this is just MY GUESS but I think that doctor that save the old man HOLY is supposed to be another one of Tezuka's main character from another series: "BLACK JACK" an unlicensed doctor who dresses like a vampire.

haha yeah, wasnt paying attention to it when i read the manga, but holy shit. seeing the panel in anime just clicked. lol that Unlicensed Doctor description was a dead giveaway but i didnt notice it

Your guess is true. If you wacth sub you might be notice his VA is Akio Otsuka. Otsuka was Black Jack's VA. and There is name BJ in japanese credit as Akio Otsuka acting. Even without those fact, "Moguri no Isha" (unlicense doctor), astronomical fee and wearing all black are literally Black Jack.
