One of the most common knee injuries in young athletes is an anterior cruciate ligament (ACL) sprain or tear. In this second part of the series, UCLA physical therapist Carole Netter, PT, MS, covers rehab principles, braces, crutches and the home exercise program (HEP) for the initial six-to-12 week post-operation period.

How are feeling after two months? I have been following these instructions for 3 months. Now I have no pain at normal walking or stretching, but it "clicks" and pains when I jump. :/

Excellent education for ACL rehab for the patient, really enjoyed the discussions with Jennifer Beck and Doug Polster (Orthopaedic Surgeons) input on expectations for recovery, unknown timeframes, reframing pain. The talk about commitment to new exercise regime like a skill in sport that you have to master this was great gems of advice. Discussing about how it impacts the family was also really useful and the video examples also were great for visual learners.

Very good useful information. I might go visit you guys to help on my recovery. Keep at it. -15 week after surgery, still some pain @ patellar tendon vicinity when stretching and weight bear. Need to get a detailed assessment like the surgeon suggests.

Stages of Post op rehab Dressing 2 to 3 days. Change to less bulky dressing and ace wrap so that bracing and ROM easier. Stage 1 (3 -4 weeks) Swelling ( taping, modalities, manual therapy) ROM ( knee flexion and ext) Strength ( knee ext- quads isom and hams Hip ) 10 to 20 min bouts about 3 times Crutches Brace- start with locked in extension. Important to wear it during weight bearing as muscles are weak etc. Stage 2 (wk 4 -16) Hip and knee weight bearing and control - Squat Lunges Diaonal and Side to side , progress to resistance. Stationary bike, treadmill inclined, walk outside. Plan ahead of time for time commitment - others can't rehab for you. 45 minutes. Motivation. Muscle memory training. While watching TV or during PE. Stage 3 - Around week 16 onwards. Start moving and prepare to return to sport. Stable enough for movement retraining. Mach of injury in ACL - I landed I planted and I heard a pop. Medial collapse. In the last stage - we use glutei and hip and knee movement. Jump, take off and land.Deceleration. Reversing. Slow and fast. 90 degree cuts. Complexity and speeds increased. By month 6 - tests for return to sport- cut Video By month 9 to 12 -- formal tests for return to sport. Muscle strength testing. Static balance testing. Hop testing. Single leg squat - return to running. Does knee feel normal. Front of knee pain - quads weakness Popping or clicking - meniscal tear and PF it.A lot of it goes away with therapy and better quads control and squat correctly - hip and knee control to avoid load on front of knee Pain - not injury alone but wrong type of movement can cause it. Restructuring our thought process around pain. How long PT ? What you do at home close to an hour most days a week. Some unknown factors. Home exercise programme- handouts, video with phone.

UCLA HEALTH, thank you for the video. I am 2 years post op. Ligament reconstruction of the ankle. There is very little info/videos discussing what to expect (pre op) and how to rebuild (post op). Please, make a video for the people having to work through the ligament reconstruction surgery. This video is very good.

Tore not only my ACL, but my LCL and MCL in a motorcycle accident. Had op on 10/12 of 19 and still go to pt twice a week. Still cant run yet, but i'm only a few weeks away. Only 20 but say I'll never fully recover but always thankful that I can at least walk again.

@@haroonanwari5021 super thankful that my life has returned to back normal. I don’t have any knee pain from day to day unless I step awkwardly with pressure on my knee or something but that rarely happens anymore. I am slower than the next guy at getting up and down but I feel like that’s expected and won’t ever go away. Scars are still a little knarly lol. When did you have your accident?

Thank you for the proper information. I will be going for cupping therapy for my back pain - PIVD as suggested by my doctor based on my MRI Scan, as I have -Lumbar spondylosis -L3-L4: Diffuse disc bulge with annular tear causing bilateral lateral recess narrowing, with indentation of bilateral traversing nerve roots, however, no secondary spinal canal stenosis -L4-L5: Diffuse disc bulge with posterocentral disc extrusion causing secondary spinal canal stenosis and bilateral neural foraminal and lateral recess narrowing with indentation of bilateral traversing nerve roots I would like to know the correct remedy for the same. Also, may I know, How will cupping help me recover from this unbearable situation? Please help 🙏

Hello Dr Netter, thanks for your video,my aunt send me your video and link from UK,I'm in Ghana and I had an ACL reconstruction surgery 6months ago but still can't bend my knee completely,I can't squat , but watching your video,it seems I'm behind, please can I make It? Our physiotherapy had been closed because of covid 19,I'm only exercising at home. I'm a bit scared.

The first daya i was walkin normally after surgery i didnt really use my crutches then the second week after the drugs wore off it was hurtin all that week and after that i was walkin normal again i waited a month n returned to basketball not runnin really just jumpshots, ball handling, and freethrows i havent been able to go to rehab due to the corona and lockdown so im not sure wha i really should be doing i have been goin to the weight room n doin weighted leg extensions but ik my leg isnt as strong because i can only put 5 pound weight on it i just want know if im doin fine

@@matthewstewart519 it was actually pretty rough first few days after the surgery but after like a week from the surgery i got used to it i did my first step without crutches but with braces 2 weeks after and now am walking almost normally without any thing hahaha

I almost at my 6 month mark from my 2nd acl reconstruction....my sports surgeon has told me still to avoid cutting. Any theories why? I didnt think to ask until i watched this

I got infection after 2 months post acl reconstruction surgery my doctor removed screws but graft retained in acl debridement cleaning Surgery is thier any chance to recover

Hi, Sorry if this is too long but I really need some help. I had ACL repair surgery using patellar graft 9 years ago. I've done pretty good for the most part but recently I started squatting and lunging, and it has been getting really sore and cracking, and I've lost mobility and strength in it since. What advice do you have? Thank you so much.

Sounds like you should find your physio again. It's really common for patients with ACLR to develop into PF joint problem. I believe you could only get the appropriate advice after your Physio do the through out assessment

So I had ACL surgery about 9 months ago and my knee seems to be pretty swollen still. I can walk fine but it feels like there's a lot of pressure built up in there and every now and then my leg won't completely straighten out. It almost feels like it needs to be popped but I don't know if I want to take the chance of popping my knee that I had surgery on and something else happening. Is that what I'm feeling? Does my knee need to be popped or something? I used to pop my knee all the time and ever since the surgery I haven't been able to. It's only popped when it wants to

Hey! What you described here happened to me as well. For me it ended up being that I still had a bucket handle tear in my meniscus and that was holding my knee from being able to straighten out as well as making it feel as if I needed to pop it. Obviously everyone is a little different and to know what is truly going on in your knee you will need an MRI. I wouldn't recommend popping it yourself, several times before I could afford to go in to surgery I was able to see my PT or a sports trainer and they were able to "pop" it back into place and that gave me some temporary relief...

@@kelseyharms4582 I remember talking to the surgeon and they checked out everything else because he initially expected it to need fixed but be said it was fine, I wonder if there was something he missed. It does pop occasionally by its self and when it does it feels great lol. I have a checkup next month so ill ask about it then and hopefully we can figure it out.

What kind of leg exercises can one do in the years that follow an ACL surgery? It seems most leg exercises make my knee hurt for a day.

Strengthen your hamstrings and don't do too much at once. You are better off doing a set of 10 simple exercises consistently and not feeling dead or weak after the exercise than doing your max set and really exhausting your leg and then being stuck in pain the next week. Start slow but be consistent

Numbness is normal better don't get panicked , it takes time for the graft to get adjusted . I am on my 9th week of ACL reconstruction, still I have numbness down the leg

Question. I am 14 month after ACL repair via hamstring graft. Knee feels great, I can squat heavier weights than before surgery, walk, bike slow run no issues, but I can't sprint for life as every time I even try to push myself to 50-60% speed my hamstring just gives up and I keep re-injuring it. Could it be that when they where taking the graft they damaged hamstring beyond repair? I am so frustrated spending countless hours working on my hamstring and not being able to return to soccer or basketball. I wish I was offered patella graft instead, I bet I would playing by now. Now my surgeon says it may take longer time for hamstring to recover but is it really just a matter of time?

swim bro. focus maybe if you can without irritating it by doing hamstring isolation workouts low weight high rep...perfect reps mind you...see where that gets you. And yes its a time game. but I can promise you if you swim and keep up on actual core workouts...(not just adominal workouts) I bet youll be fine..but if all that hurts just swim man.

@@blackguy5019 No it's not a time game. The tendons are not there as they were before so you will never be the same. I'm 15 year's after surgery . It ruined my fotboll carrier att the time, I also wished I would get the chance to chose graft. I'm still depressed over this sometimes..

Please answer my question. I only did the basic excersises that they gave me for the 1st 2 weeks of therapy. I continued to do that on and off for months but I never went to physical therapy again since I moved states. Am I okay? I have pretty much full range of motion, but I can feel it still has weakness and can get inflamed.

Jay Peter pipe-her if you still Have weakness in your knee you have to start your physiotherapy exercises. If you can’t afford a physio KZfaq has lots of videos on exercises required for ACL rehabilitation. This video in particular has already shown your the phases of rehabilitation. Now all you have to do is look for the right exercises.

Hello. Im from Europe (Croatia) Why in America is everybody speak about 9-12 month to get back on filed. In our contrys its normal to play after 6 month. Even I played after 6 month after first injury ACL. But i worked 4-5 h at day at gym. Why is so diffrend recovery, (longer) in USA? Sorry about my bad English.

Sharad Singh this will depend on how strong your knee is. First and foremost you will have to start your weight free exercises. This will ensure your knee is well balanced to carry your body. When you can do these exercises pain free you can go on climbing stairs.

GREAT video ,i did the operation 5 months ago and I wish I had watched the vedio earlier, just one IMPORTANT question WILL I return to my natural strength ,after the therapy? meaning it wont increase the possibility of getting ACL injury again ?

nezar yassin it will depend on your physiotherapy phase. You have to make sure that you do the physio correctly and ensure that your muscles surrounding your knee are strong enough to protect your knee from injuries. Even after returning to sport you still need to integrate gym sessions focused on strengthening your knee and other muscles to avoid muscle imbalances. I hope you find this information helpful.

walking normally can take some time. focus on quadriceps and hamstring

Im no expert, but you might not have full extension of a leg. I went to clinic few weeks after the surgery, got there and they told me I can bend my leg just like healthy leg and wasn't even working on getting full extension but just worked my quad. Few months later, went to different teraphist and she told me i dont have full extensions. We did some really painful stretches, but it worked. Might not be that tho..

Baemax 14 Yes you will have to return back to square one because your muscles will have lost some of their athletic movement memory. Most Especially if you had surgery done on your knee then your muscles should be weak after weeks or months of not being activated. If you can’t afford a physiotherapist KZfaq has lots of videos on how to rehabilitate after an ACl injury. This particular video has always given you an insight on the recovery time. All you have to do Now is look for the right exercises.

take it easy , consult your surgeon , make some X rays if necessary , ice it for 15 mins a day if pain is annoying , hope u get better , just don't rush it , and follow your physiotherapist protocol!

Johnny humes well I’m not 100% sure but I believe recovery time after surgery is 6-9 months. So you shouldn’t be worried but keep up with physiotherapy if u can

Hi Carol. I would like to ask about my case specifically about the popping of the knee after my acl surgery and meniscopexy. After 8months I asked the doctor for a MRI because I felt like something was wrong,so the results where there:The medial meniscus yust again torned,but the acl(hamstring graft) was beautiful on his place. The question is:What could be the reason of the huge sounds average of popping(like two bones are moving,it feels like..) after straighten the knee even when I am sitting and then standing after the squads)and yeah it's not only a terrible sound but sometimes is very painful. Could it be caused because the torning of the meniscus(I honestly don't think..). I don't trust my doctor anymore therefore searching the answer. Thanks a lot.

The Life Survivor Astria I had surgery in March 2017 and today my knee has hurt more than it has in the past 2 years. I’m worried that I’m gonna need another surgery. If I have to have another surgery, I’m gonna cry.

This is bad. I had surgery a couple months ago but I’ve been going to physical therapy and strengthening it like crazy. I think I’m going to be able to play soccer this year but my advice is start stretching, working out (DONT over work) and keep icing!

I started 10 days after surgery, but you can start even earlier. I got some exercises from my surgeon before that.

buba minteh you have to start with the basic exercises mentioned at the beginning on the video. Re watch the video but this time pay attention to the exercises mentioned during the first stages before the 16th week of physiotherapy.

Araju Barot if you do any common sports like soccer,basketball,track & field etc you surely will need an ACL reconstruction.

I am a 56 year old female. Had high-grade partial tear of the ACL no meniscus involvement while playing tennis. I keep going back and forth on rather to have surgery or not! I have a partial tear of my right ACL also but choose to rehab and have worn a Donjoy brace and continued to play tennis without issue for the past 3 years. There is no way I can possibly continue to play tennis with 2 braces! Therefore again the struggle is real....do I get my ACL repaired, go through all the rehab to play tennis again? I am able to walk, jog, ride a bike etc obviously no single stance or cutting activities. I have always been an athlete playing multiple sports in high school but at my age of 56 I am questioning surgery. I want to continue my active lifestyle. Would love to hear from anyone especially if older how your rehab has been going and if you had to do it all over again would you choose to have it repaired or go strictly rehab route with no surgery? Thank you!

Well, I probably can’t advise you the best - I am currently approaching week 3 post operation - but I am 28 years old and I found the surgery quite intense. At your age, I would be very hesitant to have the surgery done. I would suggest just doing some intense physio. Getting surgery is like going back to square one, like learning how to walk all over again...

I lived for 6 years with a full tear. It was ok. Keep the muscles strong and wear a compression sleeve. I’m 45 and hike and jog with it torn.

It depends on what ligament was injured. The medial collateral ligament(MCL) and tibial collateral ligament (TCL) usually do not require surgery after injury and they can heal by themselves between 3-6 months of rest and rehabilitation. However ACL and PCL tears will need surgery. The best thing to do is see an orthopedic for a physical exam.

hi..my ACL reconstruction and meniscus repair surgery completed on 23.01.2019 and 5th week is running..I got 120 degree flextion but still my knee have swelling...when this swelling will be removed ...is it any problem...plz suggest ..

Massage it with cream three times a day for two weeks continues after two weeks start icing it ones a day after two weeks you can ice it twice a day remember that massaging it will reduce the swelling because I expiring it.

sumit mishra you have to go back to the hospital and see the doctor. You might have some synovial fluid in your knee. In the mean time keep your knee elevated and compressed. Apply ice 3-4 times daily for 15 minutes.

Bharti Malled before you give the final verdict of the pain coming from the ligament you will need to see an orthopedic. It might be patellar,meniscus or damaged muscle tendons.

Jonatan Tomšič you need to do physiotherapy to strengthen your knee. Start off with off weight training and move on to adding on weights as the pain decreases.

So I have been suggested to do light exercise for two weeks and then increase intensity exponentially. I am still unsure if I would be able to achieve full flexion and extension in a month, that is my target. I am at three days now. Are you able to run at 3 months?

@@vhionranyyak yes. I did my acl two months ago and my pt let me run on threadmill at speed 8.5 for 15 mins. I can also jump but only on trampoline.

raj kumar after your physio sees that your knee is finally getting stable and strong. He/she will have to integrate sports movements into your physiotherapy sessions. As mentioned in the video you will be trained to cut,land and jump. After that phase you will be moved to weight training to give your muscles athletic strength and explosiveness. You will have to train your body to get back into the state it was before the injury.

i did not have braces after the surgery and my doctor did not let me stand on my leg for 3 weeks. i just started putting weight on it (it's almost 4 weeks now since the surgery). so i think it is totaly normal :)

@@tayyabzafar741 you are lucky if you had a repair as your shock absorber cartilage is intact but go slow and start quadriceps and hamstring strengthening exercises. Once your leg muscles are developed you can start your sports.Btw which sports do you play?

Dipankar Biswas it’s a pain in the butt! I’m about 6 months out of my surgery and working out/ physical therapy is a pain in the butt especially during Ramadan. Good luck! I hope for a speedy recovery and remember to workout!!!! You DO NOT want to be lazy, I promise you, you want to get 100% as quick as possible

dhaval soni yes your ACL can be reconstructed again. You probably had a second tear because you might have not done the correct rehabilitation procedures. Ensure that you follow the correct steps mentioned in this video. Please do not return to sport without having it reconstructed.

hey I think I got your question right, here is my story that will help you decide I had ACL injury and my doctor told me that I should try and do the "protective " therapy , no operation just exercises, after 3 months of doing what he said he wanted to test me so he told me to try and play football, I didnt last 5 min untill I fell to the ground and got injured again, so I went to a better doctor who told me that this kind of injuries its always better to get the operation done is soon as possible ,and it is a waste of time not to , so my advise is to : 1.choose a good doctor who knows what he is doing exactly with good experience 2. do the operation 3.exercise alot and more important to do the exercises right 4. every 4 weeks visit your surgent wish you a speedy recovery

Hey Rao, I had a partial acl tear years ago. For early stages use RICE rest, ice, compress and elevate. Then you have to do strengthening exercises provided by your physical therapist. Highly recommend going to physical therapy if your knee bothers you day to day. I recently had an ACL reconstruction using hamstring graft and i'm in the early stages of recovery. I didn't take care of my knee when I had a partial tear and now I'm doing everything from scratch. Do not make the same mistake as me :D Goodluck to everyone going through any setbacks!

If your not an athlete I wouldnt bother with the surgery. You will be out for over a year, and you really need to work hard to get it back to normal

I had a partial tear of my ACL about 4 years ago, I was told by the surgeon that if i rehabilitate my knee I should be able to get back to normal. But as time passes you will find your knee has a tendency to collapse especially when you turn sharply or pivert. I'm now 3 weeks post ACL reconstruction knee feels stable and walking with a slight limp but few more weeks and that will go. So personally I would recommend having the surgery even if it's a slight tear as by the time i had my surgery mine ended up being totally torn

I am on my 9th week still I am not trying to run , my physio said only to focus on normal stretching and squats and lunges, mrng and evng for 30min for sure , after 15 weeks he said to try to do normal jogging and cycling . For now I am driving car and bike.
