Dang it, this is spot on information...I WISH I would have found it before I filled mine out. I wasn't off the mark much, I kept in mind - time and lengths of activity and consistency ---but you have some excellent information that I could have kept in mind. Those little details that are important to explain things without talking in circles for umpteen paragraphs. HOW IS THIS VIDEO NOT BEING ADVERTISED IN EVERY FOOD PANTRY OR UTILITY ASSISTANCE BUILDING IN AMERICA? If you are in a pickle needing SSDI, it is more than likely those are the places you frequent! Your videos are the best! My favorite videos are of Peter & Judge Hyatt speaking of top 5 tips for hearings. They are so simpatico -- thank you

This is an excellent video and very thorough. I appreciate you being so detailed and providing an example narrative so that we understand how to fill out the form, but also why certain details are so important too in the process. I also appreciate hearing about your own point of view about the social security process, especially as a practicing lawyer in the field.

One critical thing is to get an attorney to review the form before you send it. Also keep copies of it printed and or scanned. If an attorney was so kind as to helping you with the form make sure you get a photocopy of it. Also I can't even begin to tell you have nice it is to be able to fill the form out on the computer. There are fillable PDF forms from many organizations and it can make filling the form out legibly a lot easier if you are like me and your handwriting is chicken scratch or at least not as good as you may prefer it to be. For one I never use cursive writing. It allows too much confusion as to what a specific word was meant to be.

There’s no time for that I got this one and 3 other forms in mail 4 days before due back date They gave me 10 days from date they sent it well mail takes 3-4 days so by the time you get it you have no time to do it it’s nuts!

My actual lawyer, 🚭 this guy.. he said if it's mental health it still effects you. Write how you feel on your worse day.

I filled out my function report to the best of my ability and scared to death that there's not enough room to put a lot of stuff on it took me 4 hours because I have bad hands and ect.

It is intimidating because they are trying to trick you into saying you can work. It maybe part of a CDR if someone tried to snitch you and now the SSA wants to see if there is any credibility at all.

Thank you for this video, my problem isn't pain except when I have muscle spasms but mine are balance issues, cognition issues, blurry vision at times and my fatigue is not consistent it is sporadic 😢

I, too, watched several videos, only started to watch some,and none were nearly as detailed as this. I appreciate the fact, Mr Evans, that you are not so afraid that you will lose a potential client because you help them fill out this form in such detail! Kudos to you!

I wish i could still ride a bike and go for long walks and do some gardening, i love planting flower's and such ,and swimming i love going swimming withthe family now i cant even do that either

There is also the similar 3380 BK that would be filled out by someone else. Would it still have to be completed if I have no friends or no family members left to do it? Also who would they send it to. Lets say if the form was intended for information about me would it go to me and I would have to get a family member to fill it out? Then about family members, what if the only ones left are the ones I don't like or communicate with anymore for any particular reason? Would I be able to request 3373 or even a hearing instead to communicate to them any abilities or disabilities? Or what if in theory there is one family member but they are always very busy and can not be bothered with additional paperwork. I suppose that I could consult an attorney if I need help with an alternative method of giving them what they need. All they are looking for is residual functionality. They are not trying to send you on a never ending quest for answers. One thing I did notice is that many if not all of the SSA's forms are PDFs you can fill out on a computer. If your handwriting is chicken scratch like mine you can opt not to fill the paper form they sent but fill out and print the PDF version of it. Just make sure it is the same form number. One thing I very highly recommend is making a photocopy of the forms before you send them in. You can print a second copy or at least scan them with document scanner. You can take a photo too if your phone is good enough, just check the photo to make sure it came out OK and not blurry or out of focus. This will help if they ever for some reason didn't get it. It also helps you review the form if you already sent it but fear you made a mistake. You can get good document scanners on eBay for less than $100. Just look and see what is there, I usually sort by lowest first and look through until I find one I like or search a specific model or product line. Try searching multiple brands and look first before buying to see if drivers are available for your OS. Some scanners will not work with modern Windows versions at all.

You said that if the side effect of medication are severe. What about if you go to a psychiatrist and they prescribe something and it has one or more side effects that make you feel worse? I have a learning disability and I am afraid that if I have to see a doctor they might prescribe something. Like if it makes me sleepy all the time or causes headaches or nausea would that be enough to win on that? If I had those problems I would very seriously have to visit the prescribing doctor and see if there is something else I can try or see if I can take a smaller dose. I would of course want it in writing that it has been modified so I can say that I am taking it as *professionally* prescribed.

OH! Thought of a question for you re: takings meds. I have been prescribed several meds, over several months, to help with both my neuropathy and depression (which thank God is not severe, but there). I will start the meds, and have side effects to the point of being so uncomfortable, I will not continue to take them. Please know, I did try the medication for a reasonable amount of time, not just a few days, before making the determination the medication wasn't helping enough, if at all, to continue experiencing the negative side effects. MY QUESTION: How might you handle the answer to medications (page 10) that you are not currently taking, but have taken and discontinued, due to side effects? (Prescribers informed, BTW) Second part to that is, how do you believe this is perceived by a judge? Thank you so much for your attention to my questions!

This doesn’t really work for my adult disabled child case for my son who recently turned 18 and has to reapply. I can’t seem to find videos for an intellectually impaired, ADHD, Autistic person. I was denied and just sent this paperwork again. I think it’s because my son doesn’t think he’s disabled and didn’t answer his like a disabled person. We did text to speech on the computer, with redirecting and extra time. Should I fill his out? I filled out the Adult function report third party.

The plural of diagnosis is diagnoses. It retains the four syllables, just that the short i at the end changes to a long e.

Thank you! If I was laid off due to anger burst, are they going to check with my employer if I provide the name of the empoyer? Because in my case it is documented as being laid off, it does not say there was anger burst, etc. please clarify!

Unfortunately, I don't have a clear yes or no answer for this. The answer is "maybe." SSA can check with your employer and have your employment records ordered. However, it is not common that I see SSA ordering employment records directly from an employer.

Lord it hurts me to lift a gallon of milk but sometimes i have to ,and to fo to the store i definitely have to have a motorized cart ,and if theres none i use a rolling cart which im hanging on to it because my legs buckle on me a lot and the arthritis kills my back and spondylitis omg im slummped over the basket to take the pressure off my hips and legs or else the pain shoots down both my legs 😢 i went from being able to climb and clean bleachers and gyms and classrooms and hallways, restrooms and walls,windows ect. Its a school so we cleaned everyday and many many rooms to clean,run buffing machines and stripping wax and rewaxing and so many detailed work duties and i cannot do añy of it, because the bad herniated discs and bulging disc's and slipped disc's along with cauda equina,arthritis, spondylitis ,spondyloses,yeah i cant squat anymore lord ill be on the floor and cant get up my legs are to weak, standing up to a few minutes to 10 minutes really hurts so much ,the burning, tingling,numbness,and peripheral neuropathy conditions, oh lord i cant put it all on here because there so much that affects me

Everyone I know on Disabled payments has two full time jobs, does that make them disabled? So i just was wondering what the qualifying status would take to get approofed?

Extremely difficult. We try not to convince people they are disabled. However, you have to balance that against telling the facts as they are.

This video this how-to video is so incredibly awesome, it is absolutely worth going through again (and even again!). If you have the ability to enlarge your screen at all, that would be very helpful, that is what I had to do. It's difficult to fit the part of the form that you're talking about to be able to see. Also, in regard to inability to hear clearly, if there is a way to adjust the volume, do that, of course, OR you may be able to do a closed caption option while watching the video, that would type out all the words for you at the bottom of the page. I had to pause and go back many times, because everything Mr Evans said was of value. Good luck to you!
