CMS does allow up to 100% reimbursement for CRNAs when using the QZ modifier.

Very interesting video, I found it helpful. I’m getting ready to start CRNA school in May, and I’ve always been interested in opening my own practice eventually. I live in a no opt-out state, and I’d really be interested in learning more about how you would go about opening an independent practice in a no-opt out state. Any references or info you could share?

I would begin by working 1099 independently for other established groups. Build a network and relationships with other anesthesia providers and surgeons. There are many CRNA practice owners in the space you will connect with or know of when you start in your program.

Utilizing the QZ modifier, why do CRNAs only get 80% even though it is independent practice? I get that AANA are trying to get the 100% but why is it 80% even with being independent?

I suppose CMS thought they could get us for a bargain? No good answer for this one which is why we're working on changing it.

I just wish everybody could realize that all healthcare workers are a team and should collaborate CRNA are important without them access to anesthesia care would be very difficult in underserved areas

CRNAs train for like 3 years in anesthesia... not 8.

@@muthoni632 Right, but nursing education has nothing to do with anesthesia training... so the prior 5 years don't help with anesthesia. Medicine recognizes this, that's why it's 4-5 years of education post medical school.

If you see it like that, anesthesiology is 12-14 years and the training isn't comparable with CRNA training at all. it's 14 years of way more advanced training according to a total different model. CRNA's are nurses and don't train by the medical model. 8 years of CRNA doesn't even come close to someone in anesthesiology residence

If CRNAs are doing the same job as MDs they should be able to bill 100% not 80%

not the same scope at all. On the surface, the work they do in the Or is similar to what anesthesiologists do, they give meds, intubate, manage hypotension, etc. but in reality, they do not receive training on how to be pre-op consultants or how to take care of critically ill patients in dynamic situations. They do not pre-op patients for the most part, they wouldn’t have the training to cancel a case for example because a patient was in decompensated heat failure. They wouldn’t have the experience to insist on additional work-up for a semi-urgent case, and while some are trained to do procedures, many would not have the adequate training to perform neuraxial anesthesia or nerve blocks. Some continue to push for independent practise but we should all be very wary of that. They have their place in working under supervision and I have worked with many great CRNAs. When I had surgery, I had a CRNA in my room (with oversight from an attending) and I was perfectly ok with that because I’m a healthy patient and there was nothing complex about my anesthesia. However, if my 92 year old grandfather with cardiac and resp co-morbidities was having surgery, I’d like a physician to be in the room with him.
