HOLY MOLY BATMAN!! My condolences to those who started watching this with earphones! In all seriousness. When I got my knee and shoulder shot up in Afghanistan, I had the luxury of having a Fentanyl lollipop in my IFAK (Individual First Aid Kit) and I was VERY happy to have it. It didn’t remove all my pain, but it did help me keep my cool (and sanity) while my buddies dragged me to safety while I had my sidearm drawn… No chance of that happening, if I didn’t have access to the fentanyl lollipop. (Something really cool about those lollipops, is that once you take them out of your mouth, enough of the fentanyl is out of your body, that surgeons (anesthesiologists) don’t have to take it into account when they have to put you under before an operation! Before synthetic fentanyl was available, morphine IV was sometimes the only alternative. Which was a problem, because the body takes a long time to “burn off” the morphine. (If I remember correctly, is it took up to four hours for the body to burn it off.)

Hi, I hope everybody is doing well. I'm a trauma RN....my family is full of medical persons. Sadly...my brother overdosed on fentanyl....he applied a 100 mcg patch on one shoulder, and applied another 100 mcg on the other shoulder. He was chasing his high, and dropped dead in the kitchen making breakfast. Fentanyl is extremely potent and dangerous. I wish a wonderful weekend for you. God bless, Tracy

I just found your channel so I'm watching this months after the fact. I do like hearing a doctor talk about the difference between legal & necessary use and abusive uses of Fentanyl. The ways it is abused is scary and obviously deadly. It also has caused such a fear that makes it impossible for many to see how critical it can be for some with chronic pain to their daily functioning. Education on the ways it is used in proper forms and doses it more important every day. @Medical Secrets The way you call your patients before surgery is amazing! I have several doctors and the trust I have in them is so important, especially the surgeons (I am disabled so have more than most people). After all the surgeries I've had I don't have any pre-op apprehension but don't know if that would be true without that trust. I have a running joke with my orthopedic surgeon's PA and always say as soon as I hear his voice I tell the anesthesiologist they can knock me out.

I am not a medical professional and never knew I would be interested in the topic of mind body in relation to anesthesia and surgery. Unfortunately I’ve had a horrible experience with anesthesia. The first time was when I was rolled into the operating room and prepared for a C-section. There was an emergency C-section and the delivery team had to leave my room to go deliver another baby. I was already given a spinal. Well I filled up an entire pan of bile to which the nurse said was “impressive” by the time the team returned I could feel everything so more anesthesia was given. They did this multiple times because unfortunately I could continue to feel whatever they were using to sharply test my stomach. Finally while delivering the baby all of a sudden I lost the ability to swallow. I couldn’t clear my throat of my own saliva that was gathering in my mouth I told the doctor to which she explained it was only a sensation. I said no it’s not. The next thing I remember my husband told me I broke out of the restraints my arms were in and I almost contaminated the surgical area and the doc was in mid pull of delivering my son. My husband said she exclaimed put her out! I don’t remember what he said happened after that. My second experience during child birth the anesthesiologist saved my life. I had another c section. While in my room they couldn’t figure out why my pulse ox wasn’t reading normal and I wasn’t experiencing any symptoms. Well they tried a couple of different monitors and they all read the same. Well I soon passed a clot and went into hemorrhagic shock. She was the first person to be in my room during a code. I still remember hearing her voice when I was going in and out of consciousness. At one time I thought I was answering her and my husband when they kept calling my name but both expressed to me that I wasn’t saying a word. I can still recall in my head hearing their voices and me responding, “yes why do you keep calling my name I’m answering you” but I don’t remember being in any distress or being aware that she was actually trying to save my life. I could not understand why they couldn’t hear me responding to them. Anyway, have two healthy boys. Anyway, thanks to an anesthesiologist who was there transfused me through my big toe as all of the rest of my veins were all collapsed she also prepared me for the hysterectomy that needed to be done to save my life as well. Anyway again, lol. You are appreciated for what you do more people should know the vital role you play and you are doing an amazing job at sharing your love for your job and your patients. God bless you and know that you are making a difference. Btw, she was the first face I saw when I was out of the ICU telling me she was so happy to see me ❤️❤️❤️

The hospital where I had surgery gave me something that knocked me out in surgery prep before they took me to the operating room. I think this is a much better practice than taking someone awake into the surgical room. Much less scary!!!

I applaud you for your brilliance, sensitivity, candor, enthusiasm, and dedication that you display. Thank you, thank you, thank you! Additionally, a couple questions pertinent to your video... When you asked what surgeries require the highest dosages of anesthetics... I recall the answers given but wonder if perhaps surgeries that disallow complete sedation (don't know if that's the correct terminology to use to refer to the idea of not being put "fully under"). For example, when I was 18 I underwent a second trimester D & C which I was not put fully under for (which is bizarre considering a traditional termination within the first trimester can and has utilized full anesthesia). Additionally, I understand the vast dosaging difference between what is used in an OR and what has been shown to be available illicitly on the streets. What I didn't feel was addressed, was how what is available on the streets (while available in mass quantities,) how does the potency and quality differ which could mitigate some of the discrepancy between the surgical dosages and street dosages? Or in other words, if the quality on the streets is so vastly less potent than that which is available surgically, a person would need to use more of the weaker product to achieve a similar result than that which is medically available? To what extent is the availability and quantity available to the layperson terrifying to you as a physician AND specially an anesthesiologist?

Hi we really appreciated your input after a long day after your list ! i know being an RN , as we have discussed following my spinal scoliosis surgery which lasted over 10 hours , but after Ketamine i had a normal recovery yes three days before i actually woke from my drug induced slumbers ! but perfectly ok then i had a PCA , which was turn on , after being sedated i was able to dose myself . and then i was moved onto a ward and then i was able to get moving .and the joy of finally putting my feet to the floor and trying to walk if a little awkward but yes when i eventually found where my feet were going it was an adventure ! if only i could meet you but sadly living in the UK its not possible , but just pass on to your patients with confidence your able to overcome anything ! my love to all liz x

Fentyal nearly killed me. I had surgery snd they gave me 2 doses and 3 doses of Dilodid. I did not wake up. BP went down to 30mmhg. They had to give me narcan to wake me up and then rushed me to ICU for 4 days. It was awful. Then I had severe pain and all they gave me was Tylenol.

I went into surgery for weight loss over 12 years ago, went into surgery at 7:30am, woke up tied to hospital bed in ICU at 8:30PM, on the ventilator, tried to move when I came to, couldn't and totally freaked out, Dr came in and put me right back out, woke up again 11:30, wondering WTF.... I was never told what happened and still to this day, I don't, however, I've been absolutely TERRORFIED of being put under again, so now when I go in for surgeries, I'm calm UNTIL the anesthesiologist comes to talk to me right before we go to the OR, I go into a full blown panic attack that triggers charlie horses and a crying fit, I start shaking so bad I get seriously sick to my stomach, then when I get into the OR, it triggers a even bigger panic attack and the fight or flight kicks in, that only happened once, but the "sand man" scares the living crap out of me

Hi mate yes that’s very interesting as a anxiety sufferer I found that very interesting about the need for higher dosage if one was used to a certain drug with me I use small amounts of valliuam each month . I am lucky in the sense I’ve only ever had one day procedure in my life for colonoscopy , it’s important I take care of myself anything to avoid an operation , nice video thanks

Hey doc I am here in CA Joshua tree area. Going to be getting like my 12th surgery I guessed 5ml on the bottle of fent. I'm going to be getting a full knee replacement but I am not the least freaked out. My first three surgeries I was 9, 12, 18 and the first one I had a bad reaction to ketamine the second one went fine but was still freaked out from the first one then the third one cause I was all by myself. Now I'm chill and just go in really ready for a great nap. I enjoy your video's and your overall demeanor stay safe and keep up the great work doc.

I told my anesthesiologist that I used to be on pain medications in the past and he almost killed me overdosing on anesthesia and causing hypoxia which affected brain and caused MINS...I will never agree to a surgery again even if my life will depend on it

I was a paramedic at the “Knife and Gun Club” Denver General Hospital. We occasionally had anesthesiology residents ride for experience controlling airways and intubating patients in non-hospital settings like home, in vehicles, streets, etc. unfortunately many of the residents didn’t make the program due to their own substance abuse issues. I remember a time magazine article back in the mid 80’s describing the issue. I recall that many of the Drs chose anesthesiology for access to narcotics. I hope it’s not an issue anymore. Anyone know? Great channel. Thank you

Another thing Doc I remember when my Dad had cancer and they told me he wasn't gonna live threw the night one day and we decided to give him something to keep him comfortable and gave us a button to use and said use as much as we want. Lastly the nurses came in with 2 needles with red tops maby 15 minutes before he passed. Do they usually administer fentanyl to patients that are dying from cancer? I was shot in the head and face almost 20 years ago. I was in the hospital for a month. They finally got tired or felt bad because I was begging to leave so I could take a shower. I had reconstructive surgery The pain was awful. So they switched me to fentanyl patches with breakthrough lortab. I had a big patch and small patch every three days. And the liquid lortab was added to my feeding tube a few times a day.

I was given a tiny bit of Fentanyl after I had a crainiotomy, fluid filled cyst that needed bursting. My pain disappeared afterwards, it was great but I needed to wait awhile before my next dosage.

Hi Doc - would like to thank you for your info was extremely helpful with my anxiety levels for my last surgery under general anaesthesia - am combative with mask so was given sedative and a tube to suck which I was in control of. Still the thought of another human having to breath for me will always cause me to become anxious.

I was given Propofal weekly when I would go for pain injections. After a while I stopped getting those injections because I liked and got used to the Propofal and the pain medication was becoming not effective.

The nurse gave me Fentanyl after my hysterectomy. I remember the nurse telling me to breathe, apparently I stopped breathing and they had to put oxygen on me. After that I have had two additional surgeries and made it perfectly clear I didn't want it!!

Hi, me again! (Sorry, im bombarding you with questions- I JUST discovered your channel today!) I come from the veterinary medicine side of medicine so all the similarities and difference are incredibly fascinating! I would like to ask you, someone asked a question "does everyone who's going under general anesthesia get a catheter?" To which, you responded with a resounding "Noo!" In vet med, pretty much everyone going under anesthesia will be getting an IV catheter for better control of drug administration and faster absorption. Especially important in an emergency situation. What situations in human med general anesthesia is a catheter not 100% necessary?

So question.. I had a tube down my throat to clean my stomach out cause my pancreas was messed up I looked 5 months pregnant. How come they couldn't knock me out like they do the camera or any other tube? And I'm having a rotation cuff surgery on thurday so whats the best way to ask for meds if I'm allergic to codeine morphine?

I have been under anesthesia over 10 times. Once when I was a kid I remember them injecting the medicine and I went to sleep and woke up with the anesthesiologist holding a mask over my mouth and I triying to get his hands off my face. . It felt like I couldn't breathe. Have you ever seen that happen. It's something I will never forget. Of course I only remember it going on for maybe 10 seconds if that and I was back out . I think I heard him tell someone to give me more of something but , I'm not sure .

Dr Kaveh Why is it wing coming out of cardiac surgery they keep you longer sedated and it takes forever for the anesthetic to wear off before they can remove the Intubation tube when I had my quadruple bypass I was done with surgery I think around 4 PM and they didn’t take out the intubation tube at approximately eight or 8:30 PM and what was weird I could hear first I couldn’t open my eyes and I couldn’t move and then I couldn’t move my neck and then start moving my toes and I just slowly got right my extremities back why is that and why do they keep you asleep longer sorry for being such a long question but thank you again Dr. for your service I’m ever so grateful for all you do for your patience

Do you give redheads more anesthesia? I'm a redhead with severe sleep apnea. I had an anesthesiologist seem to be "not thrilled" with these 2 factors when giving me anesthesia for a pain block.

next month I"m having Hemi-Colectomy, should I ask surgeon for a Epidermal (Fetanel) I do not use street drugs, (recent 2 SMA surgeries in the last 1 1/2 years ago)

I have anxiety and I have built a tolerance. My Dr just told me that last Month. But I do end up having severe nausea 😵...I wish you could come to San Antonio, Texas and be with me for my upcoming surgery 😢 I was given clonazepam b4 surgery in the past.

One appears to come around from anaesthesia in the recovery room just like that being from out to wide awake. But is it likely one has been having interactions with clinicians but simply can't recall it? Therefore, it only appears that you come around in a flash?

I have had surgeries from the age of 6 to 56. Many orthopedic. I've never been afraid of surgery or anesthesia until this year 2022. I had surgery in January and I didn't wake up for 7 hours and I didn't know where I was. My blood pressure dropped and it was very scary. I had surgery again in February and I woke up during surgery when they were sewing me up. That hurt quite a bit and I have never woken up during surgery. I may be looking at more surgery in the next month or two and now I am scared. I don't know what to expect and I have lost my trust. They told me the only thing they could think of that would make me not wake up and not know where I was was Versed. I even made it through the Ether years and that was nasty stuff. Any thoughts?

I wont get needed surgery now as last time they left me in horrible pain and told me we don't give pain medicine when I asked for relief

I have no treatment & no pain management. The VA just started handing out narcan inhalers. I though about visiting the street pharmacist a thousand times, but it's just too risky.

About 5 years ago, I could not take my back/neck pain, (I had just had an MRI, and awaiting appointment with Neurosurgeon), but the pain had me barely walking and I was crying. The ER doc knew I was hurting, as I was laying over the er bed and he knew about my MRI, he explained that he was going to give me a shot of Fentynol. I said to him, (in a sweet, nice voice because that's who I am, even in pain) " are you sure that can be used, I heard that was bad stuff" ( I was thinking of the street crap, which idk why I even went there😂) oh my gosh, I think I was pain free for almost 24 hours....It was so nice.....HOWEVER, I would NEVER seek out this crap to alleviate my pain.

What If a patient has a fentanyl patch they use everyday do you increase the amount during surgery especially if they cannot take delauded or other drugs? It seems like to keep to have enough to keep under then my bp drops and that’s why I’m always afraid of waking up including my body fights pain meds and light anesthesia. I was awake during an endoscopy and my dr was aware and decided to continue because my jaw was locked open and every time I raised my head a nurse slapped it down 3 times and the doctor tried to force the tube down my esophagus and it hit a barrier and it was extremely painful and I remembered everything and wrote it down and reported them to their head doctor and important dr chewed them out and they let me know they had taken care of the problem. He was lucky he didn’t get close enough for me to give him a reason why he couldn’t have children!

Why would u get water in ur lungs after anesthesia that u vomit out?

I was scared to get it for my surgery but I was really hard to put a spinal/epidural in for my C-section so they used fentanyl with something else I forget and it made me not feel it even though they had to try multiple times and it made me not care like almost out of body I couldn’t feel my body which was a good thing at that time.

I've been suffering from a severe congenital / injury spinal issue since age 26. I've been on Vicodin and Morphine almost continously. And drug tolerance has set in. That, along with pressures placed upon pain management doctors resulted in a total elimination of it Resulting in severe mobility issues and pain. About a year ago when getting out of bed I fell and was laid there 10 hours. At ER the nurse injected something in my IV and my pain completely disappeared. First time in 38 years. I asked her what was that....she said Fentanyl. After being in the hospital for 2 weeks we had finally developed a program with combination of Vicodin and Tramadol. However my pain management doctor will not approve this. And neither will my new doctor ( who replaced my prior retired doctor ) I'm barely living at home with Home Delivered food. Not driving because of my disease. How does someone with well diagnosed spine disease live with the restrictions being placed on General Practice Doctors outside of a institutional setting. I dont believe I'll be walking within a year in my condition being left untreated.

I am 53 years old, and I have had failed spine operations, and I suffer from severe pain. I have been using fentanyl 100 micrograms every 3 days for 10 years, and I am still continuing under the supervision of a pain doctor. Is it normal or is there less fentanyl, because I have tried several things, including a pain reliever, and it did not work. It works, the pain doctor says you are the only patient I give fentanyl to, is there a risk😊

I am on morphine but have been lowering the dose. I am on 45 mg long release. I was prescribed it every 8 hours but I am cutting it once daily. Can I get off it and on something else that will properly control my pain I find that my pain is really bad if I don’t use it.
