Therapy after total knee surgery is so important to obtain a good result. Patients need to work on range of motion, strength and endurance.
One of the common things I see are patients that don't have strength to get the leg completely straight even though the knee will straighten - this is called an extensor lag.
If the knee will not straighten at all it could be due to tight hamstrings, stiffness or scar tissue.
In this video I will show you what I mean by an extensor lag and a very good and simple exercise you can do at home to improve your strengthen so you can actively straighten the leg.
This is even good for people with knee arthritis, especially if your arthritis is bad behind the knee cap.

Extensor lag!! I've watched many of your videos pre surgery. Which were helpful to set expectations of all the work involved after. Now it's 3 weeks in, and going back to review some meet me where I am. The physical therapist found the extensor lag and quad weakness. Makes more sense now. And re-watching helps get the detail of the exercise. Thanks so much, Dr. Rosen

Adam…..I can’t thank you enough for your channel. I had a TKR yesterday and thanks to your prefab exercise routine and general understanding you have given me about the knee, I can virtually walk without crutches or a walking stick. I can’t believe how good this feels even knowing that a wave of pain may yet come. I’ve had 13 surgeries on my old knee so I’m wary of the pain knee surgery can bring. Honestly can’t thank you enough even at this stage. I’ll keep you posted!

Hi Adam, writing from the UK,, I just came across you and this straightforward, informative video, it provided the impetus for me to make a big push to getting my knee straighter and it worked, gained more strength in my quads which previously just weren't engaging, thank you.

ty, I have been trying so hard to figure out what my problem was to no avail. Just had my six week check up the doctor had no clue why I’m still in so much pain and can’t do the exercises properly. Four weeks of physical therapy has not helped so I’m definitely gonna try your Siri and see if that solves the problem. Thank you again.

Thanks for explaining this so well and all your othe videos as well I had my knee done 72 hours ago and listen to your body isn't the advice I needed didn't know if I'd caused more damage I will get on these instructions thanks again, I subscribed yesterday will be watching the rest of your videos.

Thank you so much Adam. Having watched this video I now realise that my physiotherapist changed me from this exercise far too soon, (after only 2 weeks). I’m 8 weeks post op at the moment and I have been noticing lately, that my operated leg, thigh muscle has become much smaller than my other leg. I also notice, that when I lift my leg it bends slightly at the knee, proving your words in the video, that my thigh muscles have become weak. Thanks again Adam, I’ve switched to your exercise from today. Thanks by the way for all your other videos.

Thank you! Been watching your videos. My first TKR is tomorrow. Been working on these exercises, but not easy -- I have a baker's cyst in that left knee that wreaks havoc after I do all this stuff.

Thank you Dr. Rosen..I just wish I had found you sooner. 3 mos out and only getting 103 ROM. Therapy is done after 30 sessions. I just bought a stationary bike and praying this helps. Thank you again for your great videos.

I’m almost 3 weeks post op and literally just asked my pt about getting a full extension. I will use this to strengthen and get that last 10 degrees. Thank you for a helpful video.

This is my problem. I am 8 weeks out from a TKR and I am stuck at 8 degrees. Hopefully, it is not too late. I will work on this big time from now on. It gives me hope. My PT has me doing several exercises but not this one which makes sense.

I've found that when patients are cued to push their knee down they end up pushing the whole leg down by engaging the hamstring and glute and not the quad.

Sir,What is the remedy for knee ffd 30 degree with further flexion up to 120 degree fllowing tkr?

30 degrees is a lot. You can try these -kzfaq.info/get/bejne/ZrSkZaietKvHeoU.html but you may talk to your surgeon and see if something could be done surgically - either removing scar tissue, putting in a smaller plastic insert, or revising the femoral (thigh bone) component
