You are so observant! Way to go. You're right...technically. I have some wood backing up there & I drilled through the side of it in order to slip the wire into the box. There wasn't enough room to get a romex connector in there & for the box to lay flat. I also have the cable stapled a few inches from where the cable goes into the pancake box. Will that suffice for you? would you approve?...lol Joe :0

What size hole saw is needed for the electrical cut in box?

I would not recommend installing a ceiling fan into a cutin box, since you will only have the sheetrock holding up the weight of the entire ceiling fan. Check your ceiling fan & see which size hole box you need as there is 3" & 4" hole boxes. Things can differ so check it out as size varies. Thanks for your question. Joe :0

The wiring is not code compliance ( you did you use bushing to protect the wire entering the JB
