Welcome to another Doctor Reacts video! I promised you all PART TWO of my doctor reaction video to Grey's Anatomy Season 6 Episode 6. In this episode, we break down the steps to how exactly this patient deteriorated rapidly into multisystem organ failure. We also get to the conclusion of Dr. Webber's fact finding mission to see what was the cause of the patient's death. Who does Dr. Webber ultimately blame for the demise of this patient??? Who was responsible???

This patient actually follows April throughout her time as a doctor and makes her become Chief Resident and really just makes her a better doctor altogether. She is a stickler for rules and procedure because she knows what happens when you miss one step. They do a really great job in making sure that April never forgets this error and it affects her and makes not only her a better doctor but the hospital a better hospital. Edit: also can you please react to the elevator episode? The one where George has to do surgery on someone in an elevator.

I'm rewatching Grey's, and noticed that even in passing moments you can see the influence this experience has on April. In the episode where there's a Code Orange and then a fire, she is in charge of the patients being evacuated. She makes very sure to remind everyone to check on all the patients for smoke inhalation. Its a very passive moment and if you don't know her history, you'd miss it. But knowing about this episode and experience, you have to appreciate the writer's and their attention to detail in regards to that.

one thing about this episode that I thought was profound is that no one knew who's patient it was due to limited documentation. They recorded using initials, so information wasn't brought to the right person and kept being given to Alex Karev due to the "AK" initials on the documentation, even though April Kepner "AK" did the initial assessment. Just shows how important full documentation is especially in situations like this!

During this time while watching Grey’s Anatomy, I didn’t really care much about Dr. Kepner. I was even harsh about her although I kind of felt bad, but when I actually entered med school, I felt bad for her more. I imagined myself doing the same mistake she did. Lol. Hopefully I won’t ever do that in the future 🥺 Anyway, she turned out to be such a great surgeon when she became an attending. I miss her ♥️

I think you should react to a less famous episode of GA. One that we haven't seen yet. People asks for the bomb, the ferry boat accident, the shooting, plane crash etc, but maybe the best course of action is to do one never seen before. 🤔 Congrats on this video from Brazil 🇧🇷🙋🏻‍♀️

The one where they go to San Fran to take their Boards is packed full of medical scenarios as well. It would be another great one to cover..you could tell us if they actually answered the questions right lol!! These are so awesome! I like to study anatomy and medical terminology when I'm bored so I know what they are talking about when I watch medical shows. It makes it a lot of fun! Thanks for doing these! Can't wait to see more!!

I'm so glad you reacted to this episode because it's one of my favorites. The writers did a great job building tension in a way unique to a hospital setting, the imagery was great, and they used this incident for very effective characterization, especially for Dr. Kepner. They actually show the effect that this has on her for the rest of her time on the show, and without it her arc would be completely different. The show's experiments with style in season six and seven (this episode, the coffee mug episode, etc.) really helped to breathe new life into the show after so many beloved characters left and new ones were introduced.

@@kanya9846 I was referring to S10E06 "Map of You" (I completely forgot how far along it was in the series lol), which follows the coffee mug that Derek accidentally brings to work as it is passed between almost all of the main characters in a day :)

I recommend the Grey's spinoff Private Practice season 5 episode 22 "Gone Baby Gone" This episode has to do with organ donation with an anencephalic infant on the controversy of the mothers decision. I've always wondered while watching it how real doctors feel about it due to an active brain stem but no functioning brain. Loved the video by the way!

One of the main reasons kepner was fired for a slight mistake was due to chief webber needing to make budget cuts at the time because of the merger, chief webber usually isnt that strict or heartless 😅

My favorite part of this episode is everytime you see the patient she actually looks worse. First time watching I didn’t notice of course but once you know what’s going on, you’ll see she has the beginning stages of central cynosis the second time she gets her burns treated (after the inital exam). Each time you see her she is paler and bluer, dry, dyspneic. I love this episode because I don’t think greys in the past few years has been this detailed! Thank you for this video!!

I have a question... I know April missed looking into the patient's throat but soon after that practically everyone was looking inside her throat while they were trying to intubate her... Shouldn't they have seen something?

What I always find funny in these kind of series is that the rooms are super small but completely filled with doctors. At some point you get to the point of diminishing returns. The point where there are so many people in the room that no one really can do anything. But on these series they don’t seem to have this problem, in those extremely small rooms.

I think a critical part of this episode that can be overlooked, especially for someone not following the larger story, that is the chaos happened because two hospitals merged that had different doctors fighting to keep their jobs. So the biggest issue is there was no proper agreed system which led to all this. Of course in real life everything is standardised, but in terms on this story it is why there was so much fighting and doubt. Webber asked "you don't know how to intubate?" because that was an unknown doctor to him.

7:22 anesthesiologist here, just a slight correction as it’s a common misconception even among doctors, good peripheral IV lines can deliver fluids faster than a central line - 16G (gray) gives 180 mL/min, and 14G (orange) gives 240 mL/min, whereas a CVC gives much lower flow rates, an 18G triple lumen central line gives around 30 mL/min (6 times less than a gray cannula), so it’s good to remember that a peripheral cannula of size 18G or greater is preferable to infusion by central line for rapid fluid delivery, and I believe in case of there being such a hypovolemic shock that a peripheral line can’t be obtained, intraosseal approach (basically sticking a needle into the shin bone) is preferable to a central line. So that might be a slight oversight on the show’s part.

60% of my life I've spent in a hospital and so many times I've seen how unrealistic people are. Doctors are not Gods they are humans just like everyone else. When deciding on school someone picked art and someone picked being a doctor but that doesn't make them all knowing, gods, or superman.... They are still human, they get tired, they don't know everything because it's impossible to know everything about everything in medicine, they have bad days and as every person in this world they make mistakes. I still remember one of my doctors , years ago when we still didn't know what was the problem and we were trying a lot of different and experimental things, after a failed treatment (20th in line) he came to my room and told me that it didn't work and I told his Okay, thank you for trying. First and last time in my life I've seen him crying. I ask way and he told me that in all tries we've done I was never mad at him or disappointed or what ever. I honestly did not understand at the time why would someone do that because it's easy for me and I told him: " I don't expect you to heal me but I do expect you to try everything you know to do so and if it works great and if it doesn't work well we've tried everything." I'm pretty sure she was fired because she was about to check her throat, then got distracted, and signed off anyway. Also, you said that she would have other symptoms, but the whole episode she got like 2 seconds with each doctor, and they didn't communicate when it came to her. So I could definitely see how it happened. I have a little medical training, because I work with the mentally disabled. Even we keep a log book for each client, and write every little thing in there to inform other staff members, and we have to sign it each shift, so it shows we've read it. I love learning new things about medicine, and you actually break down every little thing. Like I said, I'm medically trained, but just for the basics, and every "annual med training" is the same exact thing. Tests and all. Not a single new thing is taught. I swear I can name the 6 routes, and all the abbreviations for prescriptions in my sleep. Lol. Edit: I also have to add my favorite question on the tests. (And I'm 100% serious. This is really on there) True of False: Too soothe a 2nd or 3rd degree burn, add margarine or butter to the area...... WHAT!?

2:30 Are there different scenarios in which you would use a tracheotomy than a crichothyroidotomy? Is there a reason they chose the latter here? Usually in medical dramas I see the prior.

A cricothyroidotomy is usually used in very acute situations, particularly when injury involves the head or neck. It’s typically easier to achieve than a tracheotomy, ( because the location of the trachea in relation to the vertebrae means having to position the patient to get access, which isn’t always possible) and therefore can be performed quicker to get the airway secure. Hope that makes sense!
