I was recently asked a wonderful question by one of you regarding how hospitals treat patients who are using Suboxone and whether they have to worry about pain medication in an extreme circumstance. Great question! This video is my answer.

Here is the question for reference: "Can you possibly make a video on how hospitals handle patients that need surgery while on Suboxone? If I get into a car accident or something, do I need to fear not being able to receive pain medication in extreme circumstances? Also, I’m concerned about post-operative pain management. I’ve been putting off having surgery for fear of not being able to take pain killers during recovery. I have chronic back and neck pain and would benefit greatly from this surgery. This is something I will discuss with my surgeon when the time comes, but can you offer some insight and maybe some peace of mind for Suboxone patients?"

DR. When I hear a dr. mention Toradol for the treatment of postoperative pain. This reinforces my position on opiate-phobic Drs. The exact people are directly responsible for the opioid crisis and the Fantinly Poisoning of people who use opioids Legitimately. Now, these people are forced to submit to a routine of Peeing in a cup, in the office of some G.P. who Makes $400,000 per year. Hello from Toronto, Canada. P.S. i had a brain aneurysm, and I always remember people saying you’re in trouble if you get into an accident or something, and I remembered it when I was in the helicopter, and I just kept saying Bupinorphine , and they all went quiet, and then they all started talking again saying I was going to be OK, those helicopters in the sky flying people to the hospitals are angels, God bless them, and God Bless all who struggle with addiction ❤️

Sweet Carole, I had a brain aneurysm /artheria carotis interna/ in 1995. But unruptured. But anytime I can have a new one, I am on Methadone 40-50 per day. It is a good pain management and good for my depression. I would switch to Subi, but doctors are ignorant or some of them judgmental. It would be horrible if I have to beg for a proper pain management or if I was not able to answer I would depend on them. Because docs are human being so etical or not, they could be mean. So I know Subi is not good for everyone but I would be happily give a chance to Subi, if it has the same mood, energy effect and painmanagement effect.

How did they help u? I work in er and we had a patient that had a horrific fall. He was screaming sedate me sedate me. He was on suboxone they were giving ketamine and morphine but it didn’t touch it

@@mrs.childers8333protocol is to give high doses of the strongest pain medication available. You administer it in periodic doses until the patients pain is controlled, while monitoring vitals of course. Sometimes, the pain meds won’t work. Sometimes they will. It depends on how much Suboxone the patient is on.

I wasn’t even on Suboxone but had an old prescription from years ago when I had my daughter they look up my history and freak out on me I told that that was years ago after that the nurses treated me like I was the worst person in the world. Rushed me after birth so I could get up and take a drug test. Which later made me bleed out. It was the worst experience of my life.

We're treated like 2nd class citizens by most of society. And what's extremely sad about that is that we are some of the ones who need the most help, love, and support. I'm sorry you were treated that way. I've been treated similar. 🍄💗

Thank you so much for providing this information. I've been putting off dental surgery for the longest time because I'm afraid of the post-op pain. It's obvious that you genuinely care about your patients, and we need more doctors like you in this field. Thankfully, my doctor is amazing and takes wonderful care of me, as you do with your patients. I guess it's about time to get that procedure scheduled!

I've had kidney stones 5 times in the last 3 years, being on Suboxone it is absolute hell.. the pain each time is so unbearable and that mixed with the horrible discrimination i experienced by hospital staff has given me PTSD. Once the pain went on for over 6 hours, I started to cut my leg and bang my head against the wall as I was so broken and hopeless and just trying anything to distract myself from the pain. I did go to the hospital, they gave me an anti-inflammatory and said there's nothing they can do. The fact it can happen again at any time terrifies me every day.

Oh 😂 yeah I dealt with that since I was 13 it's the worst lot of water try to stay active to loosen it up lot of water not real cold it's easier to down when room temperature good luck

Suboxone might ALSO be helping create those kidney problems. I've been in MAT treatment for 3 years and a lot of the side effects say 👉 liver and kidney issues can arise

I had an abscess tooth and my face was swollen it was after hours and that dr treated me like i was a addict off the street he gave me nothing to help i went to my dr the next day and she called the hospital not the one I went to and told them they could give me morphine and painkillers and then of course i always have a plan someone holds it and as soon as I dont need it I go right back on subs but i was treated so badly by the dr at the first hospital love ya dr

@@DrBAddictionRecovery from my experience, i have had similar treatment with ANY possibly addictive medications. Ive been on suboxone for over 10 years and am so tired of it. I am tired of the stigma and labels at come with it. Tired of not getting equal or atleast better treatment as an individual and not some recovering addict. Actually i am treated as i am still an addict and its really distressing. I have skipped my last doc appt because of this and now nervous about what to do.

I had a kidney stone which was very painful and told the emergency room I was on Suboxone . They gave me tordal which did nothing . My pain got worse and I called a friend who got me ten Vicodin 325 . I took two and my pain was gone .a couple days later I passed the stone .

I had a torn bicep surgery while on suboxone. The surgeon spoke to my prescribing sub Dr before hand and agreed on a plan for post op pain. Surgeon left me out to dry. I called the office following day Dr didnt return my call. Prescribing sub Dr called surgeons office and wouldn't call her back. 5 days of crushing pain. Didnt sleep for a few nites. Nightmare and was always my worst fear.

I think many ppl are worried about the naloxone in their suboxone counteracting the effect of post-op opioid pain killers. Is it true that if u take suboxone the right way ie sublingually, the naloxone is inactive & it’s only added to the strips to stop ppl from misusing them eg by trying to shoot them up? I REALLY appreciate you answering this question (I think I posted it first!) perhaps I’ll go ahead now & have that total hip replacement 🥴! Thanks again for all your support, you’re absolutely amazing. Many ppl asking these questions have no knowledge of street drugs / shooting up & are in this mess purely by being prescribed opioids for waaaay too long. You’re helping reduce the embarrassing stigma & giving ppl their lives & confidence back.

Naloxone is simply ineffective when combined with buprenorphine in general do to the bupe having the higher affinity. The addition of naloxone was a marketing tactic.

Lindsey Ormsbee I’m still really confused about precipitated withdrawals. What wld happen in an emergency situation if someone on subs was given high doses of morphine by paramedics? Would they work for pain relief or would the victim’s morning suboxone stop them from working?

@@maclardy Morphine is probably not going to be as effective as Fentanyl, Dilaudid, or something like that. Precipitated withdraw only happens when buprenorphine is introduced after opiods are already occupying the receptors.

So true. I learned a long time ago that the patient must be honest with their Doctor. After all Doctors are people too. Often we try to avoid being honest with the person that can help You the most. My name is Herman and my question to you is how long does precipitated withdrawal last if I had a patient that took opiates heroin and took buprenorphine,Suboxone too early how long will they stay Stay in precipitated withdrawal? Does the medication eventually settle with opioid receptor sites? And if so how long will it take.

I always switch from my Subutex onto Morphine oral solution and maybe an iv PCA patient controlled analgesia which had morphine in too 1mg iv bolus which i could press every 5 mins as well as the continuous infusion. Then when it came to switching back onto subutex, it wasn't too difficult with Morphine being fairly short acting

Well, I don't know. You have to stop Suboxone 3 to 5 days before planned surgery. I had a hip replacement. They told me to keep taking Suboxone until the day before surgery. I got to post-op, then back to the room. My spinal block wore off, and none of the pain medication would work. They refused to give me any more than an opiate naive individual would receive, on top of the pain medication being blocked anyway. It was horrible. It was cruel. They could've easily just given me fentanyl, or whatever, while in the hospital. I left with a prescription for Dilaudid. 19 pills. Not nearly enough for a week. Thank God I had family on pain medication that helped me out. A week later, I got a prescription for Percocet, which would've been okay, (8 5mg per day), but my insurance only paid for 4 a day! The next week, I just paid cash, and two weeks later, off pain medication, back to Suboxone. A year later, I had to have hip surgery AGAIN! A new doctor, who specialized in tendon repairs. The first doctor wasn't the best. That tendon should've been fixed when I had the first surgery. This time, a new Suboxone doctor as well. We communicated the issues between the doctors. My Suboxone doctor put me on two 30mg time-release morphine for 3 days before surgery, and a prescription for Dilaudid after. I had a good surgery, and pain medication was only needed for a couple of days. This is why you have to have a plan. You can't get major surgery while on Suboxone. I'm going to have the other hip replaced, soon. I will be much better off the next time.

I’m in a situation where I need a wisdom tooth pulled. I’ve come down from 12mg subs to 1mg… took it 2 days ago… how long will this 1mg block others?

@dustinsanchezmusic5851 I'm not sure, but I'd assume the blocking wouldn't be as bad as your full dose, but at least a day off, you should be okay. Not good, but okay. If you're into withdrawal, it's most likely out of you enough to stop blocking other opioid sites.

Yep. Had a surgery while on subs. Not sure the exact meds, but three doctors held me down, lifted up my arm, and injected me in the pit with something that knocked me out. TKO! Injection hurt big time, but it did knock me out. Communication is key.

I was on Suboxone for a year and I had to have my gallbladder removed. All they said is lower my dose because I was taking 16 mg a day they said to lower my dose to8 mg a day and I would be fine. But what I actually did was stopped the Suboxone for about eight days. So I had my gallbladder removed and when I woke up I was in such severe pain and they were giving me fentanyl and also they gave me Dilaudid and it did not take my pain away. It was an awful experience. Some doctors don’t know that the Suboxone blocks the pain meds even after you stop taking it sometimes it takes 15 days to actually feel a pain pill like Percocet.So be careful if you have to have anything done where you need pain relief if you’re on Suboxone stop the Suboxone 15 days before and it’s very hard to do but if you don’t do it you don’t feel the pain meds and you suffer from pain that’s awful.

this is exactly why i havent gotten neck & back surgery..i cant get pain meds for acute issues so im scared to let them do ANY surgical procedures....so i just suffer (even though i only take 8mg per day at this point)

I just had hernia operation 3 weeks ago been on Suboxone for six years they gave it to me in the hospital after the surgery and it blocked the morphine I was in so much pain I couldn't stand it I refuse the Suboxone for the next 7 days and I am now 3 weeks off of Suboxone and two weeks off of any kind of narcotics

I am going through the 2nd of 2 shoulder surgeries including full ac joint reconstruction grade 5. My doctor switched me over to hydromorphone and the back to bup when pain meds were no longer needed. He gave me the option to increase bup to 32 mgs day. I tried it for a few days and pain was too intense, so at that point he initiated hydromorphone

Help i need help like yesterday. The pain management center i go to has me on 4 10 mg oxy an a durgesic patch 100mg for 72 hrs my pharmacist said they are slowly killing me. I want to stop all this an go on suboxone. Help me Dr

Greetings Dr B. I was supposed to have shoulder surgery last WK from a torn rotator cup. I am on 16mgs Suboxone a day. The drs said I had to quit Suboxone 3 days before the anesthesia guy, unfortunately I fell through the cracks there was no discussion between my so called Drs!! I am opiate dependant and bipolar with depression,so naturally I am being denied some level benzdiazaprine? Until next month I see mental health. I am about to give up here!! I’ve been trying to research the correlation between Suboxone use and depression and other brain chemistry problems. I feel like this topic is often overlooked...? I get my Rx from my pain specialist and don’t really feel comfortable discussing depression with him. I’ve tapered on my own from 24mg to 2mg over the course of a year. He wants to increase my dose for pain, but my mental state is more important to me and feel like I’d be better off without it. I can’t help but feel like that am ruining my brain by staying on this medication. It served its purpose at the time, but now I feel it’s doing more harm than good. I understand there can be a million causes of depression, but is it possible that Suboxone is numbing my emotions and preventing me from being happy?

There is actually some research showing it actually helps with certain resistant depression types. This is consistent with the dopamine hypothesis. Some people do say what you say as well. Hope that helps.

I have been tapering too. Tired of the unfair treatment by most doctors. Ive been refused ADHD meds for years and am suffering bc of it. I cant function and my life becomes chaotic and very disorganized then leads to depression and anxiety and i currently am so upset about this. If im not on that med i end up sleeping a lot of the day and avoid going out. Skipped my last doc appt bc they arent helping.

Gems by Jackie I’m sorry to hear you’re having troubles too. I 100% believe that Suboxone has caused my depression, chronic fatigue, brain fog and lack of focus. But I do think that tapering, VERY slowly, is the key. I got down to .25mg/day and was feeling normal again. But I got lazy/comfortable and hurt my neck again so went up to 1.0mg thinking it wouldn’t be a big deal. I don’t feel good at all...again! I know it’s all a mental game and the brain needs to “heal.” I hope, I hope it does get better! I just try to stay busy and push thru... that’s all you can do.

I’m going to get shoulder surgery , I’m afraid of the pain scale after or before. I’m afraid that the Suboxone’s not gonna allow me to have the surgery I’ve taper down to 2 mg should I be off of the suboxone completely before going into surgery? And can I take the pain medication after?

I tore my MPFL and had to have it replaced and I chose to go the cadaver ligament route. I also refused to take anything but suboxone for the surgery. Since they drilled 3 holes through my bone and he removed 8mm from my femur bone to keep my patella in place (I’d had a history of subluxation before I tore my MPFL). Of course my hardheaded ass was like “oh I’ll be fine with a nerve block and suboxone”. It was ROUGH, I won’t lie. Partly because the nerve block didn’t take. I was in some serious AGONY. But only for about 10 days. The doctors did try to talk me into switching from suboxone to roxys or fentanyl for surgery but that terrified me so I said no way. Like I said, I’m hardheaded…..so I was in excruciating pain for a good 10 days, especially since physical therapy started 4 days after surgery.😳 But I made it through! I’m fine now. It’s been a year and a half and my knee works great. So to me, it was worth it to not have to switch!

Can you make a video letting suboxone patients know what kind of over the counter pain killers or herbal remedies are ok to take while on suboxone? I have been searching and have found nothing. Im in severe pain right now due to a sinus infection and dont know what over the counter meds i can or cant take. Please and thank you

Unfortunately most drs are uneducated about suboxone. Most assume u get high from it. They don’t realize that it gets rid of the high feelings

Dr. B Addiction Recovery I have had pharmacists refuge low grade pain killers after surgery like I was giving them a forged Rx. Both drs knew about my suboxone and approved it! But the pharmacist is uneducated! But I always before any procedure let everyone involved what’s going on! I’ve actually changed pharmacy over these issues

It isn’t true that you cannot get high off suboxone. But the high is so subtle it’s really not worth it.

I was in a motorcycle accident that wasn't my fault. UCHealth has let me suffer since the day after I had a plate and screws put into my arm. My ankle was crushed and I am home with the hardware. I'm supposed to be okay at home, waiting for the swelling to go down to get follow up surgery. I need help. UCHealth Central, in Colorado Springs thinks I should be okay on 5mg oxy. I'm 250lbs. This is discrimination and I do not know what to do. I do not want to go back on suboxone, because of how I've been treated.

If your given morphine on top of buprenorphine its gonna take a whole lot more than normal to help,because the morphine has to break through the bup and it takes a lot.

@@DrBAddictionRecovery basically I was in withdrawal kinda unexpectedly and was asking if I could go to the ER for bupe and how to get started....I went this am. It is crazy I live in the San Francisco bay area, not like some rural area. ER doc told me she had never prescribed bupe before and "wasn't comfortable" with it. Also I'm 39, am a wife and a mom, a house and a pain pill problem. No one would probably guess I can pop 100 mg of oxy and barely catch a buzz..... Anyhow, a coming together of circumstances has made getting painkillers not an option (well not rx ones, I can get fentanyl or heroin all day long but I haven't ever gone there and really don't want to now) So, after insisting she consult with someone in psych she gave me a single 4 mg dose, given in the ER and none to go home. I have an appointment with someone in mental health/chemical dependency next Thursday.🙄 I have Kaiser btw. The meds def took the edge off and I'm feeling okay right now, but I don't know how long this single dose is going to last and what the heck am I going to do until next Thursday....

I have unfortunately been to the hospital on both subs and one E while I was very first getting off opiates and was on methadone. Each time was for an injury, obvious physical injuries. On methadone, I was treated awful. On subs, slightly better, it seems they know enough that subs aren't wanting pain meds, methadone they seem to think we still just want drugs.. Even though each time I told them I was on which drug, how much and time of last dose. And told them each time I didn't want pain meds, I wanted to see how bad it got before that. The stigma is ridiculous. No bedside manner compiled with ignarance will kill someone since they don't even want to go to the hospital.

I think when you're in an accident it's also common to give ketamine (with a benzodiazepine to suppress the hallucinations). An ambulance guy told me once it's one of their most used meds. I'm not a doctor but this should send you into a space where you're not aware of the pain. And I don't think it's dangerous to combine with buprenorphine if you're monitored.

Great video. I was on 16 mg of Suboxone and had pneumonia. Originally treated for bronchitis and it didn't work and became pneumonia. Doctor wouldn't give me codeine cough syrup for cough because he said it wouldn't work. That was fine with me as I used OTC cough med and it worked..interesting how the Naloxone work. So, if you're getting a tooth pulled, can you take pain meds juzt like affterwards with Suboxone? Aren't the doctors that give you narcotics as in a car wreck just flooding the receptors and over taking suboxone? I read they may have to give you much more of a dose for adequate pain relief? I think I read it on the sheet that pharmacy gave me. I'm a huge nerd. 😂👍💝👍🇺🇸💝👍

Great video! I'm actually having a breast reduction in November and am currently on suboxone. I'm very scared going in on sub and curious how it works if they do gv u pain med and I'll be on sub for surgery and after. I'm wondering how u don't go through persipitated withdrawal. I did that when starting sub and can't do that again. Also wondering if it's better to switch to subutex for surgery? Thank you

@@DrBAddictionRecovery Won't that put you in withdrawal from Suboxone?? I've tried to get off Suboxone, even tapping and it is hard. Going off cold turkey for a planned surgery even, would cause Withdrawal. I don't understand your response to this question ❓ I'm facing several surgeries and the Suboxone dose I was on didn't do enough for the pain. I too am finding that too many Doctor's don't have a clue about Suboxone. I fear too that in an emergency situation, I'd suffer tremendously.

Dr B saying they’ll have you stop suboxone 3 days before a surgery seems to contradict what he says in the video. He said during surgery, whether planned or emergent, they’ll just start pain meds ON TOP OF suboxone! So, which is it Dr?

Yes, I was given both my suboxone, as well as pain medication, after my cesarean. I got no euphoria from the pain meds, it just helped manage the pain afterwards

Suboxone is a medication used during all phases of recovery......from active addiction (when abused), detox, early recovery, as well as the maintenance phase of recovery. Some people need this medication long-term & there is nothing wrong with that! This type of thinking only perpetuates the discrimination people in recovery already receive in their own communities & it is important to educate & support those in recovery.

Hi Dr. B. I am a 67 year-old retired teacher. I am addicted to heroin. Since the war on opioids started , Narco and Percocet became rare. My pain management ended abruptly and so did my normal life. Heroin became my pain management drug for obvious rreasons. The war on drugs did it's job I belive it was intended to do. It killed a lot of undesirables and turned the rest of us into criminals. I just saw the video you did on hospitals and being on opioids. Thank you. The prior 3 surgeries to my knee replacement one, which was my last, I marked Narco on the pre-op sheet, and no one mentioned anything and all went well, The only slight issue was I needed more pain medication then they were going to give me, but saw I needed much more. My question is, can a hospital, orthopedic sergury center and surgeon, and your original surgeon, refuse to treat you because I marked Heroin on the pre-med sheet? It's been 7 months since I was 1st told by the sergury center and surgeon that they would be dissmissing me, 6 months since my original doc. declined to see me, and 5 months since I was told that the emergency room in the hospital didn't have an emergency doctor that could do my type of sergury, and they would medivac me somewhere in the state when they find one. 30 hours later, I left to go home. On my way out a doctor and nurse reminded me that no one in the town was going to help me. My knee has been infected for almost a year and I can't get help. Can you suggest something please. This is getting very difficult to deal with. Thank you

I go to spero for my suboxon and i had a bad tooth pulled and she did surgery by drilling my bone away and my dentist and my Suboxone clinic told me no pain killers deal with it it was the worst week ever i was in horrible pain these clinics only care about money and not getting into trouble

In my opinion, (burophine) is a lot better for pain management than a full agonist. In Europe it's been approved for outpatient use for over 20 years.

I have had a few surgeries and used bupe for my pain but each freaking time the inject fentynal even after I say it will not help. What is wrong with anesthesia people? They are cookie cutter and stick to their way no matter what. After surgery same thing-'Do you want pain medication?" No! I have bupe and it is working.

I was going down for op and told a doc I had 600ml day codeine addition. Told not to worry about it. In the op room I was hot up with fentanyl, which did nothing. Docs and nurses now swarming round puzzled. Whoever I told clearly didn't pass the info on. Another full hit of fentanyl and I was gone. When I came round I repeated it to my nurse and she said I should have told them 😂

I thought you said in a previous video that the Suboxone kicks other opiates out of the opiate receptors, so they don't work. I am on Suboxone 8mgs for pain, not addiction purposes. I have MS, and suffer through massive migraines, multiple times a week. I've never considered going to the ER, because I didn't think they could break through the Suboxone to help with the pain. Please advise. Thank you.

Great question and point. First of all, I hope the MS is well managed. Great choice for pain meds. Now, you are talking about breakthrough pain vs our discussion. With out going into details, you are correct in my point on old video, but does not pply here. As for your case, if you are having that much pain on 8mg of suboxone that you feel need to visit ER than one thing to consider is increasing the subs. If you think about it, the point of it for your case (chronic pain) is so you do not use straight opiate agonists. I would suggest talking to the doctor to increase the dose. Ultimately that is up to you and him, but my MS patient is on 24mg a day. I sur hope this helps and thank you.

Zephyr Medical Group, thank you for your reply. It's kind of complicated, I started out on 12mgs, but the doctor I was seeing left the practice, so the new doctor took over. He slowly started to wean my dose, without listening to my concerns. He said if I didn't start weaning now, I would never get off of it. Long story short, he is no longer with the practice, and they bumped me back up to 8mgs from 5mgs. I have an appointment next week, I'll talk to them about increasing the dose. I'm looking for a new physician, since the practice I've been going to isn't the best. I live in Northern Cali, not many choices where I live.

She said that he had been weaning everyone that he was seeing, with no reasoning. I didn't start Suboxone to wean off of it, at least not in the near future. It helps.

Yeah I’m so done with partial antagonists. We all know the subs will block any relief. At least methadone they can still make you comfortable. Really sucky part of bup if you have actual pain.

Hello Dr. B! Thank you for your very informative videos and the time that you take out of your very busy day to post for us. My question relates to anesthesia. I need to have a tooth extracted and the dentist referred me to an oral surgeon for sedation. I am taking 8 mg/day of buprenorphine tablets for pain mgmt. Someone told me that it is very hard to get put to sleep for any type of surgery while on bupe. She also said that it's possible for me to wake up during procedure bc the anesthesia doesnt work well with bupe. Is this true? I am so afraid to have the surgery done. It doesnt logically make sense to me that the oral surgeon will not be successful with sedating me but there is a tiny bit of hesitation now bc she said this. Lord knows, nobody would want to wake up in the middle of a surgery! Thank you in advance. Many blessings to you!

Sedation isn’t needed for a tooth extraction. It takes minutes & you don’t feel a thing.

@@crooked-halo I beg to differ. At times it is, especially when there is an abcess/infection and the infection eats up all the novacaine. Nobody wants to feel that! Some people have a very hard time getting numb. I'm one of them.

Same experience in oral surgery. I don't remember a thing. My surgeon was aware I was on Zubsolv, but I don't believe he did anything different for my sedation than if I was not taking it. He did send me home with eight 7.5mg Norco tablets. I didn't think I would take them , but because I cannot take anti-inflammatory meds, I found I needed more than Tylenol as the Novocaine wore off. I called the addictionologist who prescribes my subs for direction. He told me I could take the Norco as prescribed as long as I continued to take my Zubsolv 2X/day. The Norco didn't relieve all of the pain, but it relieved enough for me to sleep. I had zero euphoria from the Norco and assume that's due to the naloxone. A few months later, I was being prepped for a different type of surgery (not oral) and the doc handed me a prescription for an opioid for pain relief afterwards. I asked her if she had spoken to the Doc that prescribes my Zubsolv (as I had made a point to tell her a few days earlier at our office appt). She seemed to recall it and realized she hadn't followed through. She took the prescription back and told me to just take Tylenol. Once home, I called my Zubsolv doc and he told me to take 3X my regular Zubsolv dose for 2-3 days. It wasn't as much pain relief as I would have liked, but it taught me the lesson that I need to make sure the surgeon in any future surgeries actually follows up w/my addictionologist BEFORE surgery. It's sad, but true that many doctors and nurses are going to treat us like lepers because we don't hide our addictions. I need to have my uterus and cervix removed and I'm petrified of not getting adequate pain relief afterwards. However, in both cases, the anesthesia worked perfectly. We all have a different pain threshold level. Ignore the other reply.

pain doctor makes me sign contract that i cannot get ANY other pain med even with surgery or dental work. so many ER rooms post no opiates given. they are sending people home with tylenol after surgery and broken bones. they will NOT help. you get things like its our policy, its the law, Dea says we cant do this, all kinds of excuses. they WILL and HAVE allowed thousands of people to suffer greatly. all for the sake of overdose. our overdose numbers have gone up 400 percent in this country since switching from time tested medications. i appreciate what your doing trying to ease peoples minds but for each doctor or ER that will help there are 2 that wont help. its a huge problem but until it effects you then people dont know what they are in for. they havea bad day go to ER after an accident and find yourself locked in pain prison willing to do anything to make it stop. P.S i would love nothing more than to be wrong about this....

I tried this once and went into withdrawal bad! I was on 12mg subs.. I’ve lowered down to 1mg and have a major wisdom tooth pain. I took 1mg of sub 2 days ago. I’m needing a pain pill as this is killing until I get to the dentist. Wondering if it would put me in withdrawals again though…

If being kept on the suboxone for pain management after surgery is there a time needed to wait to take that next dose post-op? Does the medication they use during surgery going to interfere and cause precipitated withdrawals if taken right after for pain management?

Hey dr. B . Just wondering because I am on Suboxone maintenance for addiction. My question is : if I were to take hydromorphone instead of Suboxone for one or two days ,( not taking the Suboxone as well on these days ) would I be able to continue taking the Suboxone after being on hydromorphone for a day or two with no precipitated witdrawl. Would my receptors still have enough buprenorphine on the receptors after two days not taking sub, but just hydromorphone. Enough in the receptors that I could take the sub again and be ok. Kind of a relapse harm reduction question

Here’s my experience with surgery so far. My sub doctor said to take my last dose of suboxone the evening before my knee replacement surgery. I had general anesthesia at noon next day, and a nerve block when the surgery was over. I was fairly pain free till the next day when the block wore off. The surgeon prescribed Norco 7.5 which did little to ease the pain. Next day still in agony, like a hot knife in my knee so he switched me to 7.5 Percocet. I literally feel no pain relief even when I take 2. After physical therapy I’m nearly hysterical. My sub doc suggested going back to subs, on a heavier dose. Has anyone been through this? Doc?

Just as a follow up, in case someone else is looking for info as I was, after I posted I waited about 16 hrs and then took my normal sub dose, 8mg. It was definitely the right decision for me and I wish I’d done this sooner. I can’t explain why I got so much pain relief, but I did. More so than a double dose of any of the painkillers I tried. I’m a month out from surgery and taking an extra dose of sub every day. Hoping to be able to start tapering back to my normal dose in another week or 2.

@@dustinsanchezmusic5851 my sub doc explained to me like this: it’s ok to take opiates on top of subs, but not the other way around. I stopped subs 12 hrs before surgery. There was no problem with anesthesia. The opiates I took following surgery did not cause withdrawal but they barely put a dent in the pain. I had previously been on 12mg suboxone so when I gave up on the opiates for pain control, I stopped opiates, waited 12 hrs or so and got on 16 mg suboxone and that worked beautifully. My surgeon really didn’t care what I did. Sad how the medical profession talks out of both sides of their mouth. They act like there is actually help for addiction. Just try finding some when you need it. To clarify, I was on 12 mg suboxone per day before surgery, now I’m on 16mg per day. It is going to be that much harder to taper when the time comes but I did what I had to do to get through the knee replacement.

Prior to this I was in pain management for a year taking oxycodone, and years prior off and on the opioids for medically related chronic pain. I switched to suboxone tho after the year of pain management. I was clear with my surgeon and my sub dr . Long story short, I was told to stop the subs at least a day prior to surgery and the sub dr gave me a post op prescription. Waking up from surgery I was in considerable pain. I was on IV painkillers for a day which really to me felt it wasn’t taking the edge off of my pain, then the next day switch to 10mg oxy every 4 hours. I really regret the years of taking opioids because I’m assuming it made my tolerance high and was the cause of so much discomfort recovering. Not a good experience

If u lost your meds should you go to the e . R on a sat or Sunday? What will they do ? I have heard they treat u like a drug addict . I lost mine last night and don’t know what to do .

Why would you ask if you should go to the ER on a Saturday or Sunday? Lol. Weird question. And people who are serious about the suboxone program and staying clean dont LOSE their meds. They know where they are every minute of every day.

You buried the lead here and the key phrase from all of this is "HOPEFULLY when you get to the ER the department is familiar with managing suboxone vs straight opiates." Which is a HUGE problem. There's WAAAAAAAAAAAAAAAAY too much hanging on that HOPEFULLY. I've seen roughly a dozen doctors for suboxone and talked to another half dozen surgeons etc. trying to protect myself from a nightmare and NOT A SINGLE ONE KNEW WTF they are talking about. It's SCARY how ignorant even the prescribing Dr's are about suboxone. You're then left with either trying to educate a Dr that likely has an ego the size of Jupiter (along with a disdain for you because you dare have addiction in your past) or just accepting that you're F'd.

Teenie, they can easily work around methadone. but for goodness sakes, be honest with them. I had to have spinal surgery and was 100% up front about the methadone..and weed I then smoked. Anaesthetist was the one who picked up my ''Long QT'' heart issue from methadone../Plus he said ''Normal doses of morphine won't work..so we'll give you as much as you need.

I have had two major surgeries all I did was take it in the morning then the pain meds still work then just had my wife bring my prescription in once they cut pain meds start taking Suboxone at that point no problems

I was on 8 mg of subutex for 6 years when I had to have an emergency c section in 2014. Afterwards, I was given 625 mg extra strength Tylenol, as needed and fentanyl patches (on or hear the incision site). These didn’t make any difference at all. With or without patch on my body, pain was exactly the same. My doctor and the hospital were all in communication with each other. Still, for some reason, this was all that was ordered for me. I ended up telling them if they didn’t do more for my pain management bc I was so miserable due to being in major pain, I told them if they didn’t make me feel better, that I was going to kill myself. After this, I was put on 24/7 suicide watch by the hospital. This included social worker and all kinds of other people that I didn’t need to see. I also was required to have someone from hospital in my room 24/7, in order to physically watch that I wasn’t killing myself. They cut my body opened three inches and then stretched a six pound baby out of that three inch wide incision, so what did they think was going to happen after doing this? After my suicide threat, I was given anesthesia from waist down, catheter for urination, etc, and I stayed this way until I left a few days later. I wonder what statute of limitations is for doing something like this bc we should have come home and sued the shot out of them, but instead, my entire family including my mom and husband, act like I’m the one that was unhinged and crazy bc of this situation. This just makes me sad bc unfortunately, I know very few doctors know what to do or do what is right, even when they know, and I still feel the medical profession as an entirety doesn’t feel this is important, and it isn’t an issue.

You literally said nothing here…. The concerns people have are REAL. Suboxone blocks the effectiveness of opiate pain meds by 99% with a half life of up to 37 hours. So in an emergency you can’t wait for it to wear off. People aren’t worried about precipitated withdrawal. They are worried about suffering during and after an emergency or surgery. Surgeons use fentanyl to reduce the bodies natural reaction to extreme pain like sweating, twitches. Suboxone will block the fentanyl in a potential surgery. There are dozens of case studies of patients on suboxone WAKING UP during surgery because they didn’t tell their doctors. And just like the medical establishment your response here seems to be “we don’t know what to do” If you’re having a planned surgery the recommendation is to wean yourself off the suboxone and have at least 24 hours with no sub before the surgery.

Tramadol and tapentadol works. Some say Ketobemidone(Ketogan) works as well. For me its no meaning what so ever to take morphine, oxy, heroin or somerhing in 5-6 days after buprenorphine. A glass of water would have the same pain killing effect

Naloxone lasts days if not weeks. Opioids will do nothing on top of suboxone.

I just got out of the hospital today after being in there for a week they went ahead and just gave me painkillers on top of the Suboxone. Now I'm home and I've been advised by the Suboxone doctor that I should wait 24 hours, not having anything... no painkillers no Suboxone but I need to wait 24 hours to restart my Suboxone which is three strips a day 8 mg each strip...can u advise ? Would you follow this protocol is it were you
