Mini Air Crash Investigation

2 ай бұрын

This is the story of Piakistan international airlines flight 8303. On the 22nd of may 2020 almost 4 years ago at the time of recording this video, Pakistan and the world at large was emerging from the first wave of the Covid 19 pandemic. Due to the pandemic, travel and movement had been restricted on a global scale not seen in decades. But on the 22nd the worst seemed to be behind us some countries had started to open up, pakistan was one of them. Hence the flight from lahore to karachi. The A320 took off with no issues and within no time it was closing in on karachi, the pilots were in touch with karachi control and they assigned the Nawabshah 2A STAR or standard terminal arrival or star to runway 25L. Think of a STAR as an airway to get near an airport. The first officer who was flying decided to take the plane down. At 9:15 am local time the pilots wanted to get down to 10,000 feet, the controllers cleared them in and asked them to fly directly to the waypoint makli. This meant that they wouldn't have to fly the nawab shah STAR and this was shorter, heres the MAKLI and nawabshah approaches compared.
In the cockpit the pilots programmed the FMS or the flight management system for the ILS-Z approach to runway 25L. At 9:23 am the controllers came on and said “Pakistan 8303 contact Approach 125.5” but there was no reply. Target tried again no reply they tried again no reply. After three attempts the controllers tried to get a plane nearby to relay a message to the A320 but still nothing. Then out of options the controllers called flight 8303 on the guard frequency. The guard frequency for those of you that dont know is a frequency that is monitored by all pilots and controllers its there for emergencies, but 8303 still hadnt repoded after 2 calls then after the third call they came on and said ““Strength 2 sir confirm changeover to 126.5” contact was reestablished with flight 8303. Once the jet had been handed over to Karachi approach, karachi approach cleared themm all the way down to 3000 feet. By this point the jet was at MAKLI, the way point that we were talking about but there was just one small problem, they were way higher than they needed to be. You see by the time they hit makli the jet needed to be stabliszed at 3000 feet. Flight 8303 was all the way up at 9000 feet. The controllers chimed in and asked ““Pakistan 8303 confirm track mile comfortable for descent?” which basically translates to “you sure you guys can make this” the pilots just reply with affir. In the cockpit theres confusion the captain said “what has happened” take out the hold”, referring to the holding pattern on that they should have done to lose altitude. The first officer just replies with ““Hold taken out, should we report this happening?” and the captain just says “No, this could be due to Hold tell Karachi Approach that established on localizer”. The captain wanted to go ahead with the landing. In a normal landing youre supposed to come in towards the runway at a shallow 3 degree angle. Flight 8303 was no where near that they were almost at 6 degrees. Now this meant that theyd have to intercept the glideslope from above. While this isnt uncommon its much harder than intercepting it from below. To get down fast the pilots extended the speed brakes, increasing drag on the plane. Karachi tower was watching all of this unfold and called up karachi approach on the hotline to let them know that 8303 was way too high. The approach controller just said that hed let them orbit for a bit to lose altitude. But despite the speed brakes being out, the plane was still too high when they were 10nm from the runway. They were supposed to be at 3000 feet but instead they were at 7239 feet. But the pilots were committed to this landing as they dropped the gear at this point and dropped the nose in an attempt to get down in time. The plane was now 7.4 degrees nose down. Karachi approach at this point out of concern said ““Sir, orbit is available if you want”, the response was ““Negative sir, we are comfortable we can make it Insha-Allah”

I believe pilots, sea captain, train machinist, bus drivers, surgeons, operators for nuclear reactor and petrochemical operators should abandon fasting and pay fidyah instead (pay the poor with the amount of money to feed them during the day). It is more responsible and avoid any unnecessary problem. If somebody decide to do fasting, then it should be in his/her own time. Not during the1 duty that their 100% concentration is required.

Normally i like to point out that warnings should sound only as urgent as they actually are, but in this case they were ignoring so many critical warnings for no excusable reason.
@petep.2092
@petep.2092 2 ай бұрын
The airplane doesn't know when a warning is more or less urgent, so does not adjust it to sound differently. The engineers do program in a certain priority when warnings conflict or when a warning is likely to be relatively trivial for a particular phase of flight, but that usually results in suppression of a warning until later or suppression altogether, sometimes inappropriately for a particular set of circumstances as some accident or incident investigations have shown. In this case the gear aural warning was automatically suppressed in favor of the overspeed warning, engineers probably didn't anticipate an airplane coming in at overspeed so close to the runway that gear down would take priority. But doubtful that the aural gear warning-a simple repeating chime-would have affected the outcome any; human factors studies show that an aural warning is more easily ignored in favor of a visual warning that attracts and retains humans attention more easily, and they were ignoring the red gear up warning message on ECAM and the bright red arrows at the gear lever. After they silenced all warnings, the gear warning, designed to be persistent when close to the runway, resumed but was soon out-prioritized again, this time by ground proximity warnings. Normally ground prox warnings are suppressed very close to the runway to avoid nuisance warnings when the ground near the runway has steep and significant elevation variances and because by then the pilots can see the runway or they would have executed a missed approach, but in this case they were not suppressed. I'm not a ground prox system expert, but I believe the newer EGPWS uses the present trajectory and trends of flight parameters to predict whether it is going to make the runway or crash short and if so, it would be logical to enable and prioritize ground prox warnings over gear warnings, the rationale being that time and effort would be better spent trying to prevent a crash into terrain instead of trying to prevent a wheels-up landing, the former typically being less survivable.

@@igorbednarski8048They can. At least in EU. "Cancel approach clearance" can absolutely be done. Mostly this happens if there is an emergency and the runway is blocked for some reason. There is even "cancel landing clearance" depending on how far away the landing aircraft is. EDIT: forgot this - Airmanship isn't obvious to the ATC but being waaay too high way too close to the runway can be a reason for a "cancel clearance".

@@user-yi3yx2fn7g yeah, that was my point - I know that the ATC can cancel the clearance, but I thought that the clearance just means "the runway is safe to land on and you're not conflicting with any traffic on the ground or in the air" and it was up to the captain to decide if they are stabilised and configured to land. Can the ATC tell the captain that they cannot land, not because there's something wrong with the runway itself or because of some obstacles/conflicting traffic, but because they think the plane is being flown incorrectly?

I think fasting is just one of the reasons that can be mitigated since there are a lot of Muslim Pilots who don't have the same problem. The most interesting thing is that the pilot was deemed unfit as a pilot due to mental health and he almost could not fly with PIA, but then he got the second decision from his colleagues in the military.

It's clear there were a lot of other factors that played a role in this crash but Islamophobes will take every opportunity to blame Islam. According to them, Muslims can't have mental health issues because everything they do must be made out to be the fault of Islam. Unfortunately, this is what prejudice and brain-washing by Western media does to ignorant people. Current events more than prove this point

They are exempt but they still have to pay Fidya (charitable giving instead of fasting). Basically, when someone cannot fast during Ramadan for a valid reason and they cannot make it up after Ramadan, they must instead pay to a charity enough money to feed one person per day they cannot fast. If you miss a fast for no reason you must pay Kaffarah which is 60 times more than Fidya, or you must fast for 60 days consecutively.

It has nothing to do with placing your religion above people. This is about ignorance. God does not want you to take a risk when you're responsible in such a situation. Especially as a traveler, it's not even required to fast regardless of whether you're a pilot or a passenger. So, this has nothing to do with faith but with ignorance. That a person on Earth can choose to put their trust in God and then do their utmost is actually good for more motivation and strength to carry out a task. Whether it's a teacher, firefighter, or pilot, it doesn't matter. I'm actually glad that in the 21st century, there's still a stronghold in religion. So, let's keep religion and rootedness separate in this situation ;) thank you.

During the investigation after the crash, 1/3 of all pilot's licenses in Pakistan were found to be fake. As a result, all Pakistani airlines were immediately banned from flying to the EU and US. However, thanks to enormous effort of Pakistani air authorities over the last 4 years, it appears that Pakistan managed to clean up this mess. The FAA and EASA are now in talks with Pakistani authorities, and there is hope that the ban can be lifted soon.

Even amongst pilots there is a strict hierarchy and pecking order and attempts to teach them CRM and the necessity of speaking up when they notice things are going wrong appear to be a waste of time-cultural norms of respect and deference take precedence. The Asiana that crashed into a seawall at SFO when no one dared tell the Captain he was too low and needed to manually move the throttles after he inadvertently disconnected the autothrottle. Or the Japan Airlines 747 that overran the runway and ended up in knee deep water because the FO and flight engineer didn't say go-around out of respect.

The point when all good outside observations are dismissed and a " We got this " mindset is initiated is the beginning of the crash...

1) if you’re affected by fasting and you have lives in your hands, you have the option to null your fasting for the day. Allah will forgive you and you can replace said fasting day, to days outside the month of ramadhan. This is why pregnant women are given the option to not fast. She literally has a life depending on her physical/mental wellbeing. This thinking actually applies here too, even more so when you have a plane full of people. And im saying this as a muslim myself. 2) damn, if they actly commited to that landing, the plane wouldve probably overshot the runway, but im sure there would atleast be more people alive. (edited)

Please don't take this the wrong way, but your use of u, n, r, ur, etc instead of spelling the words properly makes your message almost unreadable. I am sure you made a valid point, but I can't understand most of it. You may have saved yourself a few seconds typing that way, but you added effort for everyone else trying to read it.

@@cirilloucazzu4457 come on now. This is from a religous standpoint. Just giving my informed opinion on the matter. Im sure practitioners of hinduism, judaism, christianity, shintoism, taoism, buddhism etc etc all have an opinion on the matter. They all have their respective gods, messiahs, prophets, mythologies and ofcourse, teachings. Interesting note: the semites are any people from southwest asia (essentially the arabic region) that speak the semitic language, which include arabic n hebrew. Their religion is usually (though not always the case) christianity, judaism n islam. So by that definition, im not a semite despite being a muslim, coz im genetically from southeast asia. But if you speak harshly against an arab-speaking person of arab descendant but is practices christianity… by application, you are anti-semitic😅

A great companion to the more technical video by Blancolirio (Juan Browne). I believe Airspace did one too a while back, before the final report was published.

Good video. One key point I wish you had covered was the plane was very much glidable to the airport when the last of the engines failed. This would have been their act of redemption. But they pulled the nose up (not the one at the very end but way before) and lost altitude and speed which was the final coffin.
