I am so glad to have found this video. My initial reaction to sudden minor pain on top of my left foot after finishing my 17k run was 'stress fracture'!. I have tightness in the forefoot with a very small amount of swelling but no bruising. The run was a three stage run comparing different new shoes and on reflection I think I over did the lace tension.

so i have no bruising and no swelling but hurt to walk and run even got a x ray and it showed nothing any help

Recently switched to complete barefoot running ( mostly on asphalt), after 6 sessions decided to increase the pace dramatically. Then noticed swelling and pain in a particular metatarsal region (debilitating to an extent couldn't walk naturally). Followed your advice, rest and ice pack, now the swelling has gone, slowly resumed long-distance walks ( after 20 days) and in another 10 days, will resume light running. Thank you.

How is the barefoot running going? I wonder if you overused the extensors because you feared jamming your toes into asphalt. A reasonable fear. I'm thinking if getting into barefoot trail running myself

@@B10401 Completely recovered from the initial injury. Been running barefoot ever since. I also ran half marathon barefoot in 1hr 56min in March 2020. I'm pretty much acclimatised to - Stadium turfs, asphalt.

@@B10401 The entire course is asphalt, 3-4Km may be concrete in between. After the extensor injury took things slow, walked barefoot avg 4Km on asphalt for conditioning the sole. Rest, ice and contrast bath.. that's abt it. After the injury, it took ~4 months for the complete recovery.

I got this pain at the top of my left foot (right under/below my big toe) when I walk & it gets irritated at times to the point I feel like my foot goes out & I just drag the foot making it seem like I have a limp. I have zero arches in my feet. There’s no bruising, I didn’t drop anything on it, it just hurts when I put pressure on it.😣 I’m on my feet a lot & I am athletic.

Car ran over my foot about 25 years ago. I was aware 2 toes next to big toe were damaged, but did not seek medical advice. It became obvious the toes did not bend as they should, but there was no real discomfort. Just recently, I am having frequent 1/3 minute episodes of extreme-easing burning/stinging pain in the front foot/toe area. There is no discolouration or swelling just slightly disfigured toe joint from injury. The pain level is close to naked flame and occurs 10 or more times a day. Wondering if anyone has similar problem or knows what might be the cause.

Can this be caused by wearing to tight of a snorkeling fin? Went snorkeling and then trail running after and now I have all these exact symptoms. Fin was definitely rubbing on my tendons.

I was trying to parse out if I had this or a stress fracture, so this video was exactly what I needed. I have no bruising, a bit of swelling that isn’t in the toes, but two areas up the tendon that are painful when rubbed. So, seems that I have this rather than a stress fracture, which is good news to me 😁. Will still get it checked out, but this calmed me down a bit.

Can you get Extensor Tenosynovitis from gout attacks? I don't know if I do have Extensor Tenosynovitis but I have pain on the top of my left foot and I felt this pain since I had that gout attack a year ago. It's painful when I walk, when I go down the stairs and when I lift the front foot. This past year has been hell and I could really use some information.

I recently dropped a Barbell on my foot I got a fracture on my 4th and 5th metatarsal bone, I believe I also broke my extensor tendon but I can walk with minimal to non existent pain what is this? did I break my extensor tendon

I’m Very sorry to hear about your accident. However I don’t know what it is because I haven’t seen you, haven’t looked at your foot and have not a valuated you. The top of by foot has done this about 3 times for no reason it swells up and the pain is so bad i can barely walk and if I walk on it it gets worse

It is always important to really look closely to make SURE you don't have any swelling. About 1/2 of the patient I talk to over webcam have swelling, but don't THINK they have swelling.

What if the dorsal forefoot pain with swelling follows a really bad episode of 2nd through 4th metatarsal carpal spasm (what is called when those toes freeze in the extended position?) I had to sit down and pull my big toe back to get them to stop doing that. I dont know what made me think of doing that. But I was and usually am barefoot if not I wear very good shoes. Always. My entire life. I'm searching for suggestions because my insurance wont let me to the right doctor without the PCP getting to get his office billed first I'm experiencing swelling with heat also. Plus I suddenly have tailors bunion on the same foot. I'm a mess 🤪.

@@DocOnTheRun definitely not. I’m working on maneuvering strategies. But what causes 3rd and 4th toes to painfully get stuck in extension. You know, like when you stretch sometimes? I know people experience this. I am trying to find out what it is so I can use it as a symptom. Anyway. If you can’t or won’t let me know, I understand. Thanks for your informative videos. 🙋♀️🌹
