Side-lying massage should be comfortable for both you and your client. By effectively using your body mechanics, and keeping a few strategies in mind, this can be an easy way for you to offer massage to clients who can't lay prone or supine. While this isn't a comprehensive lesson on draping, prenatal massage considerations, etc., it should offer a good technique refresher.

When communicating with clients about side-lying massage, I like to err on the side of over-explaining. Show them where their head goes, which pillow to hug, and what to do with their legs. Remind them of which side you'd like them to start on before you leave the room.
When working on the back and shoulders, keep the angle of your techniques in mind. Pressure angled up toward the head or down toward the sacrum can be most comfortable, creating plenty of tissue deformation without shoving them toward the abyss on the opposite side of the table. It can also prevent you from rolling your pregnant clients further onto their abdomen, which can be uncomfortable. As you work toward the shoulder, be aware of pressing the shoulder girdle into elevation. While this is fine for the short term, it can be uncomfortable if left that way. Draw it back to a neutral position while smoothing the trapezius out.
When working unilaterally, consider using finer tools in the upper back and shoulder regions in order to minimize pressure forward. You can still sink in, just keep their positioning in mind as you do so.
Finally, be mindful of your angle of approach when working with the limbs. It can be tempting to mostly work with the uppermost areas of the limb, which can end with too much emphasis on the adductors, abductors, and sensitive parts of the leg and upper arm. Change your angle so that you can distribute your work as you normally would. That said, definitely use the side-lying perspective to give you access to the adductors, IT band, and lateral pelvis.

Thank you, I'm in school and have yet to practice on a pregnant lady who was further along. I had one scheduled today and you video gave me the confidence to stay calm and just listen to my client and her muscles. She left very happy!

I have a client who cannot lay prone comfortably and has complicated low back issues. I will try this set up and advice the next time this client comes. In fact I shared this video with him, and he disclosed hid favorite sleeping position is side- lying. Thanks!

Love your attentive, slow myofascial way of massaging. Its quite similar to how I give. I find it helps to relax people more deeply physically and emotionally, is calming and allows people to breath fully. A meditatively nurturing quality which is at least as important as being specific about muscular & anatomical knowlege.

It's def not over explaining. I have client's I've worked on many times and when I repeat the normal instructions I have memorized to a T for them and how they should be on the table, they will suddenly be face up and I'm like "Oh, did you want to start face up?" "What?" "Oh, just, we always start face down." "OH!" *Fumble fumble fumble turn.* Lol. People get weirdly forgetful sometimes. I enjoyed the video though as I'm learning to dive into Prenatal Massage during a time where...there aren't many classes because...pandemic. However, I do have a woman who is in her first trimester and it seems she wants to work with me through her pregnancy (she is aware I am still learning), so I want to go in with some confidence and watching videos like these help with techniques.

I have been reintroducing myself to side laying work here recently, so this video is great! I do a great deal of shoulder work which this positioning is good for. It makes getting into the shoulder and some of the muscles that lay on the side of our bodies easier, especially on female clients because they are properly covered and basically holding the drape in place. You can also do some great neck stretching from side laying as well as some hip mobilization or call to motion, and getting at the piriformis for sciatic pain can be much easier. Thank you for this video, it's been a great insight, and I enjoy how you explain things so they are easily understood. Now if I could get the girls at work to watch....

Thanks for the visuals! It always helps me to see it in action. Queation: How do you turn client to the other side without it being super awkward? I always feel like I am fumbling around with pillows and body parts. I'd like to see it done more smoothly than I envision I look! lol

Haha, I can identify with you there, it can be a little tricky. I may have a video on that in the future, but for now, here's the gist: Before you have them move, ask them to give you the top part of the pillow, and then to raise their knee so you can take the bottom part. Make sure to keep the drape low and close to the client's skin as you do so to prevent a draft. Once you've got the pillow out of the picture, tent the drape slightly and have them roll onto their other side; they may roll in either direction (even if you ask them to do it in a certain way, so I just don't bother), so keep an eye on how close they get to the side of the table. Once they're on their other side, have them scoot back toward you like at the beginning of the video. Reposition the head pillow if it's at a funky angle, then grab the body pillow. Hand them the top part as you raise the edge of the drape slightly, letting them pull it in close. Raise the edge of the drape as you travel down the table (again, keeping it low), and have them raise their top knee as you insert the pillow. Most people will straighten their bottom leg at this point. If not, have them do that, and then roll toward the pillow again. This procedure is a little involved, but it minimizes the risk of falls and drafts, and it just take a minute once you get it down. Good question!

i second what someone else asked for. a client with adipose tissue around the middle in this position. can't do those nice flowing strokes as you did in this video. suggestions? i'm currently dealing with a client that can't lay prone and, no disrespect, has rolls on her side and back in this position. thanks for your videos. i've watched almost all of them and enjoy and learn from every one. your calm demeanor is inspiring.

I'll see what I can do! In the mean time, I suggest steamrolling that skin just like you would any other. It might move more, but that superficial fascia is still tugging on the muscle and fascia below. Depending on the distribution of the subcutaneous fat, you may need to change the shape of your hand tool to accommodate their different curvature as you glide along. It may also help to provide some fascial traction inferiorly as you work, especially with the lateral torso: With one hand or forearm, compress the lateral hip downward, toward the foot. With the other hand/forearm, do those slow myofascial strokes up the lateral torso. That will give a nice stretch, and it will keep the skin from folding up in front of your hand as you glide. Hi massage sloth. i love following your videos as i am a professional motocross Athlete i have substantiated many injurys over the years. and in the last year or so i have come to realise and utilize the importance of streching, massage and have recently started foam rolling but there is such mixed information about if you should foam role and if so how to do it properly. I would absolutely love to here your thoughts on foam rolling!! thank you

I think you're talking about the deep veins here (popliteal/posterior tibial, etc), and I can understand how that would be a concern, especially with the posterior knee being a potential endangerment site. Only work in ways that you're comfortable with, but I invite you to consider how well-ensconced and protected those deep veins are. They've got the nearby tibia as a stable backstop, they're completely wrapped by soleus, and they've got gastrocnemius with its thick muscle and thick connective tissue to displace force. Like superficial veins (of which there are many in the extremities, more visible in some than others), they will undergo deformation during massage, but much less so due to being so well covered. In people with healthy circulatory systems, veins are elastic and mobile, able to handle all sorts of compressive and shear forces from physical activity, sitting in weird ways, and massage. So, no pressure here, but I invite you to explore more compressive work with the low leg. This area can be the source of all sorts of client complaints, including charley horses, plantar fasciitis, and (in my opinion) all sorts of ankle and foot issues. Definitely use caution in clients where circulation is potentially compromised or clotting is likely, as in elderly clients, pregnant women, and post-COVID clients. If it's something that you'd like to experiment with, feel free to to reach out if you'd like to troubleshoot.

Great video (as usual!) but can you maybe create one showing HOW you drape both the lower and upper legs in side lying position? Particular I want to start providing some IT work on the upper leg with them" their side. Also showing transitions from side to side or side to prone / supine might be helpful...As a male I was quite nervous as an intern with draping (too many "horror" stories you know) ...I'm comfortable now of course but totally unfamiliar with really using side lying. I also want to start providing some rotator cuff, shoulder mobility work for my frozen shoulder clients.

Undraping that top leg is actually something that I have difficulty with as well. I've read that using large towels to give extra coverage and pin the drape in place can make it easier to expose the glute, but I tend to mostly work with the hip through the drape. Sorry that I can't be more help there, and I should definitely make videos about those side-to-side transitions and such!

Great tutorial, thank you, Ian! Do you have some more videos/advice on how to maintain proper body mechanics for pregnancy massage, especially during scissor stretches and mobilization of the RC region. While my clients are having a great time, I sometimes end up hurting at the end of two consecutive sessions (sympathy neck, lower back). Thank you!

I found that I can do a lot of the work on my clients' back while sitting down on my massage stool whilst behind them. It reduces the pressure, yes, but I don't find excessive pressure helpful in most pregnancy massages anyways.

Hey Ian, quick question. Do any of your videos have techniques that can help someone with lower back pain from a pinched nerve? I've heard that working with the muscles in the area can help a lot, but I'm not sure how to go about it or look for that information, or if that's even correct.

I forget to mention that too often 😭 I'm usually using some amount of jojoba oil. The speed that I glide is 100% determined by how much I use - I don't go any faster. I use enough that I get my desired amount of glide, I apply my weight, and I let the technique happen. If it ever looks like I'm barely moving, that means I either used a purposely tiny amount or none at all.

and multiple sclerosis...which I have and many of my clients have. It's difficult because all our symptoms are so different. Massage in areas of neuropathy caused by my MS is excruciating. For them, it offers relief.

I know this vid is a year old, but if you see this I was wondering if you could explain a little more on body mechanics. When I have tried using the side lying position on clients I end up with cramping/pain in my lower back and not sure if it's my table height or the way I'm applying the pressure, or something else.

My first thought is that you could probably raise your table or widen your stance (thus lowering yourself) while working. It can be easy to unconsciously hunch or bend over while trying to apply pressure toward a side-lying body. You could also try to find ways to rest your body against the table as you work, such as by keeping a thigh or hip in contact with the table. My last thought is that you may need to reposition your client if you find yourself reaching or bending; by having them scoot closer to the side of the table, you might be able to apply pressure with less discomfort. I definitely get nervous, but it helps to remember that I can do as many takes as I want. The raw footage for this video was about 45 minutes, mostly consisting of me saying "um," fiddling with lighting and camera placement, and trying to word things just right :) That said, my older videos were mostly done in a single take. It was easier for the information to flow when I was teaching every day in a school! Really, the only way to start out is to make a video. Get that first one published, and worry about all of the polish later. Ah, great question! Yes, I would have the client flip. Have them hand you the body pillow, keep the head pillow where it is, and then pin the sheet in place using your thighs. Use your hands to tent the drape slightly while having them roll, being ready to stop them if they seem to be wandering toward the far edge. Once they're settled, hand them back the body pillow: Have them grab the top of the pillow, then slip the bottom of the pillow under their top leg, making sure to keep the drape low. I can't believe I didn't cover this in the video 😵

Marlene P. I remove bolsters, except for head pillow, have client roll to get on all fours (if they’re able too, this works great for moms to be with big bellies) and then lay on the other side! Drape appropriately and rebolster. Way lah!

A client close to term will likely need to stay more upright. In cases like that (or any where the client is uncomfortable rolling forward), my angle of approach will need to start from a lower stance so that I'm not mashing them down toward the table. It may be useful to raise the table in such a situation, or to sink further into your stance, or both. As long as you focus on applying pressure at an oblique angle (up toward the head/down toward the sacrum instead of straight toward the thoracic region), this should be comfortable. I should mention that I emphasize comfort when talking to my prenatal clients. I ask them to "let me know if any position puts uncomfortable pressure on your abdomen, or if your side or hips ever start to get sore. We'll reposition you if so. This entire hour should be completely comfortable." I'll reemphasize this at least once during the massage. Great question!

My typical routine is to do some bilateral work on the back, then unilateral work along the spine, shoulder, arm, and hip. Then I'll hit up the legs (the lower leg can easily receive posterior and medial work - you could then work with the lateral and anterior regions on the upper leg, or wait until they're supine), and then I'll take their support bolsters and have them flip onto their other side, then replace the bolsters. Once they're there, I'll work bilaterally and unilaterally on the back once again, and repeat the arm and leg work. Then I'll finish them supine. That's my full body routine, but let's say you were just working the back and neck: in that case, you might choose not to flip them onto their other side. Sure, this will "leave them lopsided," but I don't think the body really cares too much. So! Flip if you're working comprehensively, but feel free to work the left and right side of the back while they're on one side if you're not so worried about hitting every area.

I do, and I can't believe I forgot to address that in the video. Oh, me. If you're wandering about the process: I'll ask for the knee pillow, then the hugging pillow, and set those aside. I'll position myself on the side they're about to roll toward, so I can provide a physical barrier to keep them from rolling off the table (not typically a problem, but just in case). Reposition the drape as they roll, making sure they don't pull it all to one side. Ask them to scoot their back toward the edge of the table. Replace pillows. Done!

I have a video for that! kzfaq.info/get/bejne/oL-DdqdyuMC4ZIk.html The basic gist is to slow down, to try different angles (a technique done from inferior to superior will feel much different from the same technique done in the opposite direction), to return to broad strokes frequently, and to take the opportunity to try new stuff (or old stuff from massage school that you've stopped using). If you can start seeing those long intervals as a time to slow down, connect, and experiment, you may get to the point where 80 minutes won't quite seem long enough!

That's a valid concern, especially when working bilaterally on a side-lying client. The positioning introduces some asymmetry for both parties. That said, I think that this is one of those "do your best and let their body sort it out" situations. Even if one side gets more attention or pressure than the other, I find that clients stand up feeling good, and no one ever walks out lopsided 🙂

Massage Sloth I was wondering whether it would be a good idea to have them lie on both sides... say 15 mins on each side, that way we could provide a more even massage. or is that too much movement for the client? TIA.

Hi Ashaki! This is something I should have been more clear about in my video: I do tend to have them flip onto their other side so that I can try to achieve some symmetry. The flipping process can be a little complicated the first time, so I should make a video on that soon!

I had to do one 2wks ago. Had not given one in over a year. My client actually talked me though the positioning......embarrassing. She was great and she was very relaxed after the massage, boy I was relieved. So thanks for this video. Now I can watch from time to time and be better prepared next time.

I have a question, do you just keep the woman on her left side the entire time or do you turn her over? Turning over from that position seems a tad complicated.

I definitely should have covered the turning procedure :) I do have them turn onto their other side, and it goes something like this: Ask them to lift their top knee so that you can start removing the body pillow. Pull the body pillow out to the side while controlling the drape and preventing it from lifting or shifting. Leave the head pillow in place. Brace your knees against the drape and grasp the opposite side of the drape and ask them to roll onto their other side. Keep the drape stable as they turn, and make sure the pillow is in a good place afterward. At this point, have them scoot again (bringing their back toward you) and give them back the body pillow, once again starting by having them lift their top knee, and keeping the drape low as you hand it back to them. Finish by having them roll toward the body pillow as in the video. I hope that helped! I will probably make a video on this in the future, but it may be a while :)

Can you do more videos with people with actual knots and adhesions. Most people I work with are not jell-o like the models you choose. they work out, they work, and they are tense and stressed. Looks like you choose from the local yoga class lol

Some people do prefer to leave some clothes on, and we're able to work with that! You might be asked to undo the back of your bra while receiving back work, but that is optional as well. Client comfort is a high priority.

Massage Sloth and how would that look like without leaving parts out or fumbling with bra straps etc? I do shiatsu which is clothed but those are totally different techniques. I do regularly use some of them during oil massage. I've never had a woman say that she wants to keep her bra on and always make it clear she will be covered by the drape but I am wondering.

@@trudyvalkyr3005 I'll ask them to undo their bra clasp rather than try to navigate it myself, then just leave it undone following work. When I flip them I'll tell them that they can feel free to reclasp it before they turn. As for dealing with straps during work, it might occasionally require pulling a shoulder strap aside, or even interrupting a technique to cross a cloth boundary. These are fairly easy adjustments to make, and if done deftly it can be difficult for the client to notice a discontinuity (for instance, switching hands rather than just stopping and then restarting). If a client leaves on even more, like a t-shirt, then it is time to change strategies. And yes, A while ago somebody told me that she finds it's very difficult to relax taking some of her clothes off so I and told her shiatsu would be better for her then.
