So you've got a client with complex hip/low back/sciatica pain, and you'd like to really mobilize their body as you work with it. A solution I've found is to do a pre-massage routine while the client is still fully clothed. It allows you to be more vigorous, move the body in an easy and natural way, and work from lots of different angles, all without having to worry about draping.

I've found this type of massage quite useful for clients with sciatica and anterior hip pain. If they're experiencing either (or both) of those symptoms, it's likely that there is a tug-of-way going on between the posterior and anterior pelvis. Psoas and iliacus are staying short and becoming irritable (and irritating the inguinal ligament and iliopsoas bursae), and all of the posterior hip extensors and rotators, like gluteus maximus, piriformis, and their friends, and inflexible and tight. There may even be some spasm.
This is an arms race, and we need to calm tensions between the two parties. To do this, I find it useful to incorporate lots of rocking, jostling, and joint motion. By doing this as I work on the legs, hips, and hip flexors, I can encourage tight muscles to release their guarding and tone. While this effect is temporary, it can provide some excellent information to the nervous system about what the body is capable of doing, and how relaxed it can safely be.
Note: Some of the techniques that I demonstrate are easy for me because I'm pretty tall, and I don't have much in the way of mammary tissue. If you find that you can't cradle the leg without breast tissue being in the way, try turning your torso more to the side and using your lateral rib cage. You can also bring the leg further up and onto your shoulder (thanks for the tip, Beth!) as you work. Experiment, and stay in good communication.

That's cool that you suggest videos to your client. I had thought that it would be weird for me to do that because I was thinking what if the clients think that the reason I'm showing them videos of other MT's is because I don't know what I'm doing as a massage therapist.

I have a client that insists on being fully clothed and your videos have saved my life! We did not go over clothed massages in school beyond chair massage and I felt so discouraged the first time this clothed client came in.

As an LMT, I have learned so much from your videos, way more than I ever learned in school! This clothed low back/hip routine with stretching is what I need someone to do on me NOW!!! My pain is excruciating in all the areas targeted on this video, just had MRI done this morning, results in two days. This routine looks like it would bring me much needed relief!! So sorry to hear you're in pain, Victoria! No matter what the MRI shows, I just want to make sure you and your doctors are pursuing physical therapy. It can often get skipped over when dealing with hip and pelvis pain, but there are PTs who specialize in those very areas (and even non-specialists can provide a useful regimen of stretching and strengthening). Basically, I wanna make sure your doctors are exploring that side of things too, and I'm sorry to jump directly into giving advice :) Best of luck!

Wow, Ian, such good stuff! I'll be incorporating much of this in my work with my clients immediately. I'm still in massage school, but intern (for free) at a local chiropractor for my clinical school work. Hip restrictions are something I see a lot of, but have been a little weak on dealing with. This sequence is considerably better than everything else I've run in to-- and I research/read/view quite a bit. Your work has been very influential for me. Your students are very fortunate!

This looks great and not too different from some thai massage moves. I have a client coming to me tomorrow complaining of unusual pain in outer thighs. He wanted deep tissue but I was thinking of this kids of thing. I'll maybe utilise some of my thai and stretching with what you've shown here. My challenge will be to do it on a massage couch and not on a thai matt on the floor!!! Thank you for all your videos 😊

I’m coming up on my one year anniversary and have been feeling this work is my next level but have been intimidated to try it (have no guinea pigs besides clients). I look forward to incorporating some of your suggestions. FYI: I supply my clients with disposable boxers (black is less see through). MS-I continue to enjoy your videos. Thank you. The majority of clients I see are middle aged. My clients are typically a little over weight, limited flexibility, arthritis, varicose veins etc.. Their body parts are a bit more cumbersome to maneuver during stretches than younger women/men. I think it would be beneficial to see massage models that reflect the different age and weight groups when possible so we can see how you make allowances. Thanks! :)

+beepine I agree with you, this does make it all look a little too easy. I was actually a little bummed that Summer was so flexible in hip extension, it made it hard to demonstrate the leg cradling that I usually do :) I'll do my best to bring some diversity to my models, but the main determinant is who's available. These are my fellow massage therapists and buddies from grad school, so I take what I can get! I'll certainly keep it in mind though. Thanks for the comment!

Hi Ian, great channel! I recently discovered that I have some kind of fibrosis condition whereby all muscles, tendons etc. appear to have fused together and wrapped around joints etc. with fatty/scar tissue and other lumpy stuff that is quite rigid and inflexible. It is hereditary whatever it is, but my case is first that has been bad enough to be detected (stunted growth, life-long musculo-skeletal pain etc.). I literally massaged out 3" of extra armspan so far ... the relief has been incredible and resolved long standing CTS immediately. I just thought I'd write to pose the question to you, given you are clearly very experienced and may be familiar with this kind of disorder; do you have any idea what it could be so I can help my medical network support me? Thanks, man!

Is there a reason to work in that direction? Sometimes I like to move on the opposite direction to try to also create space in the joint to offset some of that energy in the ligaments around the joint? Or is there a better reason that I should move up the leg rather than down when doing compression under the asis and tfl? Thanks for the video! I don't generally feel comfortable doing compressions do this gives me a really excellent way to use them! The hip flexors are not easy to release!

You know, compressions toward the ASIS and iliac crest are just things that I do as a matter of habit, but pressing distally to create a sense of decompression sounds really nice!

You compress very quickly compared to the pace I am learning in Thai massage. Are you going fast just for the sake of the video, or is it okay to pace the compressions faster?

+laialand I'm drawing much of my inspiration for this sequence from the various forms of sports massage that I've learned over the years. It's very kinetic, and it imparts some movement and rocking to the client's body. It also takes some of the pressure off of my body, as the elastic return from the client allows me to bounce up and prepare for the next compression. Thai massage (what I know of it from a single weekend workshop, and from receiving a number of sessions) tends to be much more deliberate. Less jostling, and greater emphasis on allowing the technique to sink in between compressions. So, to answer your question: This is the pace that I use, and I do try to jostle the body as I work. That said, it could easily be given a Thai aesthetic by applying each compression at a slower, measured rhythm. Great question!

+Tim Welch Yep, that's when you undrape the leg, draw the excess inferior sheet under the exposed leg, and now have the leg encircled by the sheet. This feels like a diaper, and it allows you to move the leg freely with little chance of exposure. I don't teach it because I find it uncomfortable, though it could certainly be useful for limb manipulations while the client is undressed. Hey Abigail, I consider it normal if a client feels soreness similar to what they'd have after a new workout of moderate intensity. That can last for about a day. If you've got straight-up pain or spasm, or if you feel like you've got fatigue or flu-like symptoms, that means they did too much. They did enough damage to your tissues to cause inflammation and provoke sensitivity, and that can take a few days to heal. I recommend not returning to such a massage therapist.
