Hair restoration physicians use a tool called the "Norwood Hamilton Scale" to "grade" hair loss patients. The scale goes from a grade or class I level of loss, all the way to a class VII level of loss. These "Norwood I" patients have little to no loss, whereas a "Norwood VII" patient has the most loss possible for a patient with genetic male hair loss. This level of loss not only creates a completely slick appearance throughout the entire top of the scalp, but it also involves drastic changes around the safe donor area on the sides and back of the scalp. This level of loss leaves patients with only a confined "strip" of hair on the sides and back where grafts can be extracted from. But with so much bare scalp to cover and so little available donor area, is is possible for these patients to restore their hair? More specifically: can a patient with Norwood VII hair loss undergo a hair transplant?
To help answer this question, Dr Blake Bloxham and Dr Mondarian Parissis from Feller & Bloxham Medical (www.fellermedical.com) decided to sit down and have a discussion about Norwood VII patients and whether or not they can -- or should -- undergo hair transplant surgery.
Drs. Bloxham and Parissis describe Norwood VII loss and how common this pattern occurs; they also discuss the importance of recognizing this pattern, and different forms of this pattern, and the role it plays in evaluating a patient's donor area. They further discuss the role medications like finasteride (Propecia) and minoxidil (Rogaine) play in a Norwood VII patient's care, and what other options may be available besides hair transplant surgery. Finally, they discuss the surgical approach to treating a Norwood VII patient and share an example of a Norwood VII patient who sought to restore his hair with hair transplant surgery.

I actually started naming myself NorwoodReaper on every game and website that requires a login... The lore of Norwood Reaper is one of the funniesg and truest things I have come across while researching hairloss topic :D It's like an inside joke. You either know and you can relate or you have no idea 😀

Great amount of credit to Dr. Bloxham for taking on this case. I think the approach was spot on. The FUTs gave this patient a maximum amount of grafts over the three surgeries. I think the final frontal result looks great. However the overwhelming willingness to show the midscalp and crown prior to and not after the final procedure just doesn't make sense. Yes we know it's going to be very thin with some areas that weren't grafted. But for those of us in the know, those images are just as important as the frontal images. No Dr. Is going to loose a patient or credibility by showing the full result, I would actually argue the contrary.

I’ve always thought, if my hair gets this bad, I would just have the back/side trip of hair transplanted to the top and just rock a mid fade perpetually 🤷🏻‍♂️

Back in the old days -- 30 years ago when I first started looking at options for hair replacement -- some doctors would start off by doing a scalp reduction in the crown area, then follow-up with two or more FUT procedures. I wonder whether that was ever an option for this patient?

Scalp reductions are a thing of the past. Their long term results were catastrophic for the vast majority of those recipients. There were permanent nerve damage issues, something called stretch back which eventually would expose more balding area. Not to mention irreversible horrible scaring. I would imagine in special circumstances such as catastrophic injury cases they may still be used when no other option is available.

What does his crown look like? I don’t think people question the ability for a Norwood 7 to obtain a hairline, but what will the rest of the scalp look like.

How come you are now showing pictures of full back and side images? If you did this operation it is important to show the full head after the surgery and what supply is left in the back? I feel there are times a surgeon has to say no to a Norwood 7 even if the patients wants to have an operation.

Yes beard and chest hair can be used. In fact this patient would benefit for the crown being filled in with beard hair taken from under chin. The issues to consider is that the hair grows slower and shorter on chest, beard not so much. It is a different texture that straight hair from head - if head hair is normally straight.

@@donaldfritschi5132 they are usually too weak to be put on your head...what doctors do is put those weak hairs (from chest) either at your temples (where you naturally have weak/short hair) or in between other transplanted scalp hairs to give that aspect of density, or they put it in FUE/FUT scars (afaik). The chest hair never grows the same length...but we are lucky that beard hair somewhat resembles scalp hair...it can grow to similar length, but it's usually thicker so you can't use it for your hairline, but can be used in the crown/middle area, with the same technique - in between other transplanted hairs to camouflage it (since it still have a bit of a different texture).

Imagine Donald Pleasance with a hair transplant or a wig. He'd look ridiculous. We need to stop thinking of male hair loss as a disease and start accepting it as the natural maturation of the body. It's a signal of maturity (and fitness eg you've lived long enough to go bald, you haven't died of disease or in a fight with another male prior to that).

@@s.miller2648 there’s a area of the scalp that’s immune to hair loss. (DHT resistant). That area is harvested bc it will never fall out in the future. FUE in a large amount will have to take from non DHT areas or the donor area will be patchy. Those hairs could fall out permanently in the future. That’s why it’s recommended that you do FUT (the strip) first bc you can get more hairs from that safe area, and then do FUE later on to get more hairs without the scars being noticeable. Or you can just take Rogans/Finasteride etc for the rest of your life and do the FUE harvesting all over your scalp, but there could be side effects with your mood and stuff, so it’s a gamble.

@@Islanders83 Correct me if it wrong, but he has had 3 FUTs. The last FUT was to put some extra grafts in the crown area. He just showed the front. He should have shown the midscalp at least

​@juanbonilla1006 the crown likely does not look good hence why they didn't show it at a direct angle. The crown is notorious for requiring too many grafts to provide density and sufficient coverage. Even if they used his entire donor area it likely wouldn't be enough to sufficiently cover crown and midscalp.

@@maykz2127 i don't know for the patient, but transplanted hair don't prevent non-transplanted hair from falling. Stress on the scalp caused by procedure may even increase hair loss

@@taopaille-paille4992 the question was about whether the patient used minoxidil during post operation period or not. it is important thing to know when assessing the actual result, because minoxidil could provide a considerable growth. if so you’re not able to understand which growth from the hair transplant, in which growth from minoxidil

Maybe i fall under rare category 😂. I was 27-28, when I realised i am at NW 7.23-24 age i noticed hair thinning, i just started shaving my head. By the time i was looking for transplant option it was nw 7. I have extreme genetic disadvantage & even donor gets thinned out eventually & I accept that totally. I recently finished my HT of 2 sessions. 6000 grafts, moderate coverage. All good

@@wavyeee from completely bald to l thinning just started, i am happy with that looks. Satisfied with 1st HT, 2nd HT which is done on crown takes longer time to show results, 3.5 months & i can see coverage & lot more hairs yet to come.waiting for that.

​@@jocko1283 as someone that started balding in high school and by 24 I was a NW6, I can tell you it's horrible, not only for your self image, but for your confidence and the way you socialize, because you look way older than you feel, it's a mess. Now I'm 32 and it's more normal/socially acceptable for someone my age to be bald, and still NW6, I'm lucky to look good bald (also got SMP last year), but having hair is always best (with being in between being worst)...I'm thinking of having transplants since I also have thick beard and chest hair, but I'm kinda reluctant since I know I will scar my scalp and probably won't be able to have the look that I have now when clean shaved, although SMP could mask the scars, but sounds like a head ache. I would never downplay going bald, especially when that happens under 30yo, it takes its toll on your mental health, later in life it's more acceptable.

@@TudorSicaru I guess it is just how we look at things in am norwood 7 and it just does not bother me infact I actually kind of like it , but yes i can be thoughtfully enough to realise the social impact it has on alot of people and that it can be hard to deal with , i just refuse to let it ruin the short time I have on earth
