Hello Paul Marquis, I really like the way you are explaining the logic behind your evaluations. I have injured my foot few days ago, but the MRI appointment is still few weeks ahead. So I need to make the best decisions for my current recovery and healing on my own. For the last 4 months I ran about 20 miles each month. And before that, I only did some swimming for 3 years without any high-impact sports. On the day of injury I did 1 hour of fast paced walking instead of running for the first time. My shoes were pretty worn and their sole is tilted forward. No complications after the run. In the evening I did some slightly intense foam rolling under my feet and suddenly felt a severe local pain under the ball of the left foot, somewhere between the first and second midfoot. No sensation/hearing of any snap or tear. The plantar fascia branch for the first midfoot bone is still attached, but I do not know if it is damaged on its inner part. The pain is only reproced by palpation of that local area. The next morning I got a bruise around that area on the bottom of my left foot. Standing on that foot is unproblematic. Rolling over the foot when walking becomes uncomfortable. Any flexion of my toes provoked while sleeping face down, becomes uncomfortable. When looking at the described severity of my injury, what evaluation/assumption seems the most likely one? And a more general question: What foot injury besides plantar fascia rupture can cause a local bruise directly after the incident? Best regards! :)

Dr.…you’re exactly right….some doctors diagnosed pain associated in that area with plantar fasciitis in my case MRI confirmed a slight partial ruptured of my plantar fascia , I have been receiving PRP treatment just finished my first treatment, finished steroids shots, they do not work for me….barely walking right now and when I have to walk am using an air boot, have new insoles…I’m 56 yrs old still playing sports, changed my old cleats for the same style bought new ones and everything happened…I stretch every day and it happened turning from first base to second at fast pace…just felt a burning sensation in my heel …after the game was over could not walk well, heel area was extremely irritated, it’s been almost 8 weeks and still feeling some slight pain but is not constantly like in the past , after walking with my boot. Just want to get back to normal. Thanks for the video, it was right on with me.

I was on stage playing a show when I felt mine pop! It was painful and it was only the 2nd song in! I finished the set and when i looked at my foot it had a bruise and i could feel it swell up. The next day i had a purplish bruise in the middle of my foot. Its been 3 weeks and its gotten alot better! Still has a little soreness but it’s slowly going away. Ive been using a tennis ball to roll it on and rubbing my foot with my thumbs to dig in there .

I just experienced this over a week ago. Being a nurse, I pretty much searched things out and started my own treatment at home, as well as, purchased a protective boot. I am really interested in all of the exercises that you had him do.

Still trying to figure out what I have going on. Thought it was gout of the mid foot but apparently not. Got a little better after about a week but now getting back to original pain levels ( can’t put much pressure on foot) Cuboid, Lisfranc, sprain, strain etc etc. The odd thing is I haven’t had any swelling or bruising but pain levels of 8-9 at peak.

question - How is a torn plantar aponeurosis related to a secondary sprain of the lateral collateral ligament? (i don't have knee pain, but the soleus is tight)

Old video...but asking a recent question...I just felt a tear when jogging to escape rain...wearing a new Dansko work shoe. Felt immediate left foot pain that made me stop but not too painful. (Can still walk but the tendon on my right foot feels tigther as my left foot feels like its stretched or loosen)... definitely a rip feeling when it happen. I have a slight dull ache but not severe pain more like a 3 or 4 out of 10. Is it a micro tear that still needs a boot?

Hey thanks so much for this video! So I definitely did this to my foot around 8-9 months ago, unfortunately it happened in a lockdown so I didn’t go to the doctors to get it seen to and still haven’t. But going from everything you’ve said I’m certain it’s this that has happened. I was training for a triathlon and haven’t been able to run now for 8-9 months, sometimes it feels like it’s better and then it goes straight back to square one! Sounds like I should have had a tall boot to sort the issue out? Is it to late? Thanks so much, you’re a gem 💎🙏🏼

Hi I have a question I was recently diagnosed with torn ligaments in my left ankle on top of my Complex Regional Pain Syndrome diagnosis but I have an indention in the back of my left heel and severe pain in my arch as well as burning all over my foot and ankle since this new injury could I have damaged my arch ligament as well

Dear Paul, thank you so much for the video, it helped me a lot! I ruptured the medial part of my plantar fascia in May 2022 (heard a loud snap and severe bruising), unfortunately, the doctor at the ER did not take it seriously. So I walked around injured for four weeks on pain meds before it got properly diagnosed and treated. I then wore a walker boot and used crutches with no weight bearing for 9 weeks but even after really resting it, the fascia is still very swollen, thick, stiff and painful now even when I only slightly press on it. I nevertheless started with PT since my doctor said that we need to prevent the muscles in my feet from further degenerating and I need to get moving again. Is it normal that it is still not healed at all after resting for so long? Do you think I should start walking again, even if the ligament is still visibly swollen and in pain (walking itself is not painful since the injury is in the arch)? Thank you!

Thank you for this useful video! I just searched this video and also experience a pop in my sole area (1/4 from the heel). just happened last nite. No bruises . Not too tender when I press on it. But can stand on it yet. are these symptoms of a palantar fasitis rupture?

Tough to make a guess without further evaluation. If it is a rupture then it would be very painful to walk on it for about a month, usually some bruising and very tender.

I suffer from this, plantar fasciitis I had to go to the ER after almost 2 weeks of server pain my blood pressure was extremely high and have torn ligaments would it be because of having this issue, I get no where with doctors in lost and in pain

Hi I'm male age 30. I did flatfoot surgery 7weeks ago. Was using my walking boot with crutches and slip. Weight went on my foot and I heard a pop or felt one where my arch is at. It's been eight hours, no swelling. But at my calves where I they shorten the tendon feels sensitive and stiff. Not sure if I torn the plantar fascia. Is the Achilles tendon control the plantar fascia?

I think i have been having minor plantar fascia ruptures sicne years, i feel something tearing at the bottom of my foot from time to time. But, i do not feel a big pain..i definetly felt something tearing, but there is no pain. I also have extremely flat feets, is it possible that the plantar fascia ruptured at the area where the arch of my foot usualy is? How can i build up my strength? I am trying to move less and give less pressure to the plantar fascia, but it is kind of hard since walking belongs to my daily activities.

Very late to the thread and struggling to self-diagnose. I have had plantar fasciitis several times in the past and have been able to self treat back to health. A week and a half ago in a martial arts class (with a lot of ‘stomping’). I felt a sudden pop in my heel and immediate intense pain in the heel in the place I get the fasciitis. I was unable to put any weight on it for several days but no bruising or swelling. The pain is lessening each day but I am limiting putting weight on it by using crutches around the house (WFH). I wore the boot as described in this video but it made it feel a little worse. It actually feels better with shoe orthotics making me wonder if it is actually just plantar fasciitis and not a tear or rupture. The suddenness of onset and pop in the heel don’t teals seem to fit that diagnosis though…

Hi Richard, Sounds painful. There are several reasons to hear a pop in the foot. You should probably see a medical provider for this just to make sure you don't have a small fracture or other tendon tear.

I had a mild case of plantar fasciitis I worked in 8 hours shift and I felt a squish In my foot and from that point on I then in severe pain

Thank you so much for this video. I found out on Tuesday that my MRI confirmed that I had a plantar fascia tear. My doctor has also put me in a boot for 4 weeks. The first 2 weeks he has me non weight bearing. Then the following 2 weeks, minimal/according to pain bearing. Just wondering your thoughts on this?

thank you for the video. I have a complete rupture and it happened a week ago, I am wearing custom made orthoses that I dug out of my closet along with a high walking boot. I cannot put any weight on the foot at all without extreme pain. My question is; should I do some gentle stretches at this stage of my recovery?

Hi Lyne, Yes, the key is gentle stretching but you should use the boot for 3-4 weeks and give it a chance to settle down.

Thanks for posting this video, Paul. I'm 9.5 weeks out from tearing my plantar fascia and haven't been able to find a lot of resources about what to expect in the healing process. I've got quite a lot of swelling in the medial part of my arch - looks fairly similar to what's here. In your experience, does that swelling eventually go away or is that the new normal? Thanks in advance!

Hi Tina, sorry to hear about your injury. The swelling will go away over time but...if the plantar fascia is torn, it may "ball up" a bit to make it look swollen. You may need an orthotic in the future once everything settles. Good luck!

Yes the swelling can last quite a long time. If you can find some sort of a compression sock That might help

I have a partial rupture of the medial plantar tendon (11 weeks ago). Wore the boot for 6 weeks and had 5 weeks of therapy. Now I am walking pain free for 2 miles and biking. I have been using an orthotic regularly since removing the boot. How long should I use the orthotic and should I keep rolling the plantar with a golf ball and frozen water bottle? Plan on returning to softball by March.

Hi Bill, Most of my patients continue wearing an orthotic long term because they feel better in it than out. You can get out of it but you should wean out of it a little at a time. Golf ball is fine. Ice has been well proven to not get rid of inflammation or swelling so I would only ice for pain. Best of luck!

I tore my plantar fascia near my third toe a year ago. My arch has caused me so much pain and spasms even at rest since then.. I use sole inserts but still not supportive enough.

I made the mistake if jumping off a high cement block and landed hard in my fascia. Then I didn't go to the doctor for 10 months! It didn't get better by itself. My new doc Saud he would have put a cast on me a year ago but for now he gave me inserts and orthopedic brands of gym shoes. He said I appear to have a rupture. What do u recommend?

Hi I went to a foot doctor for a discomfort on my heel. She put me in medrol medication and everything went great. Now pain back and she once again put me in medrol and she is suggesting that I might have a rupture. How I can be sure that I have a rupture. I did not feel a pop, just pain and discomfort.

I ruptured my Plantar aponeurosis in mid Feb... been on/off running since and no running at all since begin may. I have now been in. boot for 1 month but still feel a fairly continuous dull ache in the torn area. I can palpate and massage with not much discomfort too. Any advice for therapy and when to begin running again?

Hi, I' m a forefoot striker and I tore my quadrant plantar muscle while training, when I recovered from that (2 months) my physiotherapist said I had a tear in the fascia. He injected an anti-inflammatory into my heal, bandaged me up and I rested for two weeks. Three weeks back into training and it's starting to hurt again. Are there any exercises I can do to help prevent this? I've tried the towel pulling with toes, lifting pencils, calf lifts etc..... It's driving nuts . TIA

I had this same injury happen 8 months ago. I had gone to urgent care after it happened (I was out of town when it happened) and was told to just ice it and stay off it for a little while. Well 8 months later there is still a swollen tender spot in my arch, my foot is still swollen and bigger than my other foot, I just realized that my injured foot is now longer and I don't have much of an arch anymore! 😨😱 Did I ruin my foot by not seeing another doctor? Would a boot at this point help?

Hi and thank you for the great video! I have bilateral plantar fasciitis and spurs in the plantar fascia. I've barely been able to walk since March 27 due to pain. Three sessions of steroid injections, massage, stretching, new shoes and new orthotics have helped minimally, with the right foot especially resilient to treatment. I cannot take NSAIDs due to a hiatal hernia. So I was sent for an ultrasound last week of the right foot, which showed a partial tear. My podiatrist just modified my orthotics by adding some height and decreasing the pressure where the tear is. I'm concerned that in your video and everywhere else that I look on the net says the treatment is immobilisation in a boot. This video is for a complete rupture; is immobilisation unnecessary for a partial rupture?

I have this exact same injury. Been to ortho. How long do you think I should wait before attempting to run? I love running and depressed because I can't run. I am almost 6 weeks out

Plantar fasciitis is common on people that doesn't take enough water in a daily basis.

I’m curious if a partial rupture dramatically increases your chances for a repeat partial or full rupture?

I have had a plantar fascia tear for over 12 months. I thought it was just a bad case of plantar fasciitis and went a long time without diagnosis. Would you recommend going into a walking boot at this stage? I’m having a hard time getting the tear healed now. I have limited all physical activity for 2-3 months now with no improvement.

I like using a walker boot for 2-3 weeks to let it settle down but you should be in a structured program in PT after the regain motion and strength. Good luck Adam!

I tore my plantar fascia about 6 months ago but continue to have pain on the back of my heel and continued soreness. Almost feels like the back heel bone is bruised all the time. Been through PT and continue to do exercises. Will this ever go away or is there any thing to so to fix this?

Hi Dr. I injured my right foot last Tuesday and went to the ER to get X-rays. They gave me a special shoe (not boot) and said from my explanation of pain and looking at my foot, they said it was most likely a rupture plantar fascia. I have been using crutches, but wondered if I needed a tall boot as you mentioned for your patient in the video. Also, do I absolutely need to see an ortho? I have no insurance so I'm trying to figure all of this out . Thanks so much

Last week, while playing basketball, I heard a pop and felt tension in my foot. At first, I thought it was my sneakers that had ripped, but I soon realized that it was something in the bottom of my foot. Since then, I have been icing my foot twice a day and using an electric therapy device at home. The swelling in the ball of my foot has gone down, but I still feel some soreness at the heel. I think it would be good for me to have a boot for my foot. As a remote worker, I do not walk much. However, I have been incorporating calf raises and foot massages into my routine, and after just eight days, I have noticed a reduction in pain. Same here exactly. I thought I pulled a Zion Williams when he tore his Nikes up! When will I be able to play basketball again? Also, can the same plantar fascia be ruptured again? Why did this even happen in the first place? How can I prevent this from happening in the future? Thanks Doc!!! Looking forward to hearing back soon!!! Great Vid BTW!!!! 😊

This just happened to me today while playing basketball. Heard a pop under my right foot. No bruising but it’s swollen. Does this mean i tore it and not ruptured?

50 percent plantar fascia tear medial area in December. did walking boot for 4 weeks. went for a follow up ultrasound in march, still a 50 percent tear. Any ideas how to proceed?

I am two weeks out from my foot going lame during a run. I’ve been doing all of the stretches and eccentric heel raises to try and help and nothing is working. I’m thinking I might have a rupture and need to get it checked out. It’s been hard given how things are in NY right now. I can’t walk without putting weight on it.

I'm soccer player and 6 weeks ago I rupted my plantar fascia. There was no bruised, no swelling. I wasn't able to weight baring and feel pain on my heel.. Doctor advised only rest without boot. PT use dry-needling and nothing more... I feel a big bump under medial side of heel and i'm still not able to stand on foot. I feel pain and popping also under my 1 MTP joint... Should I start wear a boot now (after 6 weeks of injury)?

@@justanotheralien7625 I hadnt been able to walk without pain by 1.5 year. Now its better. I even Play Football again but this foot isn't the same like the letf one.

I just ruptured mines today and I'm nervous to go to the hospital...so far the pain was in the heel but now its traveling from the arch, ankle area (throbbing) ugh decisions?

I know it's been about 5 days now. If no better, you should definitely get it checked.If it is ruptured, the right type of immobilization can go a long way. Good luck!

@@OrthoEvalPal Hi Paul, update: I am able to walk but just alittle slow with just some pain but like pins and needles. I am able to go downstairs but when I flex I get some soreness. I noticed standing helps but after 5mins I start feeling pins and needles even sitting and putting pressure. Laying down i was feeling pain but not really now and I stretch it and it feels so good afterwards. I take Tylenol to manage pain but the inflammation has gone down tremendously which probably explains why I am able to walk. I kind of already walk with a strut because of my hips being shorter than the other and the longer hip is forcing me to use that foot on the shorter hip. I have some decent walking sneakers by nike that I just been keeping on my foot for mobilization and mobility when I walk, going barefoot is when it hurts the most and I tend to hop alittle, is that normal?

I was playing basketball, I went to make a move, felt a snap and fell. It didn’t bruise but a little swell. I’ve been having pain in the bottom of my foot for a while. I don’t want to get a big doc bill so I don’t want to get it checked out. 😩 it hurts when trying to walk or flexing my foot a certain way I’m wondering if I ruptured mine.

Very similar what happened to me, makin a move off my left foot and felt that pop - no bruising tho really. Been about a month and still can’t run, how did things work out for you?

@@directordank it ended up bruising up, I got a boot and stayed of it for a few weeks and slowly started working out again. It ended up fixing my plantar fasciitis so I’m happy it happened. It was tender for a while, I would massage it and it felt like I had little pebbles in my foot.

Can this injury self heal over time? Mine occured about a week ago, swellen is gone but still very tender on the bottom of my foot. Have been icing frequently with light stretching but still have a difficult time walking.

1 week would be way too soon to see improvement. Give it 6-8 weeks if not longer.

Hi. Mri of foot and ankle just showed Edema in plantar fascia and fascia changes … ankle showed reactive marrow edema TIP lateral malleolus. Hurts to have sock on, sneakers loosely on… and radiates up my tibia fibia calf through knee to femur and podiatrist is letting this go… any idea what I’m dealing with? My Entire right leg hurts and throwing my gait off. Thank you!

@@OrthoEvalPal hi! Thank you for responding! So much discomfort today… the pressure coming inside my foot ankle and calf unreal… hurts to bend my knee. Yes mri 6 months ago showed calf sprains and shin splints too along with longus muscle strain.. docs have done nothing so I don’t know what to do. The pressure and pain feels like compartment syndrome bc of the pressure but they check for that too. Negative. What causes this pressure and edema? Thank you!

Is surgery really the best option? During surgery, you cut the tendon, isn't that bad to do? Does it grow back?

Hi Tina, I don't typically see people have surgery for this. They usually do well with immobilization and rest, PT and orthotics.

Hi Doctor, I am 30 years old male just heard a "pop" on my right foot. I think it's definitely a rupture. my only question: Do you need surgery or it will heal on it own ? I have no bruise or swelling.

They typically don't do surgery if it's fully ruptured. During the surgery for a partial rupture, they'll cut the tendon away from the heel completely and allow for scar tissue build up. Then therapy after a month to two months of being in a boot. Hope your foot is ok by now. Would love to know exactly how long it took for full recovery as I am a tennis player and I'm really curious about other's recovery time. Thanks and hope all is well.

@@tinaswanson9743 read my above comment. Btw, I am fully recovered from a fully torn plantar fascia tendon. I am back competing in tennis with full flexibility. No surgery necessary.

It could take 3 months to walk comfortably and up to 6 months before running.

@@OrthoEvalPal wow I've never in my life thought of suicide I even make fun of people who do but the thought crossed my mind reading this. My purpose in life, playing rugby is occupation, I feel so vulnerable is there things I can do to speed up recovery..... Anything to bring it down to two months

Yes. It may not function exactly like it used to, but I've seen patients who did very well without surgery. Depends on the size of the tear.

hey whatsup man i got a steroid shot in my foot about 2 months ago and it gave me severe pain i think that tore my PF and have been doing a epat treatment since a lil more than a month ago have already done 5 already and not really any releif, im thinking of going to get a mri scan to see whats going on with me and have started to use crutches and a walking boot like i did for a first couple weeks of treatment , i still feel like its inflamed and have pain even when resting. starting to get a lil scared since i havent been able to go back to my pool construction job since i hurt it. any advice man? would really appropriate it

Have you seen an orthopedic surgeon and PT? An MRI is a good idea if an orthopedist agrees.
