Welcome to another Grey's Anatomy Doctor Reacts video. Many of you have left comments that you are enjoying my doctor reaction videos and medical tv show reviews and have found the medical education in them helpful. I am so glad that I was inspired to start this series by Doctor Mike.
For this episode, I did a medical drama review for an episode many of you requested: Season 15, Episode 21 The Good Shepard. This episode was perfect for a spine surgeon. This episode featured a patient who needed scoliosis surgery. I discuss the general treatments for scoliosis and how surgery for scoliosis is done. Amelia Shepard and her cosurgeon Link perform scoliosis surgery and I give you my opinion on how accurate is Greys Anatomy.
Anyway, I hope you enjoy this episode of Real Doctor Reacts to GREY'S ANATOMY - Spine Surgery Episode

Really love your videos, came across them today and watched all the GA reviews.. sorry to leave this comment multiple times but I really want you to review these espiodes.... Please review the HIV episode where Miranda removes HIV strain from the HIV virus and gives it to the "bubble boy" to help build his immune system. Season 10, episodes 22-23. 22 is the episode she gives him the deactivated HIV virus and 23 is the repercussions of her doing that. Also same season episode 18 shows a family that has three children have cardiomyopathy and they appear until episode 23. My questions: can what Miranda did really happen? Regarding the McNell Family, can that also really happen? I'm really interested in seeing your review (please, please, please), I've been watching GA since episode 1 first aired.

I had back surgery 20 years ago (I was 19), for scoliosis. My curvature was pretty bad, so a brace wasn’t an option. I have rods, and I’m not sure if they still do it this way, but they took a rib out too to fuse with my spine and the rods. I am happy to say that after 4 months of healing and PT, I’ve never had problems again with my back. 2 pregnancies happened, and never had problems. I was told by so many people not to have the surgery, but I’m so glad I did. My doctor said if I didn’t have it, I’d have been using a walker/wheelchair by the time I got into my 20s. Crazy what the body can do!

I also had this procedure, in 2019 (I was 13) I have rods and screws in my back. I’m not sure how they did my surgery, but I’ve watched many videos on the surgery’s and I’m glad I had mine. It’s a cool story to explain to people, only my close friends and family know because I hate people feeling bad for me.

It’s good to hear about positive outcomes. I had spinal fusion, of L2-L3 & L3-L4 in 1994, due to ruptured discs, and it worked great, until about 8-10 years ago, and which time I started getting back pain, minor at first and progressing to severe, so my surgeon removed the old instrumentation and fused L4-L5 & L5-S1, with instrumentation. Around 4 months recovering I started getting pain above the original fusion site and scans showed that a 2-3 discs above the old fusion had collapsed, so my surgeon ended up needing to fuse T9-L3. And again everything was going well, and I started getting sharp stabbing pains that circled around my entire rib cage, and scan showed I has developed, for lack of the education name, Spurs on the bottom edge of the vertebrae above this last fusion, so my surgeon had to extend the fusion up to T3, as the bone spurs were basically stabbing the nerve bundles whenever I moved. Now, those last three surgeries took place over about a 6 year period, ending a couple years ago, and while I started getting better, it wasn’t for long, as I was still bent forward considerably. My surgeon retired and referred me to a neurologist, due the my history of multiple fusions, and the neurologist finally figured out why I’m bent forward so much, from T3 to S1 I am fully instrumented, except for L3-L4, which was fused almost 30 years ago, but by being surrounded by instrumentation to stabilize my spine, that section has gradually sort of bent inward. The only solution they, or my second opinion gave me a a total revision of everything, such they will open me basically from just below my neck to my tailbone, emote all the hardware, cut notches in a couple of my vertebrae and physically force my spine into proper alignment, and then install all news hardware from T2 or T3 all the way down to S1, including doubling up on the rods, as I’m just over 6’8”. Well that idea didn’t thrill me, but it is what it is, but once they started talking about the extremely high complication rate for the surgery, plus the fact that it would require me to be under anesthesia for at least 12 hours, if there aren’t any surprises, my wife who is a surgical RN, although not in a hospital that performs this procedure, she got cold feet and wanted a second opinion. The second opinion said that my only real option is the surgery, but that he would suggest I put it off a year and try and loose some weight, as that would help improve the outcome, as well as result in less stress on the rods and thus make them less likely to break (both surgeons also had pointed out that I already had two breaks in the rods currently installed in my back.) I’m not one who is afraid of surgery, but this level of surgery is something new to me, as is the recovery. They said I will likely wake up in ICU and still be incubated, and they would remove that within a day, if my lungs cooperate, and I’d be in the hospital for at least a week, followed by 2 months or so in rehab facility. If I go ahead with this surgery, which my wife, son and in-laws all oppose, although maybe they will change their minds in 9 months, when my year is up, but only time will tell. It’s times like this I miss the old program called “The Operation,” which showed actual surgical procedures. I believe it was on The Learning Channel, but I might be wrong on that point, also they did an episode showing spinal fusion a couple weeks before my first fusion, and I found it extremely interesting, although my wife couldn’t watch it. 😀

You should react to the very recent episode in season 16 where Richard Webber is diagnosed after a long period of 'undiagnosable' illness. (I can't remember if it's the finale or a different episode number) You'll possibly be surprised when they reveal the answer and I only guessed it because I'd seen a relevant documentary on Netflix that popped the idea into my head. But it's the answer is surprisingly relevant to your specialty and none of the characters see it coming.

Yes, they do. It depends on the surgery and the surgeon though. If it's a small surgery, sometimes. But it's usually a short conversation, maybe like a joke or a short story

I’m a nursing student and some of my friends were lucky enough to score a theatre placement, so I asked them. They said it all depends on the surgeon, but all the surgeons they worked with were lovely and constantly made conversations about how their weekend went, what they’re plans for the following weekend are, asking why they chose nursing, some even let them choose the play lists for the surgery!

I've had a fused spine for almost five years now (unrelated to scoliosis even though I do have a mild case of it.). When they were bringing me in for surgery I asked if they would film it so I could watch later, they wouldn't, but now I kind of have an idea of HOW they did what they did!! This one was so good! There are a few more spinal operation ones on the show too..there's one where Derek goes semi nuts and draws a tumor on the bedroom wall (it's awesome), there's one of a little boy that Amelia works on after he was shot, and another scoliosis type one where Izzy is more involved too. I'm sure there's even more but those are the ones off the top of my head!

Hey Doc. So I was injured in 2011 where my C4 through C6 were slightly crushed caused bulging discs. It wasn't until 2022 where I met with a neurosurgeon who after looking multiple MRIs over the years had recommended surgery. What gave me the most confidence was when the Doctor asked what my goals were for him. I told him 1) Pain through my right arm to stop, 2) Tingling in my fingerings to leave and feeling to return. He told me there were so many reasons for pain and he couldn't guarantee it would stop and the feeling issues. What he could tell me is they wouldn't get worse which would continue to happen with my continued service in the Army. I loved he didn't try to BS me and spent a couple of appointmentments going through the options and the results. What I rarely see in these shows (that I would appreciate) is the time out where everyone stopped and all the people in the OR talked about what was being done and then I was put out. I think seeing it was a patients side and one of the 99% that doesn't have things go wrong is better than the drama shows where the crazy things happen. I would appreciate understanding why a neuro vs ortho surgeon did my surgery. Thanks for the time Doc. Oh and he removed the C5-C6 replacing it with a plastic type (sorry sure it is more technical term) disc and over the next year the bone regrew through the device. My outcome is great and appreciate my surgical team through the process.

This literally just happened to me. I had rods placed for kyphosis from T2-L6 seven years ago that had extreme complications and while hospitalized it took 6 surgeries to rectify. Well, here we are in 2020 and my hardware failed due to the curvature progressing rapidly. My hardware erupted through my skin due to hurdles From insurance and COVID-19 restrictions delaying appointments. It wasn't until it erupted through the skin was it considered emergent enough to be seen and operated on right away. Of course with my luck it was infected so I just got home from the hospital and am on antibiotics for 6 weeks. They did three operations total - they removed my hardware, left me open to later do one irrigation to clean out infection, and the next day they closed me up. Yeesh. It has been a nightmare and I'm glad to finally be recovering.

I’ve got a lot of issues that it would take forever to type it all out (not issues with the surgery) the surgeons were amazing and did an amazing job.

New subscriber here! I'm so far removed from being a doctor, but I would love to see you react to more Amelia episodes! She's Derek's sister and she is a kick ass neurosurgeon! Some good episodes for her are 11x14 and 14x04! I hope you end up reacting to those, I love what you've reviewed so far!!!

I had a spinal fusion down to L1 at 13 for a 34° scoliosis and a 90° kyphosis (I might have reversed those?) I had 2 ribs removed on the right side. Had to have all the hardware removed at 19 because I was starting to Bend further and one of the rods was starting to protrude at the top of my back. Today at 30 I still have restrictive lung disease.

It’s funny you mention operating the camera deals like a video game sometimes. I don’t remember the episode but at one point Mr. Dr. Shepherd is operating with a intern named Heather Brooks and she makes Pac-Man noises.

I’m not a big video game fan but my husband is..and he is sooo excited he finally got our 6 year old a little handheld Nintendo thing that’s loaded with a ton of like old classic games. And he loves it 😂 and he always convinces me cuz it’s good for his hand eye coordination.

an unique show to react to would be elementary, it's not a medical show but one of the main characters is a surgeon who was traumatized by losing a patient and quit medicine. I forget which exact episode it was but she returned to the hospital and had to deal with that trauma and noticed something wrong with the patient of one of her friends that could have caused the young girl to die on the table

Hey Doctor Dan, I have a question for you; after a very nasty car accident I had a doctor tell me I had something like trauma induced tortional scoliosis (I can't remember exactly what was said and have no idea how to spell that) but I was wondering; do you know what he was talking about? It was confusing and the suggested fix was something with a rod that had a high chance of causing paralysis... Is any of this something you could explain? If you don't want to I won't be mad 😁

Hey, Dr. Dan, Since the spine, and the spinal cord is part of the central nervous system, do you also perform procedures on the rest of the CNS (brain)?
@DoctorDan

No I focus strictly on the spine. My neurosurgeon colleagues operate on the brain and the spine.

@@DoctorDan Although you have very clearly stated that you don't do procedures in the brain, I've done some research (online, so, I can't attest to it's accuracy), but, I understand that most (not all, in my case) of ependymoma tumors occur in the ependyma of the spinal cord. What effects, or, presentations do you deal with in these specific cases, and, also, because, again, in my case, my ependymoma is recurrent, what advice do you have to avoid recurrence of these types of lesions?
