher/caseworker teams that analyze data and samples sent from the field. They do great work but have been hampered by budget cuts and political hostility which has caused low morale. They still do great work.

The CDC doctor is played for the villain in this episode, but he is 100% correct. We don't vaccinate for smallpox since the 70s. They CANNOT risk letting smallpox out, even if that means a few people have to die as a precaution. Smallpox killed 10-15 million people a year well into the 1960s. And unlike the people back then we're not vaccinated and we're virgin territory for smallpox. A smallpox outbreak would be a MASSIVE catastrophe - it would make COVID look like the sniffles. Smallpox kills around 30% of the people it infects, more if they're young. Of the survivors, many are left scarred and blind. You would NEVER risk any kind of smallpox outbreak to save one doctor, or even one doctor and a couple of patients. It's callous and cold, but the math just isn't there. Smallpox gets out and millions, maybe even HUNDREDS of millions, will die.

The patient exhibited classic symptoms of small pox. Even though the girl didn’t develop any pustules, the CDC had to treat the situation as if it was an outbreak of small pox because of the evidence. The team’s counter-evidence was weak to say the least and if they had been wrong, the girl would have died even faster and House would have definitely been infected instead of just possibly. The Doctor was doing exactly what he should have with the evidence and data he had. The need to quarantine and treat the disease as what is evident outweighs the mere possibility it could be something else. We know house’s team is right about 90% of the time but if this was a real situation, it would never had been considered and the girl would have died.

House’s team and the CDC were both in the wrong. The CDC guy bot having his workers check the corpse for any signs of some other disease and quickly choosing to clean the body was a major mistake that should not happen in real life. Yes they wanted to get House out of there but he would have been completely fine another 1-2 minutes to examine the man’s corpse for any odd markings that didn’t match smallpox. Master’s reasoning was also extremely weak as the cat losing all of it’s fur could have been something entirely unrelated to R-Pox. That reasoning could have possibly been a bit more expanded upon buttt she was pulling a House moment of her own where some random thought pops into her head and then she immediately has a moment that answers the entire problem. Overall the episode was fun to watch but it could have been a bit better and is not one of the better episodes of House M.D
