Lore wise Lillith is the mother of all humans in diablo. While the father is an angel. The bald evil dude is I believe the first of her children who has essentially the power of a god.

Right after the guy said "Hail, Daughter of Hatred", it flashes briefly to show an angel in chains. That was Lilith's lover i.e. the father of the humans in Diablo, now imprisoned for his crimes.

Jae L Simply put, Lilith and Inarius, the angel, grew tired of fighting one another and, with like minded followers, they sought to escape the war and live in peace. Many fell in love and bore children, resulting in nephalem who were much stronger than their parents. And the union of angel and demon is considered a taboo. Lilith wanted them to live but her angel lover was so afraid he tried to destroy them

The way Elias speaks to the priest, like a parent trying to calm a scared and lost child. Everything is said so matter of factly... "There is no light here", is almost said reassuringly. It's probably one of the more terrifying parts of this trailer.

Fun fact: Lillith was mentioned in jewish mythology as first wife of Adam who was made same clay as he but she didin't want to be commanded by him so she left from eden and then god made eve from hes ribs and she was obidient to her like adam wanted. pretty much Lillith was made into evil creature because she had free will... makes you think about stuff eh (i might be wrong so correct me if you want)

So there was Angels fighting eternal war vs Demons. Inarius went and fell in love with Lilith and they made offsprings which are Humans... half angles half demons! Then Inarius inprison her for her cruelty but he can never kill what he loves , so he put her in eternal prison which they just released her , you need 3 willing souls. There was an ongoing war between Heaven & Hell. Lilith (who you just met) is the daughter of Mephisto. She fell I love with the Angel Inarius. They thought their union would end the eternal war and they created the world known as Santuary. There they created the Nephalem (who we play the game as) Over time nephalem became normal mortal beings. Heaven & Hell both were searching for Lilith & inarius. Inarius eventually turned on Lilith and she was banished to the void. Her being summons back after who knows how many millenniums is where that trailer picks up from. The trailers and cinematic from Diablo 2 are so good too.

I think the reason why they made the cinematic for Diablo 4 so gory is simply because pretty much the entire Diablo community complained that Diablo 3 was too light, colorful and cartoony for being set in a world as fucked up as Sanctuary. So I think they over-played the gore to be more or less like "See, we haven't lost our touch. Fuck you." kind of thing.
