Since I started doing videos on Testosterone Replacement Therapy (TRT) with Dr. Osborn, I've had a lot of people ask me what my testosterone levels and dosage looks like - So, I had Dr Osborn pull up my original bloodwork when I first came to visit him when considering TRT and together we reviewed all my numbers as well as the subsequent visits where we made adjustments to dial things in. This video covers a lot of details on topics like SHBG (Sex Hormone Binding Globulin), Total Testosterone vs Free Testosterone, Testosterone Dosing, Estradiol/estrogenic effects like gyno, aromatase inhibitors, and the effects of stress (cortisol) on testosterone levels just to name a few.

I am very impressed for being 185lb the size you carry. I'm on trts well for medical reasons for hyperlipedimia. I've had this for over 20 years! In 1 session, it was back to normal. TRT has been a game changer for me all around. I've been given a second chance. I've stopped drinking, smoking, etc.. TRT set back the clock at least 15 years.

Sir what is connection between trt and hyperlipidemia? I am also suffering through it since 11 years and due to that i had multiole episodes of pancreatitis(i dont drink though)

I am in the same boat as James, although not near the lean muscle. I am a software developer and the last 2 years my Testosterone went from 605 to 450 and my ability to concentrate and motivation is in the toilet. I can sit at home on a Saturday and get little to nothing done. I am an active Crossfitter and I do enjoy working out and plenty strong and great body composition for a 56 yr old. I really want to have some giddy up as my first priority, Libido and muscle growth fat loss in that order. Thanks for being transparent, very helpful

Dr. Osborn is super. I’ve been a patient for 5 years . This video was a superb discussion and reminder why we pay attention to our blood work, energy etc. I like how he said that you must put in your 50%. It’s both sides really… the extra help w the T and the eating, sleeping and fitness.

Dude these 3 videos with dr osbourne have been exactly what i was looking for. Thank you and him very much. I just turned 30 and had my blood work recently done. i have a 357 Total T but other metrics are normal. Been feeling very groggy and unmotivated and i suspect thats why. I know i have a few things a in my life i have to optimize before even considering Trt but This is the knowledge i was searching for. my doctor said because said that because i fell in that 350-1000 range that im totally normal but i wasnt really buyin that since i was borderline. Love this James. Been following you and your wife for years. And your always building your health. A1c is at 7. And with other meds test dropped besides feeling tired I hadn’t noticed trt going low. Started trt therapy a few weeks ago. Thanks for putting this info out

Fantastic video James, I recently had my first blood test (to see what my test levels are like) and it was an eye opener, bottom range of normal and low free test but high end of normal SHGB. Definitely considering TRT and this video has been invaluable

So the one thing you’ll have to define for yourself is what results you expect. For me, because I have been training hard for so long (since ‘91) the TRT didn’t do a lot for physique (except when my levels briefly got too high). The results I was hoping for were centered around energy. I was really dragging at the time and that’s where the TRT made a huge difference.

Wow my bloodwork looks almost identical to your initial numbers. Especially the cortisol and glucose. This was super helpful in terms of figuring out if I should try TRT.

This was highly informative. I recommend that every male get their bloodwork done, including testosterone and estrogen. Knowing this info will make your lives much easier. I've been pulling panels for the past four years ... I am 60 today ... incredibly helpful to me. Great job, James!

You should have made sure you never ever even had to check your bloodwork. When you are stupid or a unmanly unnatural man you will end up needing to check your blood because the blood is degenerated already. Pumping test into your system because you already failed is not going to fix it and make you a simple junkie

I wish there were more doctors like Dr Osborn and easier for people to access them. I am 42 and energy levels suck most of the time. Its just expensive and insurance does nothing. I'm not trying to become jacked. Just want to feel energetic again without going broke

Important Side Note: One thing that didn't get covered in this video is that my first labs were done in Jan. of 2016. The strategies and philosophies have changed a bit from that time, especially as it pertains to using an anti-aromatase. If you listen through the video you'll hear us discuss it.

James, can you please talk more about your A1c? I'm in the same boat. I'm around your age. I work out regularly, I'm natural, and I'm lean. But I was surprised to see my A1c at 5.7 (getting pre diabetic)!! Has your A1c improved since you started with TRT?

I’m 43 and have been doing a lot of KZfaq research on this subject. Have felt pretty shitty for a few years now. Gained quite a bit of weight and no drive. Seeing that this Dr is in Palm Beach area is awesome because I’m close. Need to setup an appointment with him. Thank you for this

@@michaelclifford6468 - Every man needs it eventually if they want to feel the best and perform the best. Nobody beats Father Time no matter what they do, which is why these treatments exist. It gives people who have tried everything else an opportunity to regain some youth. When my libido drops and my energy levels drop due to age I’m heading straight to the clinic because I’ll be 48 this year and I know my time is short. Sylvester Stallone is 75 and still doing weighted pull-ups because he’s on the good stuff.

A man with a Testosterone level of over 500 given 200 mg injections of Testosterone a week so he can get his levels over 1500. He will have to phlebotomize because his HCT is going to be dangerously high. Do u agree with 200 mg of Test a week in this case.

@@pm2007est Yes, your Hct and Hgb count will go through the roof. Increased RBC count will increased risk of stroke and blood clots. You have to give blood at that point or risk self harm. I wouldn’t ever want my Test level to be higher than 900 for that reason. If body builders are doing it they probably have their labs being strictly monitored.

Thanks so much for sharing your lab work. This is very helpful. Seems like you could have just started slower on smaller , more frequent doses without the AI and slowly increased from there until you found your best levels / symptoms/ bloodwork

Thanks my friend. I appreciate it. You know the conversation about genetics is an interesting one. If I hadn't worked out hard and consistently for the past 30+ years and didn't always watch what I eat, what would I look like - and would anyone ever say that I had good genetics? It's hard to say. There's a chance that I may look slightly better than someone else with the same lifestyle habits but probably not much different. Genetics is often an easy scapegoat for lifelong lifestyle habits.

Great video. I do think you missed talking about how this is affecting your lipids, especially Apo-b and hdl. These inevitably can turn into bad values.

I month ago Started Nebio 1000 mg for every 12 weeks. I just had a blood test today, but need to wait for result a couple days. Sleep improved a lot, strength improved, libido is same...but testis are sore..hence blood test...😁 I really appreciate this info what you put out for us. 👍👍

Not since Rich Piana have I seen such a honest,to the point, break down of a hormone adjustment. You're much better than me Gage. If I shot T levels up, like yours did, i'd freak if I had to give blood to start over again. lol I'm a guy who doesn't do needles,vaccines, or even aspirin,well I took Tylenol all 4 times I had corona. That's it. 64 and probably negative T in my body. But I just keep fighting thru. i'm a candidate for sure. So a video like this, is certainly inevitable if if continue bow hunting with an 80 pound bow the next several years if I stay healthy. i'm gonna need a boost. 200 Mg was too much,so maybe like 25 mg,just a little boost. Might work for a guy like me who isn't into taking meds. BTW love your content,and think you're fairly local to me in Palm Beach-Dade ? When I started back up in the gym when corona went down, I did the 20 reps thing to get range of motion,and restrengthen tendons etc. Got the tendonitis and your videos on it helped me immensely,the Doctor from the Dolphins ? Warm the body part up first before stretching it ? Turned my life around !! No bullshit. Shoulders were killing me. I bow hunt and shoot a ton of archery. It was the BICEP the whole time, not the rotator or labrum. I warm up good, and stretch,do a set, stretch again, and i'm good. I know you don't like doing it, but i'd like to see how you handle and workout using heavier weight maxing @ 12 reps or so ? Seems many guys like John Means and even Jay Cutler, feel 12 is a good way to work the muscle ? 20-16-12 is sorta what i've started doing. When I was your age, I maxxed @ 8 reps,,,,,lol 8 reps was lite weight.

James - and anyone that reads this - I’m 49 now (2022/March) and I have been feeling like chit since I was 38. For the next 5-7 years I tried to find out what was wrong with me and all my doctors that I visited said I was good. I didn’t have drive and could sleep for hours only to wake up to get back to sleep. After 45 years old I ran into issues like my appendix then COVID and just now - after all this time I get some answers to my questions thanks to you James. My man , I don’t think I will go on TRT but will be trying to maximize on what I have for this year but I will do a follow- up post in the future. 🙏🙏👌💪

I feel exactly the same way James is describing how he is feeling in this video. The difference is that I live in Canada where the sun doesn't shine that much over the winter time and James lives in Florida where the weather and sun are suppose to benefit your health even during the winter months.

I was the same as far as my level is for being prediabetic. I just started my TRT protocol and hopefully within my next bloodwork I’ll be healthy again.

Awesome video. Great information. I'm a natural bodybuilder, with a high haemoglobin and haematocrit, is donating blood something I should consider. Where I live, doctors arnt very clued up on this kind of stuff. Cheers

When the doctor mentioned it’s a CEO profile, that really caught my attention. I had a same result profile and I was running a company too. He explains it so well exactly what makes it a CEO profile.

Excellent video. This doctor definitely cares about his patients. I like many others enjoyed and reaped the benefits of TRT when I first started. That was short lived. Now I feel absolutely no benefits and I feel just as bad as I did before starting TRT. I've tried every protocol out there. Nothing has worked. Libido dead. Energy dead. Drive dead. Horrible. Tried countless endos and doctors. No one can fix it. Enjoy the honeymoon phase my friends, because after that, its gone forever.

@@LuisGonzalez-ip8jp thank you. I have two children and we aren’t looking to have anymore. HCG has been shown to work for conceiving. So you always have that option. I just switched to one dose a week IM. Second week of this. I feel a little better but still no where near where I was originally. I’m not giving up.

Wish there was a clinic here in southeast michigan that was as knowledgeable as Dr.Osborn, I just had my blood work done and my test levels is now 199 and I just turned 50. Dr has referred me to a urologist, but I worry they will try to tell me its normal and I have to accept the depression like state im in.

FYI - the ranges between labs don’t matter as much as some people think. The lab sets their “normal” ranges so that the result can be Interpreted correctly. So as long as you’re interpreting the lab result based on the normal range reported from that same company at the same time you got the test done then you’re fine. In other words, the absolute numbers don’t matter as much as the result’s relation to the range. For context I’m a clinical pharmacist and look at/interpret labs all day, every day so I know more than the average bear

7:35 mind you the average in 1984-85 in men ages 22 to 32 was 1200. That range is something newer that the medical community adopted within the last 15 years or so. I take 170 mg every Wednesday and my resting T is 780ish. My medical team started at 50 mg and increased the dosing slowly on a monthly basis.

I really love your honesty about it and especially about the libido part because it's a very important one , so now you only take cypionate , nothing else , but how much growth hormones are you taking with it

Appreciate it my friend. Yes, cypionate is the only thing I take. I don't take growth hormone but I'm sure there are plenty of doctors that would give the thumbs up if it was done the right way (to optimize healthy normal levels).

@@JamesGrage - I had a consultation with Evolve and they recommended HCG to keep the nuts hanging and CJC 1295 to stimulate HGH secretion. I hadn’t had any blood work done, but that was the basic protocol.

I started about 3 months ago 160 a week all I really want to do is lean out add a bit of muscle but feel good and super healthy... I love to be active and want to have energy...

Wow I'm 40 and I gave been feeling the way you describe and it sux I have all the symptoms as low t last time I had mine checked was over 10,yrs ago and it was in the 200s Dr wouldn't do shit they wanted to blame it on everything else so now my weight has went up no energy it horrible I have been looking at trt and trying to find the right Dr thanks for the video and some knowledge about trt

James, it would be useful to learn the cost of your regimen for those on the lower end of the economic scale. I was on TRT for 13 years and felt amazing, unlike today with all the classic low T symptoms. I am super health conscious, but mental health is important as well. If we could get an idea of the ongoing cost with a competent knowledgable physician it would be very helpful. For some it might mean just tightening our belt in order to get a new lease on life.

I pay $40 for 10 weeks of testosterone and $12 for 10 weeks of anastrazole. Biannual doctor visits and blood work costs around $250 per year. Comes out to around $75 per month all in for the year.

On .5ml 100mg test cyp I’m at 1160total and 58 on free test. I metabolize it slowly. Get blood draw every 60 days to keep hemoglobin good level.

Great video, very helpful information. Unfortunately i live in Czech Republic and it’s absolutely tabu to get TRT from our doctors. They say that it is normal to have a feeling to have sex once per 14 days :-( So then they don’t offer any possibilities how to improve that :-/

Pretty darn informative. I’m doing TRT I’m on quite a bit older. Obviously, you’re super cut up because I’m the same height as you but you look much bigger. I’m in great shape, but I don’t look as big as you do. I’m at 205 and I feel I could even get a bit bigger. So my question is, do I need to lose more body fat? I would like to get more cut, but it would take more sacrificing. I would have to really monitor my food intake.

Thank you for the very informative video. TRT has it's merits, but I'm wondering why the good doctor did not try to stop your body from making so much cortisol so your body could product more testosterone naturally?

our bloods look very similar , I had issues with my H/H , Learned to do things smarter.. We have exact MCV MCHC , RDW Platelets, ect... One thing i was curious on was your liver enzymes were a little high i believe? I may have got confused or got distracted. What was the reason for that to go up? And just to share my last blood test, i tried to lower my hematocrit and red bloods by taking a month of TRT.. Well I dont advise ever just stopping.. I felt great for the 1st 3 wks but by week 4, very tired, pale got sick with fever and just in a catabolic state.. My total test was 35 free was 0.1 and i thoght i ws reading the test wrong.. It def lowered my bloods into the 16 ng/dl for hemaglobin. but i did that on my own and when i went to the doc and we went over the bloods he was happy to see them lower.. and even said maybe stay off test and let the body come back.. Problem is , im 45 and the reason i went to him in the first place is cus my testosterone levels were low in the low 300s and Nope i wasnt messing with Gear prior. Its not an Looks thing for me either,its feeling great and being optimal im looking for.. So anyway im writing and doing 5 other things at once , so some of this may come out scattered, but this was a great video, thank you for sharing your blood work..

Dr. dropped me down to .3ml 80mg from .5ml 100mg. I started crying for no reason after 2wks. I switched Dr. new Dr. is on my side of getting blood draws to keep hemoglobin down and stay at higher numbers to feel happy.

While on trt my gyno went crazy, my estradiol was 60. I used to to cycle tamoxifen to manage the gyno, but I saw one comment one day of a doctor that said to take 0.5 milligrams of anaztrasol per week. That’s a super low dose and I started taking 1 mg per week. Never had issue with gyno anymore. I’ll probably lower to 0.5 mg

Lol when he said he started him at 200mg right away, I knew it would be too high from watching MPMD. I even said should have started at 125mg and then later on in the video that's what he ended up at.

Im all natty test levels in mid 500s a bit lighter in weight than i want at 6ft 1 but i feel fine plemty of strength very lean mid 40s but my lactic acid starts up quicker than i can notice which i hate at times i tend to fatigue quicker if i dont pace is all why u want over 200lbs anyway? USA diet takes care of that all sugars, preservatives all crap i cut out u dont need it more veggies also a good change

When starting TRT, how long does it take for your body to completely shut down its own factory? I started two weeks ago at 100 mg per week and my numbers were over 800 test. Does that mean that my body is still making its own test in combination with the exogenous test I am injecting weekly? Also, I am doing this without HCG right now, just Test. Also, what are your feelings about taking TRT with Fadogia? I have heard that Fadogia Agrestis could help in preventing testicular atrophy.

I lack motivation and feel low and demotivated, but my go-to has never been trying to get on TRT. I know that like a lot of people I have good phases, indifferent phases and bad. It's the ups and downs of being 61. What I find irritating is how so many people who talk up the benefits of TRT look significantly more muscular than most other people who workout hard. It's a symptom of a world where we can have something, then therefore think we're entitled to it. I'm always striving to improve my physique and fitness, and on some days it's hard and sore. I'm not going to rush out and get TRT, however, because hard gym work is difficult and often tough on the body. It's a price I'm willing to pay, and I don't want any injections which will make my gains feel fake.

This is what I need. I had a doctor that gave test cyp but was scared to give me more than 50mg/wk. I would have had much better results on a higher dose and all these other labs tested.

50 may in fact lower your test. Switch doctors.

His HGB was actually only 17, which is high normal. His Hematocrit was 51. That’s a bit high. I do agree with being careful with what we put in our bodies because it affects hormones. However they get out of wack on their own as we age. So doing nothing to address that may not be the best option. Some people are afraid of TRT, but not afraid to go through life on low testosterone.

Thanks James, very intesting. One question though: in your first 2 results (at 5:30 and 8:00), the Free T reference range is 6.8 to 21.5 pg/ml, while in the third result (at 12:40), it is 35 to 155, still pg/ml. What is this incoherence ? As a side note, if the free T is to be 2% of total T, only the last range can be correct. I saw in another comment from a pharmacist that the difference in ranges doesn't really matter, and we should just look at where we position relative to whatever range our lab has established. But still, this is disturbing to get a result which means our free T is 0.2% of total T...

Maybe it’s different in the states, maybe it’s different depending on the clinic/doctor…but up here in Canada I was told the top high end of bound test is 700 max…the clinic I deal with capped my dosage when I was around 550 test…so weird…

What my first test was 87then second was 11 zero estrogen blood work was perfect I can't even or want to do anything tried several doctors for replacement 75 a week then they recalled what I was on kinda lost faith in the doctors I run on pure will power I've bin a household mover for 35 years now I can't do anything list almost all muscle mass tired all the time can't believe how old I've gotten so fast and weak depressed I use to be 24/7 and worked hard all my live and enjoyed moving heavy things all day 54yrs old 6ft6 300lbs of fluff now so heart broken when I look in the mirror
