Mental Health VA Disability Claims And Disability Benefit Questionnaires

Mental Health disabilities like depression, anxiety, and PTSD can be tough to deal with and even harder to submit a claim for. It's important that seeking a DBQ, Disability Benefits Questionnaire from outside the VA, that the Veteran understands what is needed and communicates that to their provider, especially if the provider has never seen a DBQ before.

Can OCD and depression be added on the PTSD DBQ or do they need to be separate on a mental health DBQ?

So if the total occ and soc inpairment box isn’t checked but in symptoms it has multiple 70% and 100% symptoms checked does the Box 3A or 4 matters in this case? Reason asking is because my C&P Examiner checked “Occupational and social impairment with reduced reliability and productivity” but I have a lot of severe symptoms that’s check off in symptoms for 70% and 100%

In section 1a. Under "note", it states "Also, if the veteran has a diagnosis of PTSD, the VA form 21-0960p-4 , ititial ptsd disability benefits questionaire must be completed by VHA staff or a contract examiner in lieu of this questionaire." My question is, I originally went to a c&p for Fibromyalgia. After completing the exam, the va requested a Mental Disorders other than ptsd or eating disorders exam. I am already rated at 70 % by the va for ptsd. Depression and anxiety were mentioned during the Fibromyalgia c&p exam. So will the examiner fill out both dbq forms. Or only perform the initial ptsd exam as stated in section 1a.? Ty

The examiner will complete what the request is from the VA. When it comes to Fibro, once of the many 'not known' causes is trauma. So I'm guessing the examiner was checking all boxes to as required when it comes to Fibro.

So if I was seen and they checked the "total social and occupational impairment" box on my DBQ. I also was taken to the ER and hospitalized for suicidal ideations about 2 weeks prior. I tested really high on the PTSD questionnaire as well. I'm at 70% for PTSD currently. My doc said I also have a probable TBI with no evidence, anxiety, depression, inability to establish and maintain relationships, and memory loss issues. Do you think they would bump me to 100% for that?

how likely is it to receive 100% if the VA used their own examiner from the VA to do my C&P exam and the Examiner checked the box for Total occupational and social impairment and then also wrote is more then likely then not and atleast as likely as not connecting my 3 mental health conditions to my service connected disabilities

"More likely than not" means 50% or greater connected to service (basically service-connected). "Total occupational and social impairment" indicates your mental health could be rated at 100% on it's own.

My damn examiner got my DBQ paperwork mixed up with another person! There were people/ words/ doctors ive never came into contact in my life with! My veteran service officer had me write a letter to the senator! And we are in the process with the VA. Anyone else went thru this?

I will have C&P exam soon for claiming as directed-service connection of insomnia but I didn't submit the DBQ as you mentioned. My insomnia is due to back-pain, rhinitis, and sleep disorder. Will C&P examiner write DBQ to the VA for me?

Most likely, but not always. The more important piece I'd like to know is what is your evidence suggesting that insomnia is secondary to those conditions?

@@AccreditedVSO My back pain and allergic rhinitis are serviced-connected. I have insomnia indicating in my STR which doubt of sinusitis, Meninggitis or CNS Bleeding. That is why I claim as direct-service condition instead of secondary. Also I have upper respiratory infection treatment in my STR as well. They gave me medication for insomnia and upper respiratory infection symptoms.

@@genjustice4766 Based on this...You have the in-service event (complaints/treatment plan) and I'm assuming that treatment has been continued since you left service which would lead to a 'current' diagnosis. File the claim.

@@AccreditedVSO Thanks, yes I have VA treatments but they are not for insomnia. For insomnia treatment was only during service. The VA treatments are all about back pain, sleep apnea, and rhinitis which I used it as reasons for insomnia. I just did my C&P EXAM. The examiner said my insomnia was due to physical illness not the metal health. I spent only 20 minutes and being asked about my life before, during, and after service. I said my back pain and rhinitis caused my insomnia. Due to that symptoms I could not sleep and kept moving a lot at night and bothered my wife. I had to separate room with her to sleep which leaded to our bad relationship. However, since I have been working the same place and the same job for 10 years, I can feel that the examiner seems to be surprised that I could keep my job and I shouldn't have any problem with occupational impairment. Not really sure what rating I would get but I just need only 10% from insomnia to make my combined goal rating at 70% then I will stop filing claim.

You have the same symptoms I have. How did your CNP go?
