Hello Mr.Cooper,, i was discussing it yesterday with a pharmacist after discussing it with my gastroenterologist that i take PPIs 40Mg like 3-4 times a week and with the correct diet it seems i 'm fine. The pharmacist though told me that i should not take PPIs like that 3-4 times a week because they do not work how they should. I should take them like for a few months and the stop for couple of weeks. My gastroenterologist told me it was fine to take them 3-4 times a week to avoid any potential side effect in the LT. However, the pharmacist told me is completely wrong taking PPIs that way. Any idea which one is the correct way of taking PPIs? Also, told me that taking gaviscon double action daily is going to destroy my stomach lining in the long term is that true too?

Either way is reasonable. I would just say it’s wrong to say one way is wrong. If your symptoms are improved that’s the goal.

Are these “symptoms” very suspicious of this? very gradually get a lump in the throat feeling with a bit of tightness there come on and linger for a couple hours after I eat something then usually stops, but then will come back on without anything some other time, doesn’t affect my breathing at all though, sometimes the tightness discomfort it makes it harder to sleep though when it happening at night

Basically living on ice cream and milk for 3 years 90% of the time, which is the worse for my stomach and I lay on my stomach to sleep but not much gerd symptoms.. I have had 3 colonoscopy’s in the last 3 months. First one was my first ever. They found 2 small polyps and a “flat polyp 20mm wide”! Since my first colonoscopy was in the “dr.office” building they would not remove the flat polyp there. They wanted me in the hospital in case anything went wrong. Guess the flat polyp removal isn’t as worry free as a regular polyp removal. So i went to hospital for flat polyp removal. I had to drink the gallon of golyte half at 5pm the night before and the other half was to be finished “AT LEAST” 3 hours prior to my colonoscopy. I’m highlighting the “at least” because I finished my 2nd half at exactly 3 hours before my appointment time. I aspirated on the table….apparently not all the golyte had gotten through my system and I vomited on the table while asleep. They had to cram something down my throat to open my airways. I guess my breathing came back ok …but whatever they crammed down my throat made me cough continuously. They had to call my flat polyp removal off and reschedule. If you can imagine the pure hell of 3 colonoscopy’s in 3 months!!! Today I did things a little different. I drank 16 ounces extra of that mess in the first dose the night before….leaving me only 48 ounces for the second half. I also left 5.5 hours of time before my last drink of prep or any other fluids so I was sure to have no fluids left to vomit. It worked….today I got my flat polyp removed and I cannot tell you the relief. Want to express the importance of finishing your prep…..the flat polyp may not have been discovered had I not been so cleaned out. It takes a trained eye to find it….but even a trained eye can’t see through poop! And a big FYI…..the prep is 100 times worse than the actual colonoscopy. The colonoscopy was a great nap and I truly worried for nothing! It’s the prep that’s so dreadful! P.s…my first 2 colonoscopies were at 7:30 in the morning. So if you can imagine getting up at like 2am to drink my second half of prep…..be sure to imagine it miserable…because it was. I think a lot of people have this “get it out of the way early” thought process. After 3 preps I can testify a big fat “NO” to that. Afternoon is so much more relaxed. My 3rd colonoscopy was at 2:15 pm. I did my night before at 5pm like usual…..but I was able to go to sleep….sleep a full night of sleep….started my second half at 7:15 am to 9:15 am. Left me several hours to finish cleaning out, take a shower, get dressed, and had no stress time wise. Will do afternoon appointment next time as well.

So glad to finally have a diagnosis. So many times I’ve stopped eating and just stared off hoping for the pain and sensation to end. Can’t speak during these episodes, so the best I can do is give a thumbs up to reassure those around me I’m alright. The pain is horrible, can’t even swallow saliva, and the choking sensation is terrifying. Diaphragm spasms, neck aches. What sucks is I’m an athlete, so I need those high proteins and sometimes run into a trigger food! Finally said enough when I had an episode so bad I actually stopped breathing. It was nothing more than chicken broth, and just a spoonful…. Thankfully I had an endoscopy and I believe my eosinophils were around… 26? I think? High. My esophagus was constantly inflammed, to the point where a swallow study pill couldn’t pass by without pain. Anyway I was prescribed a PPI and that has helped so so much! Life is back to normal - in fact, it’s better than what used to be normal ☺️

(As well as making a list of what allergens I MUST avoid and which ones I should eat in moderation. Elimination diet kinda works for me, and allergen tests didn’t help due to, and I quote my immunologist “The tissues in the skin respond to allergens differently than what may trigger the tissues in your throat”. Really is a Trial and Error process that takes patience and kindness with yourself. I’ve been there folks, we’re in this together - you’re not alone ❤)

Can we just take B12, magnisium, Vitamin D and calcium along with PPIs to avoid these risks? or is not working like that?
@MarkCooperMD
@MarkCooperMD 8 күн бұрын
Vitamin D is fat soluble so acidity shouldn’t matter at all for its absorption. The others are theoretical risks but little evidence that most people should need vitamins. A good diet should have more than enough nutrients. Most of your GI tract is neutral anyhow so acidity is not essential for a lot of absorption. B12 and magnesium are easy enough to supplement however if you take a PPI for a very long time.
@mari051986
@mari051986 7 күн бұрын
@@MarkCooperMD tbh you are one of the few in here saying that PPIs long term for men and women are not that bad...with very low chances to face any issue on the long term! Many thanks for that, really appreciate it. I live in EU and my gastroenterologist suggested to take them long term with good diet because when i was born i had Esophageal atresia and some other issues that were fixed finger crossed. Now at the age of 40 i found out i have relatively small hiatal hernia at 1.8cm, lose sphincter, gastritis and GERD. I take seriously my health so i'm very careful of what i'm eating from the day i found out (9 months ago) but inflammation is still there. I take PPIs 4 times a week twice 40mg and twice 20mg pariet. I had two endoscopies within 9 months both times i have inflammation but i think is reduced now due to my diet and PPIs. Only symptom is LPR but once in a month i might get the pain under my chest. However, inflammation is still there.. In general i was worried about the long term side effects of PPIs so thank you for your explanation and giving me hopes that i can use them for the next 20-30 years

Had an attack of Diverticulitis out of the blue, was sent home with antibiotics, a follow up CT scan 1 month later showed an abscess that they couldn't drain, bowel perforation, large local lymph nodes and fat stranding. The doctor said to let nature take its course and wanted to wait for swelling to go down before I had a colonoscopy. Never had a normal bowel motion since, have passed blood, blood clots, mucus and mucosy poop. Now am feeling better other than constant soft stool sometimes with mucus or blood and another month on another CT now shows I have adhesions to the bladder and womb, an ovarian abcess and a bowel to bladder fistula, have a sigmoidoscopy tomorrow as they suspect a tumor, am scared, doctor keeps telling me im young to have this gone from mild to very complicated.
