I have actually thought about this question a lot. Gender specific insults, and how there seems to be a worse one for women then for men. I consider this an acceptable compromise. From now on, I shall poison males that I hate rather than use words.

Not sure about this particular instance, but it's often the case in movies and tv that people only pretend to consume cigarettes, food and beverages. Consider that often times scenes have to be shot and reshot over and over, sometimes dozens of times in a row. Only a very heavy smoker would be comfortable smoking 10 cigarettes in a row. Watch an old movie sometime and try to spot people eating invisible soup out of bowls and smoking unlit cigarettes.

Here it can be a term of endearment amongst your close friends (when there's no kids around), or your harshest insult. I think the former comes from an ironic sense of you not meaning it as an insult so it is somehow flipped into a nice, but extremely uncoof term lol.
