The video is about non surgical but most effective treatment of frozen shoulder. Everything done through a small needle. Three injections can help relieve pain in frozen shoulder and increase mobility or range of motion: 1) Bursal steroid injection 2) Hydrodilatation of rotator interval and 3) Suprascapular nerve block.
One of these or specially combination of these injections can significantly improve pain and increase joint motion or range leading to better physiotherapy which can then help relieve pain completely and allow complete range of motion. I am having AC, a week back I slipped and fell on ground with pre-existing AC condition, my frozen shoulder muscles damaged. It's even more painful now. On pain killers now. How much it will take to recover ? What will be time line to encounter the pain ? Does xrays diagnos the muscles damage ?

Hi...I am a retired cardiac anesthesiologist; I am 68 years of age. Firstly, you have good presentation skills. Now, I have problems in both my shoulders. Left side: non-traumatic recurrent dislocation of very long-standing duration (first episode in 1974). Arthroscopic Bankart's repair in Dec 2007 with good results. Open Letarjet operation in Sept 2020. Indication was spontaneous re-dislocation. Between 2007 and 2020, I enjoyed good functionality of my shoulder. Current status: good functionality. I am NOT going to challenging its stability. 😃. For obvious reasons. Right shoulder: first symptoms started in 1995/6 with painful arc, for which multi-modal management strategy was followed. I had repeated sub-acromial steroid injections. Since about ten to twelve years, symptoms have worsened, though, I have always had full ROM during remissions. MRI was done on a few occasions ln which, reduced subacromial space was reported. Some radiologists also reported a type II or III acromion. I continued with Physiotherapy and several USG guided subacromial steroid injections Of late, I had been experiencing gradually shortened symptom-free intervals between injections. Finally, I gathered my courage and subjected myself for surgical decompression of subacromial space with acromioplasty in February this year. 12 weeks after the operation, my pain has worsened with added complication of severe nighttime pain, forcing me to take continued analgesics including GABA-pentine class of agents. I am also forced to take benzodiazepenes for sleeping. Nothing has helped. My physiotherapist is discouraged to advance his treatment plans as it results in severe pain. USG one week ago (5 weeks post-op) has shown definite signs of early adhesive capsulitis. I am not a diabetic, and my general health, including other joints are in normal status. I take Telmisartan and Cilnidepine for controlling blood pressure. I have one question. 1. What would be the ideal time in my case for any possible IR modality of treatment? 2. What is the role of SSN block in my case? I would be happy to read any references that you think might be of help in coping with my condition. Thank you for your patience.

Hello sir. Thank you for sharing the detailed history. Due to various guidelines, it's difficult to share specific advice on social media comment section. Will urge you to just whatsapp your name on 9082394787. I will contact you and speak in detail. Answering principles now. When I treat an early adhesive capsulitis with injections. I usually use 3: a) hydrodilatation to create space within rotator interval. b) SSN block. c) Subacromial steroid injection. So that all aspects are covered. Even isolated SSN block works. Main intention is to allow physiotherapy better. Which is main treatment. But often limited due to low ROM and pain. 2nd best option is shoulder artery embolization. Where we slow down flow to the spaces where inflammation is going on - soon to be replaced with scarring. Devised by Japanese doctors like Dr. Yuki Okuno but now being accepted widely by the world including landmark trials by Dr. Sandeep Bagla and Dr. Ari Isaacson in USA.
