I'm not a christian myself but over the last few months Ive gained a whole lot of respect for people of that faith (at least with the affirming churches and congregations), The counter arguments I've seen in response to the bigots shows a level of theological and spiritual knowledge that puts most clergy across many faiths to shame. I'm really glad to see the Calvinist and Augustinian brands of aberhamic faith falling out of style. Legitimately brought to tears by some of the testimonies, but in a good way.

I wanted to give a quick bit of info to the person asking about stress and weight gain. Cortisol (stress hormone produced by your adrenal glands) stimulates your fat and carbohydrate metabolism, creating a surge of energy in your body. While this process is essential for survival situations, it also increases your appetite. Additionally, elevated cortisol levels can cause cravings for sweet, fatty and salty foods. My recommendation for stress reduction would be some form of meditation to reduce it psychologically and exercise to reduce it physically. Stepping away from triggers, if possible, would also hugely help the reduction of stress. Subjective stuff: Stress, as far as I understand it, is caused by a situation or thought process that you feel unable to escape from. Sometimes the solution escapes you because your brain isn't operating well enough. I know from being homeless for 2.5 years that the #1 problem I faced (aside from the obvious) is the lack of quality sleep. You always have to be on guard. This can lead to brain fog and bad choices. So the "8 hours of sleep" phrase is incomplete. It should be "However much restful sleep your body needs to feel recharged. If you have to have caffeine or an alarm to wake up, you're sleep deprived and will make poor choices." So go say hi to your pillow. Your pillow is your friend 🙂 Hope this helps

I resonate with the struggle between wanting good health but also the effects of traditional workouts. On the one hand, you want to feel good and look good. On the other hand, what "looks good" based on our gender assigned at birth may not be aligned with how we want to look. The majority of workout support out there, however, tends to be exceedingly binarily separated. And there is the natural worry about how muscle will react on a testosterone profile while HRT is doing its thing. This is where I will sometimes lean into what I refer to as "movement" rather than "exercise" or "working out." Developing healthy movement patterns such as crawling, lifting objects, and deep squatting can help create a foundationally healthy body but without the classic "get a big chest with 5x5 bench press" and stuff like that. Things like yoga, primal movement, and animal flows are a fun alternative to the classic workout that a person who is transitioning toward more feminine expression might find helpful in maintaining or developing good health while minimizing the worry about muscle bulk. Please note: I am not a physician, physical therapist, or personal trainer. This is just insight based on my own experiences as a nonbinary person who enjoys working out but does not want to pack on a lot of muscle while navigating the very binary traditional fitness info out there.

Wonderful questions and your responses, Dr. Z.! I have had crippling inflammation for 15 years resulting in years of lost productivity, and really walloping my self esteem and emotional and mental buoyancy., not to mention finances. Diet and exercise have certainly helped me, in between onsets, but when discovering my dysphoria and confronting that and finally accepting that has basically cured that and now I can exercise regularly but with more joy. I credit my confronting my true condition and inner self is largely responsible for my improved condition ( I have always been following my primary care doctors' care, as well, but medications alone have not made a major improvement.): I have suffered from PTSD as well as depression and anxiety. All of these are greatly diminished and after 4 mo. of HRT, I had my first normal blood pressure reading in 5 + years. I am inclined to see the body/mind /spirit inner connection as very relevant and interconnected. Just not having to shore up and maintain the learned and fearful mountain of role behaviors has really cleared the decks so to speak inside. the spaciousness remaining is a salve and release in itself. A lot of states let Medicaid or Medicare pay for trans hormones and surgeries and trans affirming therapy now. This is the only way I’m able to afford them. Those who aren’t working or work part time or are self-employed but don’t file taxes (tell them you’re unemployed) are eligible for Medicaid (free insurance). Not enough people know this. The only way I found out about this was from a trans man I met. I’m a trans woman. Please speak out about this so more people will know and it will save lives.

I find it interesting that wide shoulders are a source of dysphoria for many, because for me it is the opposite now. I always felt that I had unusual narrow shoulders for a man, and it was actually a source for insecurities when growing up and still being in denial and trying to fit into the male physical stereotype. Since my teenage years and throughout adulthood it has been my perception that my shoulders were narrower and hips wider than my male friends and it made feel very insecure. Also, I have always struggled with low muscle tone and no matter how much time I spent working out in the gym, I couldn't build up large muscles and I always struggled with passing physical strength tests for adult males. Since puberty, I also had spine problem with scoliosis and kyphosis which caused me low self-esteem and feeling like the hunchback of Notre Dame. Overall, I felt like a Frankenstein monster put together with body parts that didn't really fit together. Since accepting myself and starting HRT, my old insecurities about many body parts has vanished. My unusually narrow shoulders for being a guy suddenly feels perfect for a woman. My unusually wide hips for a guy suddenly feels acceptable for a woman. My low natural muscle-tone that was a curse for so long suddenly has become a benefit. Two months into HRT I had an anterior pelvic tilt changing my posture and gait; and suddenly my spine seems perfectly fine. I can no longer detect the scoliosis and kyphosis that had been a chronic problem since puberty. How common is it for transgender people to have body parts that doesn't really seems to fit well together?

It depends on starting age and genes mostly. I'm a bit of an outlier in several ways. I have unusually large ribs and shoulders while having an abnormally small pelvis for a man. I'm only 5'7 so a bit short for a man but my shoulders and ribcage are larger than most men I meet. Be glad. There are a lot of trans people that are in the androgynous range or closer to the dimorphism of their gender and sex but there are a lot of trans people that have strong dimorphic features that do not align with their gender. My body doesn't even match itself let alone my gender.

It's not dysphoric to me so much as I can't find tops that fit. OF they fit the shoulders and bust then they are super boxy and baggy. Same with dresses, if they fit up top (XL or XXL even) they don't fit my medium bottom half.

@@Valerie_Valkyrie315 I started tying my tops of at the waist or ubderbust depending on what look I am going for. Waist belts are also great with a lot of styles too.

@@Maelstrome123 I'm 6'2, but fortunately my height isn't a source of dysphoria for me as I come from a tall family. My cis gender sister is the same height as me and have the same shoe size. My nephews are 6'8-6'9. That helps tremendously dealing with the fact of being above average height for a woman.

+DRZPHD *Concerning Question 3 (Time **13:16**), suggest that Client exercise the legs and abdomen to build the lower muscles.* Hopefully reduced dysphoría that way.

Hi Dr Z. I have done a lot of research on multiple health issues I have learned that many Drs on KZfaq have expressed that there is a gut brain connection. That unhealthy gut can cause an unhealthy brain. So I wonder if those with dysphoria improved their gut health if that would help with their dysphoria? I know it is a newer topic in the medical field. It has 15:58 been discussed for the past 1 1/2-2 years. You have answered a few of my questions in the past and I thank you for helping me with those questions. Take care and I look forward to hearing more content.

@@DRZPHD they say that the small intestine creates a hormone that helps elevate serotonin and other brain chemicals. If I’m not mistaken the GI tract makes the hormones that actually effect the 5 main brain chemicals. Overall creating a euphoria in your brain by balancing the neurotransmitters. I will try to find the video and let you know who has done research on this. I’ve seen it in several neuroscience phd people along with psychologist and medical drs. Didn’t know if that would help with dysphoria in trans individuals.

@@DRZPHD on KZfaq you can type in brain cut connections. The gut makes more seratonin and dopamine then your brain does. Some of what is talked about is above my understanding. Neuro and psych are my weak points as a nurse but I get by each day with what I do know. There is a Dr snipes who does a lot of research and Jordan Peterson. Schools like Stafford Harvard and a couple others on east coast research this topic. From what I’ve learned it’s been talked about for couple years now. I just learned it in past couple months

This was told me by my doctor in relation with different medication, but would be relevant to hormones as well: medication requires a specific concentration in the tissue to do its job. If the body is larger (which a high BMI causes, obviously) a higher dosage is needed to attain and maintain that concentration in the tissue. Thus a higher dosage will need to be prescribed. In practical terms, as an example, I have less benefit from 1000mg or paracetamol painkillers than my sister, who is a lot smaller than me, gets from 500mg.

Wait omg this totally just made me realize, one of the reasons I'd always wear a hoodie in school starting around puberty was I wanted to wear tight and girly jeans but I would always get randomly excited for no reason, and I was always weirdly uncomfortable with myself getting excited like that since it started. Like I did not like it at all. Always just wanted it to go away. Even knew what it was and what it was for and I've always been very shy and reluctant to use it for that and really only have because of others asking me to. Another sign I've been thinking about for a while, I was also super shy about the idea of changing in the locker rooms for gym class with other biological males. I didn't want them to see me changing at all and I know for some that's just a privacy thing but for me it was more of the worry of an uncomfortable feeling of anyone bringing any attention to my.. umm well I'm trying to be KZfaq comment safe here.. I think you get it 😅

Thank you i worry a lot about having a muscly body before and trying to kill and let the muscles die. I think i do too much running and exercise. I run 4 times a week and cycle and walk everywhere. Ive been on hormones a year and its working but i hate my arms and upper body. I guess its a slow process. I was 35 when i started and wished i started earlier. I feel really sad seeing younger trans people. But on the other hand at least ive started. Tiff

I wish I could teach a class or talk to individuals who are struggling like I did. I know it’s hard to have patience. But we truly have to. I’ve been HRT for 10 years. I’ve had two FFS surgeries, breast augmentation and in May of 2024 I’m having SRS. I’ve known since I was 4 yrs old. I was a U.S. Marine for six years and a Deputy Sheriff for 20yrs. I got to the point where I could bench press 400lbs! I’m 5’7 and am around 180lbs. I rarely get miss gendered now if ever. I’m doing karaoke with friends every weekend. I was asked last weekend if I was going through menopause because I always have a fan at our table. 😁 So, hang in there, be patient (I know it’s hard) but it WILL come. About my 2nd year of HRT is when I saw results. I became a B cup breast size and my skin became very soft. And for information, I’m 57 try’s old. Something they don’t tell you about HRT is you will look younger. Pretty sure it’s the skin, melatonin etc. one thing I will suggest ladies, start having laser hair removal NOW. Mustache,beard and neck. Good luck and LOVE YOURSELF! You CAN do this!!

Hi Dr.Z, thanks as always for your wonderful work. I have a question which I don't know if you have discussed in some of your videos, but, have you ever seen for yourself the consequences of not dealing with gender dysphoria or not transitioning when someone actually accepts they're trans? Maybe people come to you to get support for long term coping or maybe there is some study regarding this topic, I don't know. The web is flooded with testimonials from people saying "do it now or your life will be hell and you'll regret later on" but, out of logic, transition is a modern phenomenon as a solution for gender non conformity and you have yourself said that choosing not to transition is a valid option, and apparently there are folks out there who manage to high-function in that setting, so I wonder what are usually the outcomes of such decission. Thanks in advance!

I am in the same situation with 12% body fat and 173 lbs before HRT. I wanted to lose 25 lbs of that but I also gained 30lbs due to HRT. so Now I have over 40 lbs to lose of fat + muscle.

Weight is a concern for sure. I'm 5'6" and 155 which as a male who works out isn't bad but as a trans female who's testosterone is nearly gone I'm scared to lose the muscle and gain body fat in its place because it won't come back easy if at all. It's so important to keep up with diet and exercise. As far as shoulders go it makes clothes shopping hard because dresses that fit up top are like a mumu everywhere else and tops have similar problems.

Ugh the fat distribution... I'm very heavy but healthy and like my size. (5'5 360) Figure I'd lose like 20lbs and then put it back on and it should be where I want it as a result of being post op for bottom surgery. For libido I find horny to be fun as opposed to being frustrating on fem hormones (especially the progesterone) my libido went from like an 8/10 to a 14/10. I only get erections when I'm actually excited as opposed to "whoops boner" and I can put the thoughts away very easily or just sit with the feeling or even do something positively about it.

Hello Dr. Z, I'm a trans woman working in a mainly female work place and I struggle with dressing and socially being there because I think it makes them uncomfortable and my state has the right to fire for any reason. So my question is, do you have any suggestions navigating the environment. Should I just dress in stealth or just commit my identity and risk losing my income anything would help with this.

If God did indeed create all of us in his own image, then the way he created all of us and the way we are, apparently since trans Are Born This Way, one could interpret that that would be the exact way he intended in the first place, I think the discrepancy is when human beings try to interpret the will of God

Please don't think I'm phobic I'm a failed trans person considering unaliving in real life inside of Minecraft if you understand what I mean. I never seen the gross amount of trash some older trans people watched, generally because I'm well adjusted. Literally clicked a random persons yt profile in the comments they said they were trans and they had favourited a sissy amsr video. It reminds me of what some adults do when they do "age regression" and infantalism, they have a gross and fetishitic view on women. Or a perverted form of the idea of what a woman is. Not to say trans women aren't women, but a few bad apples that fall from the tree seem to spoil all the fruit. I wanted to be a woman because of a deep seated pain, hatred for being homosexual, many sisters, being punished for exuding effeminate behavior in childhood, beaten for effeminate behavior, failure to connect to any strong male role model to reinforce a masculine identity, watching my mother been beaten. I tried to push. I tried so hard and its all dust in the air now, its choking me. I'm suffocating. My body is distorted, I don't have a body, or a voice, or a face, I have nothing but fear and distortion. I removed my own gonads for Petesake and that only made me a eunuch. I'm loosing hair to autoimmune alopecia, I'm in my early to mid 20's. The nhs will have their hands stained with my blood. Its over, I'm tired. I tried. Failed. I'm a broken man. Good night, good morning and good bye.
