You said you didn't recognize the German city of Nürnberg. That's just the German name for Nuremberg, so you are probably familiar with it. The grouping of some of these cities doesn't make a lot of sense. Defining a city is already a difficult task but here it's done really inconsistent. The top German city (Ruhr) is not a single city but a region of closely integrated cities in the west German industrial heartland (think Dortmund, Düsseldorf, Essen). But if you group these cities, a lot of different cities should be grouped as well. The Peal River Delta for example would land at the top of this list (HongKong, Shenzhen, Guangzhou,...). However, if you group Tokyo with the other cities in its Metro area (Yokohama, Saitama, Chiba), they would probably still top this list.

Regarding the French cities, I assume Lyon and Marseilles were close to be included, but yeah in general the French population is spread out in a lot of middle-sized cities, Paris is really just the big exception. There are probably a lot of reason but one of them for example compared with the UK (which has Birmingham and Manchester), is that France has a relatively big territory for a population similar to the UK (way smaller).

The visa free access mainly comes down to two things: 1) The political situation between the two countries. 2) The economy/standard of living of the nation one is traveling from. The first is pretty self explanatory while the second requires a bit more context, so if the home nation is well off in terms of wealth the likelihood of the individual going back home once their visitation period is done is vastly higher, as opposed to somebody coming from a poor economy/standard of living nation, where they will likely not want to return to given the poor conditions. You can see this most vividly reflected in the US map regarding that topic. Also China is currently hitting a economic downturn so the wealth of those cities have likely altered since the video was posted, in a similar vein while Detroit basically hit rock bottom for quite a number of years especially when a majority of the automotive industry left, they have since started to make a bit of a comeback around 6ish years ago however, its still likely going to take quite a few years for the city to become what it once was again.

The heleny index doesnt really show the full picture as they also count us and uk territories such as guam, st helena island, montserat etc, so a passport with visa free acces to the uk and usa has somewhere between 10 and 15 extra points. Passport index has a more accurate representation.

My guess for the top visa free passports was quite on point. The Nordics, Canada, parts of central/western Europe, Australia/New Zealand. Basically all the countries that take top spots no matter the metric. press freedom index? democracy index? censorship? moral freedom? economic freedom? corruption index? metal bands per capita? Yeah those countries all rank at the top spots for each of those.
