A runner with a partial tear of the plantar fascia asked…
“I am struggling with a partial tear - going on 6 months of pain with running. I’ve tried everything - PT, rest, PRP, and most recently embryonic membrane injections.
This injection was four weeks ago - with crutches for 5 days after and boot for two weeks.
I’ve also had four weeks of no running at all and still have the pain.
I would love to know what your “standard routine” is for this condition.”
This video by Dr. Christopher Segler talks about the standard routine for healing a partial tear of the plantar fascia in a runner.

I have a very clear memory of being on a run in January 2013, after several months without running, and felling a sharp pain at the bottom of the foot. I kept running until I got home, despite the pain. It's 2023 and I still can't run 10km without the pain returning. Aside from that and avoiding jump rope and harsh landings on jumps, it allows me to live a normal life. But now I want to move from cycling to triathlon, and I don't know if this 10 year old injury is in any way possible to heal, in order to make larger distances in running (the aim is marathon, for iron man). Thank you for this series of videos. I wish I'd seen them sooner.

I got injured end of August, but kept running on it making it worse for two more weeks till I couldn’t walk anymore. I heard a pop at initial injury, I didn’t think it was the plantar fascia, but months later , it probably is. I just got custom orthotics and neutral shoes and went from running with full pain to nothing for months to being able to do a quarter mile at a time at an extremely slow pace right now. I can life over 100lbs while walking but not with full flexion. Will I ever be able to run fast again? Or even sprint? Will it just tear again?

Hello Rodney, Sorry to hear about the plantar fascia injury! It sounds like you should really think about which options will help you get back to running faster. We could discuss all of the options given your situation during a web cam call. www.docontherun.com/talk-to-the-expert/

Long distance runner here and I finally pushed my plantar fascia to the tearing point. Dr tells me to take it easy so i have continued my twice a week strength workouts (switching over from my boot to custom ort and arch supported gym shoes for workouts) as well as instead of running now walking. My question is what I am doing ok to allow the plantar fascia to heal? I will say that ever since I stopped running as well as no exercises focusing on my calfs, the pain level has reduced. But I want to make I am allowing sufficient time to heal.

Got my MRI back and I have a partial tear in my plantar. Doc put me in a boot for 6-8 weeks. The boot is immobilizing my ankle, but is that the actual goal? Or the best thing to help me heal, to simply keep weight off my foot? Is that the real goal? Because walking in the boot, I'm still putting weight on it, and in fact there is very little arch support in this boot. I want to get better and if I'm going to be fairly immobile for 6 weeks, then I may as well suck it up and be immobile and get crutches.

@@HUGGYDRABITS I never got a great answer from this channel. But, I decided to go on crutches for my entire 8 weeks in the boot to take as much weight off it as possible. I swam for exercise (no pushing off the wall and used a pull buoy to not kick). It helped a ton. A month more of nothing strenuous after that. Then I started going for walks around the neighborhood and eventually going on 5-6 mile hikes with no pain, though it wasn't quite "right". Could be attributed to lack of use, or maybe that's my new norm. I started added in short run sections into my walks about 6 months after being off the crutches, just a little at a time. 3 mile walk, with a 1/2 mile run in the middle. Then slowly adding in more running. At about 3 miles of running, it started to feel weird and I got worried I was pushing it too much, so I backed off. Treadmill running seems to be better than road running for me right now. I can control my pace so I don't push myself and I have no pain, though it still feels "weird". I'd love to get back to 10 milers, but I fear that may never happen and I'm afraid to push it. Super bummed, but I am biking and swimming with no issues, so I'm planning a 185 mile bike ride on the C&O instead and try the Chesapeake bay 4.4 mile swim. :)

Hi Doc. I have a race this weekend. and I have an interstitial tear in the planter fascia. I train at gym strength and cardio 5 days a week and run has been reduced quite a bit from 50k to 16 per week. This weekend is a half marathon. Would you think some taping will help. I want to try and complete the entire event at a slower pace.

Thank you for watching and I hope you found the video helpful. I can't offer individualized medical advice based on that amount of information. However if you wanted to discuss your upcoming race, strategies for decreasing stress and strain to the plantar plate ligament during the race, and trying to make sure you can complete your half marathon without further damage, we could get on a webcam call or phone call to discuss in detail.
