How do bile acid sequestrants work?
Bile acid sequestrants are used to treat hypercholesterolemia or high LDL levels. Anti-hyperlipidemia therapy seeks to prevent the complications heart attack, stroke and other ASCVD risks.

This pharmacology lecture covers topics such as dyslipidemia drugs. We talk about Bile acid sequestrants and how they block the reabsorbtion of bile, causing the liver to make more bile thus lowering LDL levels. Also we talk about the pathophysiology of LDL cholesterol, regulation of LDL, HDL, LDL and other complications of bad cholesterol buildup. Thank you for this video. I was misdiagnised with IBS 15 years ago as I had my gallbladder removed and was experiencing IBS -D. I could not leave the house without running to the bathroom or feeling pain every time I ate. As a result I have developed panic attacks and agoraphobia due to the fear of having accidents in public. It has affected my marriage, my childs life, my working abilities and lead to major depression. I am now taking a Bile Sequestrant and it has changed my life. I only wish that the doctor who removed my gallbladder informed me of BAM so I did not waste 15 years of my life in terrifying fear. I now have to spend time on the emotional and mental scar of leaving the house but I am hoping I can gain the confidence back knowing my internal system can function properly. Thank God for Science, Research and Doctors.

Exact same thing happened to me! It took me 12 years to finally figure it out on my own, even after being told it was definitely not BAM causing my issues. Glad to hear there are others figuring it out for themselves too. I wish the SeHCAT test were offered in the US,

@@basedgodjoe9541 not the OP, but my symptoms that were fixed from bile acid sequestrants were focused around a false urgency I would get soon after eating. It felt as if the food I just ate had already passed through me, which obviously was not the case(although there was definitely a rushed transit time I could observe through eating corn or some greens, 10-14 hours or so at times), but I would feel the need to rush to the toilet and then would be unable to go. This was all accompanied by pain almost all the time. If the pain subsided and I'd get up, the urgency and pain would then come back. I spent far too much time on or around the toilet with no relief. When I finally went, it was almost always diarrhea. I only ate 1 meal a day for around a decade, never breakfast or lunch. I would go to basketball practice having not eaten yet that day. I avoided going out to eat or going places where I would not be close to a bathroom with which I felt comfortable. I missed so much school, and since starting sequestrants ~1.5 years ago, I have missed 0 days due to those symptoms. I still feel pain and discomfort sometimes, but it pales in comparison. I now eat what I want and religiously take my Colesevelam alongside it. Good riddance low FODMAP diet. I think the false urgency is due to the accumulation of too many bile acids in the colon that would then draw in substantial water and salts(which I believe is a natural mechanism to form stool, why 5% aren't normally absorbed as it is), which, along with the gastrocolic reflex after eating, pushes food too fast through the intestines. Our already sensitive guts would likely feel that related pain and interpret that surge as immediate urgency when, in reality, it is still not quite there. I know this is late, but I hope Colestryamine has brought you some relief.

I had a similar experience. I suffered for 20 years after having my gallbladder out before a Doctor told me to take Bile Acid Sequestrants. It changed my life! I also was afraid to go out of the house!!!!

I started wuth resin cholesteramina powder about 6 weeks ago. I take one packet in water every morning and it helped immediately

Great video. I have had bowel issues since birth which has progressively got worse in the past 7 years. I have tried every IBS diet and medication there is : none worked. All my tests at the gp always come back normal. I am only now being referred gastro at the age of 27. I eat a balanced diet and exersice as much as i can (disabled). It does not matter what i eat or drink, healthy or unhealthy. They are now finally looking to see if i have BAM or IBD. Symptoms: Loose stools or diarrhoea (3-6+ or even upto 12+ when ill) Yellow ish when i wipe. Acidic or eggy smell. Can't digest certain foods like sweetcorn. Daily nausea. Fatigue. Bad days, i get chills, shakes, vertigo and feel like throwing up. Lower abdominal pain. Weight gain NOT weight loss. It's tough as the NHS in the UK is so behind in so many areas hence why it's taken so long to get help. I have been started of colestyramine to trial and see if they work. I am on day 3 and still no change. Waiting on sehcat scan and results from bloods & stool. Thanks for the video. Xx

I truly wish that you were continuing to make videos. The knowledge that I am gaining and understanding for pharmacology for nursing school has come through your videos. Being an auditory and visual learner can be hard in nursing school. Reading and pictures in a book do nothing for my brain. So this is why I’ve had diarrhea for the past 5+ years ever since I had my gall bladder removed. My gall bladder isn’t there to regulate the bile acid and my liver is just pushing it out. So esp when I eat anything fatty it makes it worse. My body can’t handle all the oil.

I'm having similar issues. Been taking cholestryamine and it's not working, Sucralfate had been working but my doctor took me off of it for some reason.

Interesting, so an elevated LDL in conjunction with slow/poor digestion makes sense. If the liver is full of bile but not releasing much and the gallbladder is always full, you’re gonna end up with unused LDL floating around.

Bile is essential and not in sufficient levels with many people. Incidentally high LDL appears to be highly over-rated.

@@jdakid156 I still do after five years. After a year I tapered down from one packet per day to one every three days. Seems I'll stay there forever according to my doctor

Great video. I need cholesterol. My levels are too low. Sequestrants are also used for malabsorption of bile in the intestines. I have primary BAM with retention at 2%

SeHCAT test. It's supposed to be a capsule containing bile salts and a radioactive tracer. You have two gamma scans done 1 week apart. Anyway, my symptoms have long gone since my last comment. @@glengarbera7367

My life partner just passed away very suddenly. The only thing he did differently was 3 doses of colestipol. I found him already gone in the morning. He was prescribed this for diarrhea. He had no gall bladder.

I have been gallbladder-less and drugless for over 15 years, if you stick to a low fat diet (low processed foods), I have no need for meds

People here describe taking cholestyramin for BAM (bile acid malabsorption), too. So I'm a bit confused.... Edit: in BAM it "just" prevents the laxative effects of BAs

This is actually one of the other uses besides lowering LDL. With no gallbladder, the liver can continuously drain bile, and it can cause "bile acid diarrhea" and there is also "bile acid malabsorption" there the body does not recycle the bile. If it isn't bound to anything, it is a powerful laxative. So taking a BAS can bind to all the excess (that would normally be kept in the gallbladder until you eat food) and prevent the laxative effect.

I wonder if this helps as well because I was exposed to mold for 4 yrs before then I already had issues with my digestive system due to not having a gallbladder and mold exposure made it worse even though it’s been a yr since I moved out of that toxic environment

Binding the bile acids should prevent them from having the effect of drawing in excess salt and water that cause the urgency and diarrhea of BAM. From what I have read on the matter, the 3-5% of BA that are not reabsorbed help to form proper stool in a healthy person, but in excess, they cause these symptoms. When they are bound by BAS, they do not draw in water and salt, which I think is why constipation is a side effect.

I have just been diagnosed after 20 years, all my hair has fallen out due to malabsorption !!!! Now I cannot get the prescription drug needed for this condition. I was told by the pharmacy that because of brexit. Also Colestryramine side effects are bad, causes malabsorption of vitamins ADEK Zinc. Which is useless.

My dr has prescribed cholestryamine because I don’t have my gallbladder any more it’s been yrs since I had it removed they didn’t tell me that I would experience bam I constantly have the runs to the bathroom after eating along with a lot of mucus and cramping which I thought ibs-d the Gi dr never gave me anything for this I have notice after 2 days of taking it I don’t have the runs yet and iam extremely full with just taking 1 -2 grams daily with just water I don’t know if this is an appetite suppressant or not so hopefully this medication will work

They should tell us when we have are gall bladders out it would speed up the help we need twice I was taken in to hospital by ambulance after bad extreme bouts of diareha wouldn’t of happened if I had be told about it but thankfully I had a sechat scan yesterday got to go back in to hospital one week later then get the meds to start living again any questions please contact me your not alone.

So if 97% of it is reused how does that work for people that don’t have a gallbladder anymore? The liver is still what produces the bile so how is that reused?

The different bile acids are reabsorbed (to the blood stream) in the small intestines (terminal ileum). Then the liver filters it out (probably it has some storage capacity too). Gallbladder has nothing to do with reabsorption. It's there to store and regulate the release into the guts (duodenum) ...but honestly I don't know how one functions without it. Going to look it up.

What else does our body need cholesterol for? A form from skin is made into vite D? Wonder if hi lipid people have low bile. SO they need to take (or put on skin?) the fat soluble vitamins (A,D,E,K) if take this.

Hey man I have a question, hope you can give me a advice? I’m taking 625mg colesevelam for one year and I have man boobs and belly fat. Do you think colesevelam can caused fatty liver ? And can I take choline (as it’s good for fatty liver) while I’m on colesevelam. Thanks
