The joke is that a starfish's anus is on top, as in real life they typically only lie in the sand one way (mouth on the bottom in the sand, anus on the top). I never got this joke until I took a university zoology course.

If you watch the rest of the scene, Patrick says that, for a moment, he forgot he was laying on his back. I just thought he was laying in some weird twisted way. Seeing as Stephen Hillenburg was an oceanography teacher before going to Spongebob, this makes a lot more sense.

@@DMTInfinity It can be seen as a dirty joke but its mainly because a starfish's anus is on top and they only lie down one way as Patrick is showing here (anus on top and mouth on the bottom so when they are hungry, they can move up, eject their stomach and eat their food) It makes sense really given how Stephen Hillenburg was a marine biologist.
